Спосіб виконують наступним чином. Медіальним доступом у верхній третині гомілки виділяють дистальний прохідний сегмент підколінної артерії, беруть його на тримачі. Доступом на стегні виділяють поверхневу чи загальну стегнову артерію, впевнюються у її прохідності, беруть на турнікети. З окремих доступів на стегні та гомілці виконують дисекцію великої підшкірної вени на необхідному для шунтування протязі. Вену промивають та перевіряють на герметичність. Вимірюють діаметр вени, та після реверсії розміщують її в армований штучний графт відповідно більшого на 1-2 мм розміру. Виконують поздовжню артеріотомію стегнової артерії. Формують проксимальний анастомоз аутовенозного шунта з стегновою артерією, за допомогою тунеллера шунт в міжм'язевому просторі проводять на гомілку. Виконують артеріотомію підколінної артерії формують дистальний анастомоз шунта з підколінною артерією. Послідовно відтискають затискачі з стегнової та підколінної артерій. Поступив у відділення хірургії магістральних судин 02.04.2019р. з скаргами переміжну кульгавість до 50 метрів, болі у спокої вночі у лівій нижній кінцівці, зниження чутливості у пальцях лівої ступні За даними мультиспіральної комп'ютерної томографії з контрастуванням виявлено оклюзію лівої поверхневої стегнової та підколінної артерії (проксимальна третина). 04.04.2019р. хворому виконали оперативне втручання під загальним наркозом. Спосіб виконували наступним чином. Медіальним доступом у верхній третині гомілюи виділили дистальний прохідний сегмент підколінної артерії, взяли його на тримачі. Латеральним доступом у верхній третині стегна виділили загальну та глибоку стегнову артерію, артерії прохідні та пульсують, взяли на турнікети. Під час операції та в післяопераційному періоді ускладнень не спостерігалось. Рани гоїлись первинним натягом. Хворий в задовільному стані виписаний на 7 добу після операції для подальшого амбулаторного лікування. На момент виписки за даними дуплексного сканування магістральні артерії лівої нижньої кінцівки та шунт прохідні, товщина стінки аутовенозного шунта 11 мм. Проведено дослідження товщини стінки аутовенозного шунта на рівні колінного суглобу через 6 міс. після втручання - 1,2 мм. По запропонованому способу прооперовано 5 хворих, через 6 місяців прохідність шунта збережена у всіх хворих, товщина стінки аутовенозного шунта за даними ультразвукового дослідження склала 1,2:20,3 мм. Порівняно у 5 хворих оперованих за аналогом - у 1 хворого трапилась реоклюзія шунта, товщина стінки аутовенозного шунта склала 2,А40,4 мм на 6 місяці спостереження.No complications were observed during the operation and in the postoperative period.
