Abstract
A method of local treatment of areas of paranecrosis in deep burns involves the introduction into the areas of the paranecrosis of a composition containing antiseptics, stimulants of regenerative processes and water-based painkillers. The introduction of drugs is carried out, starting from the first day after burns, through the day by infiltration, in addition, cytoprotectants and antioxidants of direct specific action on the aqueous mixture of succinic acid (reamberin) and lipin are introduced into the composition of the preparations. actions (pentoxifylline antiplatelet agent and nadroparin anticoagulant), and the antiseptic and analgesic agent are combined in dioxidine solution with lidocaine.
Description
A useful model belongs to combustiology and can be used to treat areas of paranecrosis in deep burns.
In case of deep burns, the basis of tissue damage is the activation of free radical oxidation (VDO) of cell membranes with disruption of their functions and intracellular energy balance, which ultimately leads to cell apoptosis in the areas of paranecrotic changes. Diagnosing the stage of the process of BPO activation and lipid peroxidation in the vast majority of clinical cases is rather difficult due to their avalanche-like growth. Therefore, it is necessary to create a combination of antioxidant drugs and adaptogens that would maximally protect the lipid membrane of cells from destruction and restrain the action of stress-implementing systems, that is, create a combination of drugs that would have a positive effect on all links of the developing pathological process.

Він включає введення в зони паранекрозу присипок, які включають антибактеріальні речовини (карбонат кальцію, гідрокарбонат кальцію), стимулятор регенераторних процесів (фосфат кальцію), адсорбент (фторид кальцію), антисептик (магній та марганець) у відповідних співвідношеннях: фосфат кальцію - 8025 95, карбонат кальцію - 10-44 95, фторид кальцію - 4-1 95, гідрокарбонат кальцію - 5 9о, магній - 0,5 95, марганець - 0,5 95.A known method of treatment of deep burns according to patent Me2223751 IV, application 08.01.2003. published 20.02.2004, Medicine for the treatment of burns. It includes the introduction of powders into the areas of paranecrosis, which include antibacterial substances (calcium carbonate, calcium bicarbonate), a stimulator of regenerative processes (calcium phosphate), an adsorbent (calcium fluoride), an antiseptic (magnesium and manganese) in the appropriate ratios: calcium phosphate - 8025 95, calcium carbonate - 10-44 95, calcium fluoride - 4-1 95, calcium bicarbonate - 5 9o, magnesium - 0.5 95, manganese - 0.5 95.
The method makes it possible to increase the speed of reparative processes of post-necrotomous wounds by 2-3 times, reducing the healing time of wounds by an average of 6-10 days. But the method affects the surface cell layers of post-necrotomy wounds and can be used only after necrotomy. Therefore, the process of degenerative-destructive changes continues in the area of paranecrosis and its cellular components, the performed necrotomy may be inadequate, and the subsequent autotransplantation may be unsuccessful.
The closest to the useful model is the method according to patent No. 2177314 |B, application. 22.03.2000, publ. 27.12.2001, Composition with antiseptic, reparative and pain-relieving properties). It includes the introduction into paranecrosis zones of a composition of drugs, which consists of antiseptics, stimulants of regenerative processes, and water-based pain relievers. After the necroctomy, the composition of drugs is administered to the paranecrosis zones
Композиція препаратів, яка додається рідкої основи, складається з мірамістину (антисептик), депангенолу або кальцію пантотенату (стимулятори регенеративних процесів) і тримекаїну (знеболювальний засіб).It is carried out by application (napkins impregnated with an aqueous mixture of polyethylene oxide and propylene glycol). The composition of drugs, which is added to the liquid base, consists of miramistin (an antiseptic), depangenol or calcium pantothenate (stimulators of regenerative processes) and trimecain (analgesic agent).
The method makes it possible to provide a complex effect on the post-necrotomous paranecrotic zone, namely antimicrobial, anti-inflammatory, analgesic and regenerative effects with the help of the composition of medicinal products. But the described method is designed to affect only the surface layers of the post-necrotome wound by application and includes the activation of reparative processes in the islands of epithelization and the edges of the wound. In case of deep burns, the method cannot be used, as the skin is irreversibly damaged to the full depth at the time of exposure to the injurious agent. The selected medicinal composition does not take into account the state of the membranes and the energy balance of cells in the paranecrosis zone.
