The claimed useful model relates to medicine, in particular to dentistry, and is intended for 2 prevention of caries of permanent teeth in children of preschool and primary school age with chronic diseases of the digestive organs burdened by parasitic infestation.
71.Prevention of dental caries in children is one of the most urgent problems of modern pediatric dentistry, which is due to the high prevalence and significant intensity of dental caries. According to the literature, the prevalence of dental caries among the children's population of Ukraine reaches 63.3-98.3905. According to modern 70 concepts, the development of the carious process occurs as a result of the complex interaction of local and general factors, which are realized in the microorganism-saliva-structure of enamel system. An important role in the pathogenesis of carious lesions is played by general somatic diseases, in particular chronic diseases of the digestive organs, which in recent years have a tendency to a progressive increase in chronic forms, primarily in periods of 6-7, 9-10 years 71.
При порушеннях імунного статусу дитини навіть при активній хіміотерапії не наступає ефект і швидко наступає повторне зараження (ентеробіоз, лямбліоз).The prevalence and intensity of dental caries in children with chronic diseases of the digestive organs is higher 12 than in practically healthy children |1, 2, 3). The results of many studies show that in children with chronic diseases of the digestive organs, the ineffectiveness of dental caries prevention is due to undiagnosed parasitic diseases and failure to take into account the child's immune status at the time of dental caries prevention. In recent years, there has been a tendency to increase the incidence of some helminthiasis, primarily nematodes (enterobiosis and ascariasis) (4, 5, 6). Parasitic diseases are often the last in the chain of differential diagnostic thinking of a doctor, although many of them (giardiasis, intestinal helminths) can be the cause of pathology of the organs of the gastrointestinal tract and a decrease in systemic immunity. In case of violations of the child's immune status, even with active chemotherapy, there is no effect and re-infection quickly occurs (enterobiosis, giardiasis).
«-Diseases and changes in the functional state of organs and systems of the body, a decrease in the body's reactivity is mediated by the main pathogenetic local factors in the development of dental diseases - the resistance of solid tissues), the composition and properties of the oral fluid, in particular the state of its protective mechanisms. Disorders in the digestive system during the period of development, formation and maturation of teeth can significantly affect the composition and structure of dental tissues and, accordingly, can weaken their resistance to caries, and also actively affect the occurrence and course of the carious process due to changes in the composition and properties of saliva . In view of the above, it becomes obvious to M that the preventive direction in modern children's dentistry needs priority development. "-
The given data indicate that the existing methods of caries prevention in children cannot be considered sufficiently effective, because they do not fully take into account such determining cariogenic factors as the individual state of immunity of each child at the time of caries prevention. Ge")
The closest analogue (prototype) of the proposed useful model is the method of prevention of the main dental diseases, which includes: training in rational hygienic care of the oral cavity, remineralization of hard tissues of permanent teeth, sealing of intact fissures of permanent molars and premolars, aerosol inhalations 0.195 Levamisole solution to correct the secretory immunity of the oral cavity (10-14 sessions). This method is based on the detected decrease in the content of secretory immunoglobulin A in "dental caries in children with diseases of the gastrointestinal tract. Local use of levamisole led to a 50% correction of this indicator, which had a positive effect on the condition of the teeth of this contingent of children. However, this method has certain disadvantages. Yes, it does not take into account the state of systemic immunity, which is in the majority
Most of such children's teeth are disturbed, which is also reflected in the condition of the hard tissues of the teeth. In addition, this method involves the use of a unified prevention scheme for all children, while an individualized impact on the body's defense systems is more effective, taking into account their condition in each specific child.
The task solved by the proposed useful model is to create an effective way to prevent caries of permanent teeth in children with chronic diseases of the digestive organs due to the effect on the (se) immune status of the child at the time of prevention using an individually selected immunotropic drug. and The technical result achieved by the useful model consists in increasing the effectiveness of prevention - to 70 of dental caries in children with chronic diseases of the digestive organs against the background of improving the state of local immunity.

чIn children with diseases of the digestive system, a decrease in the absolute and relative number of T-lymphocytes in M was found compared to the same indicators in the control group, as well as relative to age norms. In children up to 6 years of age, there was a decrease in the number of SOZ-lymphocytes to 51.99-1.5090 and 1.25--0.32x109/l, a decrease in the number of CO4 helpers to 33.36--1.20965 and 0.77 --0.08х109/l, as well as suppressors up to 18.63--1.4295 and 0.58--0.05хХ10905. h
The average value of the immunoregulatory index remained within the normal range. In children under 10 years of age, there was a decrease in the number of SOC lymphocytes to 33.84 42.58906 and 1.08-0.21х1095, a decrease in СО4ж- lymphocytes to 18.24-1.8195 and 0.38-0.04Х1096, a decrease in СО8ж - suppressors up to 19.48 -1.51956 and 0.97-0.09X1095. A decrease in the immunoregulatory index to 1.14 -0.13 was noted. In healthy children, the immunoregulatory index corresponded to normal values in all cases, in children with diseases of the gastrointestinal tract and with healthy teeth - in 7095 cases, and in the presence of caries in sick children - only in 2595 cases. The level of CO22-z" lymphocytes, as well as the amount of immunoglobulin A, were the lowest in children with caries against the background of diseases of the digestive organs - respectively, 27.96--2.7095 and 0.644-0.09 x1095 and 1.08g/l, increasing to normal values in children with diseases of the digestive organs without caries and increased even more in healthy children. The phagocytic number in 45 children with diseases of the digestive organs exceeded the normal values - 8.92, which can be explained by the activity of compensatory mechanisms of the phagocytic system against the background of a decrease in the activity of the cellular link of immunity. (Se) The number of circulating immune complexes was the maximum - 65.5 and exceeded the normal values in children with diseases of the digestive organs and dental caries. There was a decrease in phagocytic activity and salivary neutrophils according to the level of the phagocytic index to 42.3-4.1956 and the phagocytic number to 3.2, a decrease - 70 in the concentration and ratio of IDSS and IDA to 4.85 --0.35g/l and 0.58 --0.5 g/l. GT" was 0.38-0.03 g/l. Thus, changes in indicators of systemic immunity observed in children with diseases of the digestive organs are more pronounced in teeth examined with caries against the background of diseases of the gastrointestinal tract. Tooth decay in children with diseases of the digestive system can be a marker of deeper changes in the functioning of the immune system. Children with chronic diseases of the digestive organs were prescribed the proposed method of preventing dental caries. c The effectiveness of the proposed complex was evaluated according to indicators of caries growth reduction (after 24 months from the start of its use), as well as on the basis of normalization of indicators of systemic and local immunity. It should be noted that after 24 months, the reduction in the increase in caries was 7295. There was an improvement in systemic immunity in children due to an increase in the number of SOYUZ lymphocytes to 61.30--0.9895, an increase in the number of CO4 lymphocytes to 44.20--0.5595 growth CO8 suppressors to values that approached the data of healthy children.
The phagocytic activity of neutrophils increased - the phagocytic index increased to 64.95-1.4495. There was a decrease in the concentration of circulating immune complexes to normal age indicators. The concentration of Id A increased to 2.12-0.11 g/l. There was an improvement in local immunity due to an increase in the phagocytic b5 activity of salivary neutrophils at the level of the phagocytic index to 59.2-4.895 and the phagocytic number to 4.9, and the normalization of the number and ratio of Id b and A in saliva after prophylaxis to 3.5:1:0, 35 and 0.69 -0.04 g/l.
The concentration of secretory Id A increased to 0.69--0.05 g/l.
