Спосіб екстреної кератопластики перфоративної виразки роговиці включає формування аутосклерального шматка, отсепаровування його до країв роговиці. Відсепарований шматок накладають його внутрішньою поверхнею на роговицю, закриваючи її дефект, і фіксують до країв роговиці вузловими швами з наступною фіксацією лікувальною контактною лінзою.The method of emergency keratoplasty of perforated corneal ulcer involves the formation of an autoscleral piece, separating it to the corneal edges. The detached piece is placed on its inner surface on the cornea, closing its defect, and fixed to the corneal edges by nodal sutures, followed by the fixation of the healing contact lens.

Корисна модель належить до медицини, а саме до кератопатології й може бути використана для екстреного закриття дефекту роговиці при її перфорації.The useful model belongs to medicine, namely to keratopathology and can be used for emergency closure of a corneal defect in case of perforation.
Відомий спосіб виконання кератопластики при перфорації роговиці, взятий як найближчим аналог, при якому виконують операцію кератопластики аутосклерою (1) шляхом формування 2- х склеральних шматків із діаметрально протилежних сторін очного яблука, відсепарував їх до країв роговиці, потім шматки перевертають на роговицю, покриваючи її дефект, і зшивають вільні краї цих 2-х шматків.The well-known method of performing keratoplasty in the case of corneal perforation, taken as the closest analogue, in which the autoscleral keratoplasty operation is performed (1) by forming 2 scleral pieces from diametrically opposite sides of the eyeball, separating them to the edges of the cornea, then the pieces are turned over to the cornea, covering it defect, and sew the free edges of these 2 pieces.
Однак даний спосіб має наступні недоліки: шматки виявляються переверненими щодо свого вихідного стану, що є протиприродним, і тому вони піддаються досить швидкому лізируванню (у плині 1.5-2 тижнів), не встигнувши заєпітелізувати дефект у роговиці. Наявність вузлових швів між шматками дратує слизувату внутрішньої поверхні повік і очних яблук, підтримуючи запальний процес.However, this method has the following disadvantages: the pieces turn out to be inverted in relation to their original state, which is unnatural, and therefore they are subjected to rather rapid lysis (in the course of 1.5-2 weeks), without having time to epithelize the defect in the cornea. The presence of knotted seams between the pieces irritates the mucous membrane of the inner surface of the eyelids and eyeballs, supporting the inflammatory process.
В основу корисної моделі поставлена задача вдосконалення способу екстреної кератопластики перфоративної виразки роговиці у якому забезпечують підвищення ефективності за рахунок форми й способу фіксації трансплантата, більш щільної й тривалої адаптації трансплантата до поверхні роговиці.The basis of a useful model is the task of improving the method of emergency keratoplasty of a perforated corneal ulcer, which provides increased efficiency due to the shape and method of fixation of the graft, more dense and long-term adaptation of the graft to the surface of the cornea.
Поставлена задача вирішується тим, що в способі екстреної кератопластики перфоративної виразки роговиці що включає формування аутосклерального шматка, відсепаровування його до країв роговиці, згідно з корисною моделлю, відсепарований шматок накладають його внутрішньою поверхнею на роговицю, закриваючи її дефект, і фіксують до країв роговиці вузловими швами з наступною фіксацією лікувальною контактною лінзою.The task is solved by the fact that in the method of emergency keratoplasty of a perforated corneal ulcer, which includes the formation of an autoscleral piece, its separation to the edges of the cornea, according to a useful model, the separated piece is placed on the cornea with its inner surface, closing its defect, and fixed to the edges of the cornea with knotted sutures followed by fixation with a therapeutic contact lens.