The basis of a useful model is the task of creating a method of local treatment of paranecrosis zones in case of deep burns, which allows to preserve and functionally restore the maximum volume of cells in the paranecrosis zone by maintaining their energy balance and protecting lipid membranes that are partially damaged by trauma.
The task is solved by the fact that in the method of local treatment of paranecrosis zones in case of deep burns, which includes the introduction into the paranecrosis zones of a composition of drugs containing antiseptics, stimulators of regenerative processes and water-based pain relievers, according to a useful model, the introduction of drugs is carried out starting from on the first day after the burn, after a day by infiltration, cytoprotectors and antioxidants of direct specific action on an aqueous mixture of succinic acid (reamberin) and lipin are additionally introduced into the composition of the drugs, while ozonized lipin and indirect antihypoxants (antiaggregant pentoxifylline and anticoagulant nadroparin) are chosen as a stimulator of regenerative processes ), and an antiseptic and an anesthetic are combined in a solution of dioxidine with lidocaine.
0,06.To achieve the optimal clinical effect, it is advisable to choose the composition of the drugs in the following composition and ratio of components (wt. b): ozonized reamberin 47.8 ozonized lipin 47.8 solution of dioxidine with lidocaine 2.9 pentoxifylline 1.44 nadroparin 0.06.
The introduction of drugs by infiltration allows to evenly distribute the medicinal composition throughout the entire layer of the paranecrosis zone, which provides the possibility of maximum preservation of its partially damaged cells.
The choice of drugs is determined by the following considerations.
The introduction of reamberin (47,895) as a regulatory antihypoxant (infusion solution of succinic acid and M-methylglucamine), which penetrates into damaged cells, allows to stabilize the structure and function of cellular mitochondria, increase cellular energy (increase ATP synthesis), improve the synthesis of cellular proteins and the nature of ion exchange . The basis of the preparation is a polyunsaturated salt solution, and during ozonation it becomes a carrier of both active forms of oxygen and ozonides. In a complex with succinic acid and dextran (glucose polymer), it allows you to significantly activate aerobic processes in cells by stimulating the enzymatic processes of the Krebs cycle. It should be added that the introduction of ozonated reamberin as one of the main elements of the medicinal composition provides a synergistic effect from all other components of the created solution. Depending on the weight of the patient, its amount can vary between 60052200 ml.
Phosphaditylcholine (Lipin, lecithin) together with reamberin is the main component of the composition (47.8 95), it is an antioxidant with an anti-hypoxant indirect effect, it performs the function of a non-specific detoxifier and immunocorrector. Its ozonized preparation increases the rate of oxygen tissue diffusion of ozonides and peroxides, which take part in the reconstruction of cell membranes, improving the energy metabolism of cells, inhibits the peroxidation of lipids in tissues and increases the activity of antioxidant systems. The drug also improves tissue microcirculation and blood rheology, eliminates tissue hypoxia and endothelial hypofunction. Depending on the weight of the patient, its amount can vary between 600-2200 ml.
The solution of dioxidin with lidocaine is a combined drug that includes one of the most powerful antioxidants - dioxidin (1.0 95 solution) and lidocaine chloride (1 95 solution) based on water-soluble polyatomic alcohols. It has anti-inflammatory, antimicrobial and analgesic effects, stimulates natural resistance factors and energy metabolism of cells in areas of paranecrotic damage. Antioxidant action is associated with inactivation
В залежності від ваги хворого його кількість може варіювати в межах 362-412 мл (діоксидин - 30--10, лідокаїн - б--2).From hydroxyl radicals and improvement of metabolic processes in the inflammation zone. The drug enhances the penetration of medicinal substances into cells, has a fibrinolytic effect and inhibits the growth of all aerobic and facultatively anaerobic, as well as polyresistant bacterial strains. Depending on the weight of the patient, its amount can vary between 362-412 ml (dioxidin - 30--10, lidocaine - b--2).
