Diabetic foot syndrome is an important medical and social problem today, as its purulent-necrotic 2 complications are the leading cause of loss of reproductive function of the lower extremities. To date, a large number of methods of local treatment of this pathology using various means have been proposed: ointments on a hydrophilic and lipophilic basis with numerous pharmacological compositions of a multidirectional mechanism of action; solutions of drugs with an antibacterial effect for sanitation of the wound cavity; biologically active agents (preparations of proteolytic enzymes). The main goal of local treatment of 70 foci of the purulent-necrotic process in diabetic foot syndrome in | the phase of the wound process is to ensure adequate outflow of wound exudate and elimination of necrosis. During the treatment of purulent and purulent-necrotic wounds in the diabetic foot in the specified phase of the wound process, one of the most effective measures is the use of sorption-application therapy, which is based on the use of medical sorbents.
О,.,A known method of local treatment of foci of purulent-necrotic lesions in diabetic foot syndrome, 12 which involves the use of a draining sorbent, "Celesorb" (trade name, "Regenkur") | Lyapis M.O,.
О. Gerasimchuk P.O. Diabetic foot syndrome. - Ternopil: Ukrmedknyga, 2001. - P.240-2414. The method of local treatment using the specified sorbent is as follows: after surgical treatment and sanitation, the wound is filled with celesorb, the wound cavity is filled with dry powder. The drug, adsorbing exudate and necrotic tissue, increases in volume and slowly fills the entire wound cavity on its own.
The use of drains in this technique is not provided for, the wound is closed with a gauze bandage. During dressings, the formed gel is removed with a Volkman spoon, and then - by washing with an antiseptic solution under pressure.
The main disadvantages of this method of treatment are insufficient efficiency due to excessive drying of tissues and the local occurrence of foci of secondary necrosis, which can contribute to the prolongation of the process, especially in the presence of necrostridial anaerobic microflora. The occurrence of these complications is associated with reduced tissue tolerance against the background of microcirculation disorders and metabolic distress.
--The closest in terms of technical essence and achievable result is the method of local treatment of wounds using application bandages, which contain pharmacological compositions with sorption activity. They carry out surgical treatment of the wound, sanitation of the wound cavity with antiseptic solutions, followed by the application of an application bandage "Wave" I, 1995. - C 178 - 1794, which is a sorption-active carbon fiber located in gauze bags. The dressing has sufficient sorption activity, due to the use of a carbon base, the elimination of exudate with microorganisms, their toxins and products is ensured tissue decay. Sorption continues until the exudate is saturated with 3o of the entire layer of the sorption material. --
The disadvantages of this method of local treatment include insufficient efficiency, which is due to the need for frequent replacement of the sorbent as the sorbent becomes saturated, the difficulty of densely filling the wound cavity with packets of sorbent in the case of a complex topographic-anatomical structure of the wound cavity, lack of necrolytic action, and the relatively high cost of treatment. With 50 The basis of a useful model is the task of improving the method of local treatment of purulent-necrotic lesions in diabetic foot syndrome, in which through the combined use of various methods of elimination

Control over the course of the wound process is carried out by carrying out revision of the wound, cytological examination, planimetry and bacteriological examination of foci of the purulent-necrotic process in dynamics.
Application of the proposed method of local treatment of purulent-necrotic lesions in diabetic foot syndrome ensures high economic efficiency due to the implementation of material-saving technology. The use of a combined multilayer bandage, in which an atraumatic napkin made of carbon /5 Fabric is the first layer, allows replacing the surface gauze layers as they are soaked with exudate, without removing the sorbent itself, however, it is enough to replace the napkin once a day.
An example of practical use. Patient V., 57 years old, has been suffering from type 1 diabetes for 6 years, at the time of consultation he was diagnosed with a severe form of diabetes, stable course, insulin dependence.
Upon admission to the surgical department, the diagnosis was established: diabetic foot syndrome, neuroischemic 2nd form, OA art. for U/adpeg; bilateral occlusion of the lower leg arteries; chronic ischemia of the lower extremities B century.
тDiabetic distal vegetosensory polyneuropathy; epifascial phlegmon of the plantar part of the right foot. On the first day, urgent surgical treatment was performed - dissection and drainage of phlegmon. Bacteriological examination revealed Ziary. acygeiz5, sensitive to amoxicillin, ciprofloxacin, ceftazidime. In the postoperative period, local treatment was carried out: daily dressings with staged mechanical necroctomies, application of chymotrypsin to areas of fixed surface necrosis, application of a sorbent in the form of an atraumatic napkin made of carbon fabric as part of a multilayer bandage. t
The surface layers of the bandage were changed twice a day, the sorbent was changed once a day. Transition of the wound in
вThe P phase of the wound process was diagnosed on the b day, and the ISH phase on the 25th day. The duration of inpatient treatment was 28 days, the total duration of treatment was 52 days (the average duration of treatment for this stage of diabetic foot syndrome is 67-5.4 days). The weight-bearing function of the lower limbs was preserved in full as a result of the treatment, the patient was discharged for light work. in