Спосіб здійснюють наступним чином. Після ретробульбарної ін'єкції знеболюючого препарату (наприклад 2 95 розчин лідокаїну 1.5 мл.), у верхньо-зовнішньому секторі очного яблука зі склери радіарно до роговиці викроюють смужку склери на 75 її товщини. По площі склеральний шматок викроюють більшого розміру, чим дефект, що прикривається їм, на 3-5 мм, тому що він після відділення від підлягаючих тканин істотно скорочується й зменшується в розмірі.The method is carried out as follows. Приклад використання способу. У відділення мікрохірургії ока надійшла дівчина 25 років зі скаргами на біль, сльозотечу, втрату зору на ліве око. Гострота зору лівого ока -0,02 не кор. Виражена світлобоязнь, помірна гіперемія країв і слизуватої повік, нижню половину роговиці проростають поверхневі судини, доходячи до центра. В парацентральній зоні роговиці -наскрізний круглий дефект діаметром 1.5 мм виконаний клітинним детритом і райдужкою, що вставилася. Через перфорацію виділяється водяниста волога, передня камера щелевидна, різка гіпотонія, зіниця деформована задніми синехіями. (Фото 1).The visual acuity of the left eye is -0.02 not cor. З огляду на ургентний стан лівого ока, у терміновому порядку хворій була зроблена пластика роговиці аутосклерою з лікувально-тектонічною метою за пропонованою методикою.In view of the urgent condition of the left eye, the patient was urgently treated with autoscleral corneal plastic surgery with therapeutic and tectonic purposes according to the proposed method.
Зроблена ретробульбарна ін'єкція 2 95 лідокаїну 1.5 мл. У верхньо-зовнішньому секторі очного яблука зі склери радіарно викроєна смужка шириною 4 мм довжиною 9 мм на Мі товщини склери, яка була відсічена, а потім накладена на роговицю, прикриваючи її дефект і пришита внутрішньою поверхнею до периферії роговиці 4 вузловими швами. Склеральний шматок викроювали більшого розміру, чим дефект, що прикривається їм, тому що він після відділення від підлягаючих тканин сильно скорочується й зменшується в розмірі. Вже при огляді наступного дня було виявлено, що роздратування ока стало менш вираженим, краї рани адаптовані шви лежать добре, дефект роговиці закритий аутотрансплантатом, передня камера відновилася, нормалізувався офтальмотонус. У післяопераційному періоді були призначені загальні й місцеві антибактеріальні препарати, глюкокортикоїди, кератопротектори.In the postoperative period, general and local antibacterial drugs, glucocorticoids, and keratoprotectors were prescribed.
При огляді на 7 добу після операції: око більш спокійне, набряк і інфільтрація роговиці регресували, видима частина роговиці стала прозорою, положення трансплантата правильне, бо дефект роговиці закритий.At the examination on the 7th day after the operation: the eye is calmer, swelling and infiltration of the cornea have regressed, the visible part of the cornea has become transparent, the position of the transplant is correct, because the corneal defect is closed.
При подальшому офтальмологічному спостереженні через З місяці було відзначене поступове лізирування шматка аутосклери, на місці колишнього дефекту роговиці сформувалося ніжне парацентральне помутніння, передня камера середньої глибини, реакція зіниці на світло збережена, офтальмотонус пальпаторно у нормі. Підвищилася гострота зору, МО5-0,5 не кор. Через 4 місяці після операції аутотрансплантат продовжував лізируватися, роговиця ставала більше прозорою, гострота зору залишалася- 0.5 не кор., у зв'язку з ніжним парацентральним помутнінням (Фото 4).4 months after the operation, the autograft continued to lyse, the cornea became more transparent, the visual acuity remained - 0.5 cor., in connection with gentle paracentral opacification (Photo 4).

Спосіб екстреної допомоги, що заявляється, є природним, не потребуючим додаткового донорського матеріалу, адаптованим в антигенному відношенні, що дозволяє трансплантату довгостроково - до 4х місяців утримуватися на роговиці до повного її загоєння й самостійно лізируватися в міру закриття наскрізного дефекту роговиці, залишаючи ніжне помутніння в області колишньої перфорації.The proposed emergency aid method is natural, does not require additional donor material, and is antigenically adapted, which allows the graft to remain on the cornea for a long time - up to 4 months, until it is completely healed, and to lyse independently as the through-through corneal defect closes, leaving a gentle cloudiness in areas of former perforation.
Зазначений спосіб лікування успішно був застосований у 12 пацієнтів з перфоративною виразкою роговиці. Він дозволяє зберегти око як орган і навіть одержати зір.This method of treatment was successfully used in 12 patients with a perforated corneal ulcer.
