The useful model refers to the field of medicine, in particular pediatrics, neurology, rehabilitation and can be applied in medical practice to treat children with epilepsy, reduce their disability and social maladjustment.
Епілепсія в структурі захворювань нервової системи у дітей займає друге місце після дитячого церебрального паралічу (ДЦП).Epilepsy in the structure of diseases of the nervous system in children takes second place after cerebral palsy (CP).
Epileptic seizures in children occur as a result of various etiological factors: pre-perinatal lesions of the central nervous system (CNS) (including congenital malformations of the brain, hypoxic-ischemic and traumatic lesions of the CNS), traumatic, infectious and toxic-allergic, hereditary, which lead to disability and social maladjustment of the child.
Taking into account the etiopathogenetic factors of the development of epilepsy in children, the basis of treatment is anticonvulsant drugs. Over the last decade, a number of quite effective 72 anticonvulsants have been proposed in the world, but their action is aimed at one pathogenetic mechanism of epileptic discharge in the brain: GABA-ergic system, sodium or calcium channels, glutamate level. Also, the action of standard anticonvulsants is aimed at a certain nature of attacks.
There are a sufficient number of proposed methods of treating epilepsy in children, which are based on the use of standard anticonvulsants for various forms of seizures.
Thus, in the treatment of minor forms of epilepsy, the use of ethosuccinide and bromisoval is suggested - Ipatent
України 61135 А, ЗАСІБ ДЛЯ ЛІКУВАННЯ МАЛИХ ФОРМ ЕПІЛЕПСІЇ), при лікуванні парціальних та генералізованих форм епілепсії - дифенін та клоназепам - (патент України 59664А СПОСІБ ЛІКУВАННЯof Ukraine 61135 A, MEDICINE FOR THE TREATMENT OF SMALL FORMS OF EPILEPSY), in the treatment of partial and generalized forms of epilepsy - difenin and clonazepam - (patent of Ukraine 59664A METHOD OF TREATMENT
EPILEPSY PATIENTS). However, the developed methods are based on the treatment of certain types of attacks, and in preschool children their polymorphism prevails, which requires the appointment of several drugs at the same time. 29 The closest in terms of technical essence to the method proposed by us is the method of complex treatment of pt) epilepsy in adolescent children, which includes the appointment of anticonvulsants, course intake of vitamins, activators of brain metabolism, antioxidants and vasoactive drugs, in addition, the use of melatonin before bedtime -- (patent of Ukraine 56053A METHOD OF TREATMENT OF EPILEPSY.
оHowever, the method developed for the treatment of children of adolescent age includes standard anticonvulsant therapy, effective on certain types of seizures and does not take into account the peculiarities of the course of the disease in children of preschool age "f - polymorphism of seizures, the presence of organic and functional changes of the central nervous system, mainly of pre-perinatal origin. at
The basis of a useful model is the task of creating a method of treating epilepsy in children of preschool age, which is solved with the help of basic metabolic therapy and a broad-spectrum anticonvulsant, which will ensure the cessation or reduction of the frequency of attacks, the improvement of the psychomotor and language development of the child, (57 the reduction of psychoemotional disorders, attention deficit hyperactivity disorder, which will contribute to the reduction of children's disability and social maladjustment.
The task of treatment of epilepsy in preschool children is solved by applying "basic metabolic therapy, according to a useful model, an anticonvulsant with a wide spectrum of action - Topamax is additionally prescribed in an individual, well-founded treatment scheme. c In the Department of Child Psychoneurology of the Institute of PAH of the Medical Academy of Ukraine, a method of treatment has been developed and tested
From" epilepsy in preschool children with the use of a modern anticonvulsant drug of a wide spectrum of pathogenetic action - Topamax.
The cause-and-effect relationship is due to the fact that the main mechanisms of action of anticonvulsant drugs 49 are: influence on ion channels (calcium or sodium) of the neuron membrane, increasing the activity of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA); blockade of the activity of the excitatory neurotransmitter (Te) glutamate. Each of the standard anticonvulsant drugs has a unidirectional pathogenetic effect and affects a certain type of seizures. i-th The only drug acting on all chains of the pathogenetic basis of the occurrence of paroxysmal epileptic "surge" 20 activity in the brain, which covers all the mechanisms of anticonvulsant action, at the present time, is Topamax (developed by the Ddapzzep-Sniad company). Important in the action of Topamax is the absence of a decrease of cognitive functioning and minimal side effects. The use of the anticonvulsant drug Topamax will allow to influence all types of seizures in children (generalized, partial and secondary-generalized, mixed) and thus, thanks to their disappearance or reduction in frequency, contribute to the child's further psychomotor and language development, social adaptation, and reduction of disability. c Pharmacological action: "Topamax" - the effectiveness of the drug is due to the effect on ion channels (sodium- and voltage-dependent calcium), correction of the disturbed balance in the inhibitory GABA-ergic and excitability of the glutamatergic system by strengthening the first and reducing the activity of the second. These mechanisms were confirmed by the results of 60 direct studies of the excitability of the human motor cortex, which revealed its decrease. As for the psychotropic effect, topiramate exerts a normotonic effect. The drug is well absorbed, almost completely excreted unchanged, has a half-life of 2 hours, slightly binds to blood proteins. Equally important is the linearity of pharmacokinetics, that is, the direct dependence of the concentration of the drug in the blood plasma on its dose. All this determines the absence of hepatotoxicity, the possibility of taking it once or twice a day, the lack of influence on the therapeutic activity of other antiepileptic drugs. "Pantogam" (calcium hopanthenate) - hopanthenic acid, combines stimulating activity in relation to various manifestations of cerebral insufficiency of exogenous-organic genesis with anticonvulsant properties, helps to reduce motor excitability with simultaneous regulation of behavior and activating effect on mental activity. The drug penetrates
