Description
The utility model belongs to the field of pharmacy and medicine, namely to drugs in the form of an ointment for the treatment of dermatological diseases.
Over the past decades, the specific weight of dermatitis in the structure of the general morbidity of the population has especially increased, primarily due to medicinal toxicodermas and other skin lesions of an allergic and inflammatory nature. The incidence of allergic dermatoses is about 6-1,595, psoriasis - 2-4,96 of the population of the globe, which determines the relevance of the problem and its socio-economic significance.
Vigilant attention to this problem is also due to the fact that the etiology and pathogenesis of dermatological diseases have not yet been definitively clarified. In addition, there is a tendency to increase the number of patients with chronic dermatoses with a tendency to disseminate skin rashes, as well as the addition of bacterial and fungal infections.
For the effective treatment of dermatological diseases, agents with a wide spectrum of pharmacological action are needed, in particular those that exhibit complex anti-allergic, anti-inflammatory, antimicrobial activity.
In this direction, work is being carried out on the development of combined means of external therapy, which include various combinations of corticosteroids and antimicrobial substances with a wide spectrum of action, which make it possible to exert a targeted influence on the localization of the pathological process and to minimize complications arising from oral or parenteral use.
The well-known drug "Triderm" is in the form of an ointment for the treatment of dermatoses sensitive to corticosteroids, in the presence (or suspicion) of bacterial and/or fungal infections caused by microorganisms sensitive to the components of the drug. The combination of anti-allergenic, antibacterial and antifungal effects in one drug makes "Triderm" the optimal drug for the local treatment of complicated diseases of the skin and mucous membranes. "Gryderm" combines the anti-inflammatory, anti-pruritic, anti-allergic and anti-exudative effects of betamethasone dipropionate glucocorticosteroid (SC) with the antifungal activity of clotrimazole and the broad antibacterial effect of gentamicin sulfate. Unfortunately, the inherent disadvantages of GCS (development of osteoporosis, increased blood pressure, water retention in the body, exacerbation of hidden foci of infection, increased blood glucose levels, insomnia, atrophy of the adrenal gland) apply to the drug in general.
The presence of an antibiotic from the group of aminoglycosides - gentamicin in the preparation does not provide the necessary bactericidal effect, because it is an effective means of treating infections caused mainly by gram-negative microorganisms.
Of the latest generation corticosteroids, mometasone furoate is of great interest.
Due to its biochemical and pharmacological properties, mometasone furoate has found wide application in medical practice, including in pediatrics. Compared to other strong corticosteroids, mometasone furoate has less pronounced side effects when used topically, which allows it to be used in children aged 2 to 12 years. Compared to betamethasone, mometasone furoate exhibits a more pronounced anti-inflammatory effect and a longer duration of action, and at the same time has a lower potential for systemic side effects.
Mometasone furoate does not contain a fluorine atom, which is due to its high safety compared to fluorinated corticosteroids, which include most corticosteroids, including betamethasone.

The known composition is used to treat or prevent psoriasis, atopic dermatitis (atopic eczema) and other skin diseases.
The disadvantage of the known composition is a narrow spectrum of pharmacological action due to the presence of only one active substance in its composition. Thus, in the case of joining a bacterial or fungal infection, if it is suspected or if there is a high probability of its development, the known composition is ineffective.
The task of the useful model is to create a highly effective pharmaceutical composition for external use with anti-allergic, antibacterial and antimicrobial action with a minimal risk of side effects for the treatment of dermatological diseases.
100,00The task is solved by the fact that the pharmaceutical composition in the form of an ointment for the treatment of dermatological diseases, containing mometasone furoate as an active substance and auxiliary substances, additionally contains the active substances naftifine hydrochloride and mupirocin, and as auxiliary substances it contains macrogolglycerol hydroxystearate 40, petroleum jelly and petroleum jelly , with the following ratio of components, g: mometasone furoate (per 100 95 dry matter) 010 Naftifin hydrochloride (per 100 95 dry matter) 1.00 mupirocin (per 100 95 dry matter) 2.00 macrogolglycerol hydroxystearate 40 8.00 Vaseline oil 5.00 Vaseline up to 100.00
Naftifin is a synthetic antifungal drug that belongs to the group of broad-spectrum allylamines. According to the Cochrane meta-analysis, allylamines are more effective than azoles, to which clotrimazole belongs. One of these drugs is naftifine, which is considered more active than clotrimazole. Naftifin has a number of advantages over other antifungal drugs. It has a high selectivity of action relative to the fungal cell. Naftifin shows high fungicidal activity and minimal severity of side effects. Naftifin is active against dermatophytes such as trichophyton, epidermophyton and microsporum, yeast (Sapaiida), molds (Azregaishnv5) and other fungi (for example, 5rogoingih zspepskii). Naftifin also exhibits antibacterial activity against gram-positive and gram-negative microorganisms that can cause secondary bacterial infections along with mycotic lesions. In addition, naftifine has anti-inflammatory properties. The "gold standard" of local antibacterial therapy is the use of mupirocin.
Mupirocin is an antibiotic originally isolated from the culture of Rzendotopaz Pyogezsepv. It is a derivative of butenoylnonanoic acid, a mixture of pseudomonium (monocarboquinic) acids with a predominance of pseudomonium acid A (90 95), as well as acids B, C and Y. Mupirocin strongly inhibits the synthesis of protein and RNA. while the effect on DNA and cell wall formation is much less pronounced. At low concentrations, the action of the drug is bacteriostatic, at high concentrations, a bactericidal effect develops. Important is the fact that mupirocin does not have cross-resistance with other antibiotics. The spectrum of action of mupirocin is quite wide.
Antibacterial effectiveness of mupirocin is higher compared to gentamicin, and microbial resistance occurs less often.
The composition of the drug includes the following auxiliary substances: macrogolglycerol hydroxystearate 40, petroleum jelly, petroleum jelly.
Macrogolglycerol hydroxystearate 40 is a solvent for mupirocin. Vaseline oil is used during the technological process as a dispersion medium in the preparation of a suspension of mometasone furoate and in the preparation of a suspension and grinding of naphthifine hydrochloride, vaseline is used as the basis of a dispersion medium for an ointment.
