Спосіб передопераційної підготовки хворого до хірургічного лікування з приводу деструктивного туберкульозу легень включає ретростернальне введення протитуберкульозних препаратів разом з одночасним системним їх застосуванням. Виконують спіральну комп'ютерну томографію, ЕХО-кардіографію і діагностичну фібробронхоскопію, одночасно створюють лікувальний штучний пневмоторакс на стороні ураження і штучний пневмоперитонеум, повторюючи їх кожні 2 тижні під контролем ЕХО-кардіографії.The method of preoperative preparation of the patient for surgical treatment of destructive pulmonary tuberculosis includes retrosternal administration of anti-tuberculosis drugs together with their simultaneous systemic use. Передопераційна підготовка хірургічних хворих фтизіатричного профілю є дуже важливою частиною лікувального процесу у зв'язку з виснаженістю і дисметаболічними змінами спричиненими туберкульозною інфекцією. Підвищення ефективності лікування хворих з деструктивними формами туберкульозу легень можливе за рахунок застосування хірургічного етапу. В свою чергу, ефективне використання оперативних втручань корелює з проведенням інтенсивної передопераційної підготовки, метою якої є стабілізація туберкульозного процесу, розсмоктування інфільтрації, казеозних вогнищ, поліпшення стану хворого. Результати передопераційної підготовки значно покращуються з використанням методів лімфотропної терапії (див. Відомий спосіб передопераційної підготовки хворого до хірургічного лікування з приводу деструктивного туберкульозу легень, який полягає у тому, що проводять озонотерапію шляхом внутрішньовенного введення озонованого фізіологічного розчину на фоні лікування хворого антимікобактеріальними препаратами. Для озонування фізіологічного розчину використовується апарат "Озон УМ-80" (може бути використаний будь-який апарат для озонування крові і розчинів). Щоденно ретростернально вводять протитуберкульозні препарати та призначають системну протитуберкульозну терапію. Одночасно створюють лікувальний штучний пневмоторакс на стороні ураження і штучний пневмоперитонеум. Кожні 2 тижні повторюють лікувальний пневмоторакс і пневмоперитонеум під контролем ЕХО-кардіографії. Після досягнення клініко- рентгенологічної стабілізації туберкульозного процесу виконують оперативне втручання.Anti-tuberculosis drugs are injected retrosternal daily and systemic anti-tuberculosis therapy is prescribed. Після поступлення хворого в стаціонар йому катетеризували за Сельдінгером ретростернальний простір. У положенні хворого на спині виконали поверхневу анестезію 1 9о розчином лідокаїну зони мечоподібного відростка грудини. Ретроксифоїдально ввели голку і провели її позаду грудини у напрямку ІЇ ребра гідравлічним препаруванням тканий, через голку ввели провідник, по якому у ретростернальний простір вкрутили катетер з набору для катетеризації центральної вени. Було діагностовано нагноєння клітковини середостіння. Катетер було видалено. Встановлено дренаж у верхнє середостіння, призначена антибіотикотерапія та дезінтоксикаційна терапія. ЕХО-кардіографію і діагностичну фібробронхоскопію. За даними досліджень виявлено порожнину деструкції верхньої частки правої легені без рівня рідини на фоні інфільтрації і множинних щільних тіней з петрифікатами. За даними ЕХО-кардіографії і фібробронхоскопії патологічних змін не виявлено. Після консультації фтизіатра була призначена системна протитуберкульозна терапія за 2 категорією. Після загально-клінічного обстеження одночасно створили лікувальний штучний пневмоторакс на стороні ураження і штучний пневмоперитонеум. Кожні 2 тижні повторювали лікувальний пневмоторакс і пневмоперитонеум під контролем ЕХО-кардіографії. Щоденно ретростернально вводили протитуберкульозні препарати. Після досягнення клініко-рентгенологічної стабілізації туберкульозного процесу виконали оперативне втручання - типову резекцію верхньої частки правої легені.Echocardiography and diagnostic fibrobronchoscopy. Післяопераційний період перебігав без ускладнень. Хворий у задовільному стані був виписаний із стаціонару через 25 днів для продовження лікування у фтизіатричному стаціонарі за місцем проживання.The postoperative period was uneventful. Нами був проведений ретроспективний аналіз 37 випадків передопераційної підготовки хворого до хірургічного лікування з приводу деструктивного туберкульозу легень.We conducted a retrospective analysis of 37 cases of preoperative preparation of a patient for surgical treatment for destructive pulmonary tuberculosis.
Запропонований спосіб був застосований у 17 хворих, 20 пацієнтам проводилась передопераційна підготовка за способом-аналогом.