Through the blood-brain barrier, it is excreted unchanged. It is especially effective for mixed and minor epileptic seizures. "Actovegin" is a drug that activates metabolism in tissues, improves trophicity and stimulates the regeneration process. The active substance is a deproteinized hemoderivative from calf blood with low-molecular peptides and nucleic acid derivatives. Actovegin activates cellular metabolism 7/0 by increasing the transport and accumulation of glucose and oxygen, increasing intracellular utilization. These processes lead to an increase in ATP metabolism and an increase in the energy resources of the cell. Under conditions that limit the normal functions of energy metabolism (hypoxia, insufficient substrate), actovegin stimulates the energy processes of functional metabolism. A secondary effect is increased blood supply. "Neurovitan" is an optimally balanced complex preparation consisting of vitamins of group B for oral use, the effectiveness of which is due to the synergistic effect of vitamins. The advantages of the drug include the unique action of octothiamine, which is contained in Neurovitan and is a complex of thiamine and lipoic (thioctic) acid - TATP, has a prolonged property, higher acid resistance, absorbability and effectiveness compared to thiamine hydrochloride. Thanks to the rational combination of 2 components of Neurovitan, vitamins that mutually complement each other, the most pronounced therapeutic effect is revealed. Each vitamin is in a separate starch microcapsule, each capsule has a different diameter, so vitamins are absorbed in the small intestine by active transport sequentially and gradually, depending on their size. This prevents unwanted interaction of vitamins of group B with each other (as you know, vitamin B destroys vitamin B and vitamin B potentiates the allergic properties of vitamin B).
The mechanism of action of group B vitamins in the nervous system is based mainly on the potentiation of the action of acetylcholine, gamma-aminobutyric acid and serotonin.
The criterion of the clinical effectiveness of anticonvulsant and metabolic therapy was: cessation or reduction of the frequency of attacks (percentage assessment: 096, "5090, 50905 and 10095), improvement of psychomotor and language development, psychoemotional state, reduction of manifestations of hyperactivity syndrome, attention deficit (095, 25905, 5090 and 100905) in 35 s from Children with epilepsy.
Among the forms of epilepsy in the examined children, the symptomatic one prevailed. The comparison was made with a similar group of children who received similar metabolic therapy, and standard anticonvulsants (phenobarbital, clonazepam, valproate, finlepsin, lamictal) as an anticonvulsant.
Our use of the broad-spectrum anticonvulsant Topamax in complex therapy with brain me) metabolites (actovegin, pantogam, neurovitan) contributed to more pronounced positive dynamics of the course of epilepsy. A positive anticonvulsant effect was observed in almost all children with varying degrees of expressiveness (seizure cessation - 54.395, frequency reduction 25095-4090, "50905-5.790). Of these, 65,790 were treated with monotherapy, and 34,39 with polytherapy (in combination with phenobarbital, depakine, clonazepam).

The first tonic-clonic attack appeared in 3 years, on the 5th day after DTP vaccination against the background of hyperthermia, lasting 40 minutes. After 2 months, simple absences appeared - up to 10 per day (it froze for a few seconds, when moving - fell), hyperactivity and attention deficit syndrome appeared.
In 2008, he was examined at the IPAG for 8 months.
EEG mapping of the brain revealed signs of significant disturbances in the bioelectrical activity of the brain of an irritative-paroxysmal nature, residual organic genesis with the formation of a stable focus of epileptiform and specific epileptic activity in the occipital-parietal fields of the cerebral cortex, in the form of bilateral-synchronous generalized "sharp" waves, with a frequency of 4 cycles/s, as well as discharges of spikes and complexes "sharp wave - slow wave" according to the type of absences.
MRI of the brain: foci of increased intensity of the MK signal due to immature myelination are determined periventricularly in relation to the posterior parts of the lateral ventricles.
-Against the background of prescribed anticonvulsant therapy - Topamax 25 mg per day - the attacks stopped. -
At 4 years and 7 months, on the background of otitis with hyperthermia, attacks resumed in the form of complex absences (rolling of eyes under the forehead, loss of consciousness, tongue sticking out, lasting up to 1-3 minutes; periodic (2-5 times a month) simple absences occurred. After re-examination at the Institute of PAH, the dose of Topamax was increased to 75 mg per day (average therapeutic dose) by the method of titration, and the seizures stopped. The period of clinical remission lasts 7 months.
В поєднанні з антиконвульсивною терапією дитинна періодично (кожні З місяці) отримує базову терапію - мозкові метаболіти, вазоактивні препарати, вітамінотерапію (пантогам, актовегін, нейровітан).In combination with anticonvulsant therapy, the child periodically (every 3 months) receives basic therapy - brain metabolites, vasoactive drugs, vitamin therapy (pantogam, actovegin, neurovitan).
Along with the cessation of attacks, the syndrome of hyperactivity and attention deficit decreased, the function of the "facial nerve" improved. The child is preparing to enter school, he has mastered the preschool program according to his age. c c Thus, the developed method of treating epilepsy in preschool children with the use of basic therapy (pantogam, actovegin, neurovitan) and the anticonvulsant drug Topamax contributes to the cessation of reducing the frequency of attacks, improving psychomotor and language development, psycho-emotional state, reducing manifestations of hyperactivity and attention deficit syndrome, and the child becomes more capable of learning.
The proposed method can be used in consulting polyclinics and neurological hospitals. se)