В залежності від ваги хворого його кількість може варіювати в межах 15-20 мл (300-400 мг).Pentoxifylline (Trental), which improves the rheological properties of blood, normalizes the elasticity of erythrocytes, reduces platelet aggregation and blood viscosity, improves microcirculation (angioprotective action). In addition, it reduces the plasma concentration of fibrinogen, improving fibrinolysis. Under the influence of the drug, cyclic adenosine monophosphate accumulates in the smooth muscles of the vascular wall, blood and body tissues, showing a myotropic, vasodilating effect. This stimulates collateral blood circulation, improves microcirculation, cellular respiration in tissues and their oxygen saturation. Metabolites of pentoxifylline, which are formed when passing through the liver, have a similar pharmacological effect, that is, it remains for a long time in the body, enhancing the effect of fibrinolytic properties of anticoagulants (nadroparin). Trental is released in the form of a concentrated solution, which is dissolved in a mixture of remberin and lipin. Depending on the weight of the patient, its amount may vary within 15-20 ml (300-400 mg).
Nadroparin (fraxiparin) is a direct-acting anticoagulant drug, a low-molecular derivative of heparin. It effectively inhibits the Xa factor, showing high antithrombotic activity, also stimulates the tissue conductivity factor inhibitor, activates fibrinolysis processes, reduces blood viscosity, and improves platelet fluidity. Without affecting platelet aggregation and primary hemostasis, it combines immediate and long-term antithrombotic effects. Depending on the weight of the patient, its amount can vary within 0.6-0.9 ml.
This composition has a complex effect on all the main mechanisms of damage to cell membranes, including mitochondrial ones, allowing at all stages of the development of the pathological process to stop the avalanche-like increase in the activity of free radical oxidation and lipid peroxidation, i.e. acting as anti-radical inhibitors. At the same time, the composition not only protects the lipid membrane of the cell and preserves its functional capabilities,
але і забезпечує енергетичний внутрішньоклітинний баланс, купіруючи масивний клітинний апоптоз.but also provides energy intracellular balance, stopping massive cell apoptosis.
The mode of administration (every other day) is due to the fact that with minimal traumatization of the tissues, the medicinal composition deposited in the paranecrosis zone retains its effect.
The method allows from the first hours of admission of a patient with deep burns to prevent the spread of the destructive process in the cells of the paranecrosis zone and significantly reduce the volume of irreversibly dead tissue even before necrotomy, thereby reducing the volume of necrotomy and clearly separating the demarcation line between damaged and undamaged tissues.
First of all, they prepare organic ozonated solutions that make up the basis of the medicinal composition, namely, a solution of lipin (phosphatidylcholine) according to the method described in the patent Mo 97030 (application. 09.26.2014, publ. 02.25.2015, Method of increasing the protective properties of lipin ( phosphatidylcholine), as well as a solution of reamberin (succinic acid) according to patent Mo 98398 (application 17.11.2014, publ. 27.04.2015, Method of treatment of burn disease and paranecrosis zones) with the same volume, which is calculated for a specific patient depending on its weight. The resulting liposomal emulsion allows the main energy component (succinic acid) to be delivered to cell membranes and intracellular organelles - mitochondria for their construction and cell functioning. The following combination is added to the resulting solution in a mass-percentage ratio to the total amount of the combination: ozonated reamberin - 47.8 965, ozonated lipin - 47.8 956, dioxidine solution with lidocaine - 2.9 95, pentoxifylline - 1.44 95, nadroparin - 0.06 95. At the same time 2-4 ampoules of dioxidine and lidocaine ampoules of 1 95 ampoules are mixed with ether and added to the base solution (ozonated reamberin and lipin).
The described method was used in the treatment of 17 patients who were treated in
Kharkiv burn center for deep burns. All patients had a positive result, all were discharged in a satisfactory condition, even with widespread injuries, among which 10-18 95 were deep burns (PB-I degree). Treatment according to the described method made it possible to reduce intoxication, eliminate stressors, perform an adequate necrotomy and maximally prepare the tissues of the paranecrosis zone for autotransplantation of skin flaps. The obtained positive result was achieved at the expense of the maximum
To support the energy balance of cells in the paranecrosis zone and protect lipid membranes that are partially damaged by trauma. Ultimately, the volume of cells that underwent traumatic apoptosis was reduced, and the skin in these patients recovered an average of 6 days earlier compared to patients with similar injuries who received standard treatment. It should be added that a smaller volume of surgical interventions, which are aimed at restoring the skin, was needed due to the improvement of the grafting of autodermal grafts on a previously prepared wound.
Thus, performing the method according to a useful model allows you to prepare the burn wound after the primary necrotomy for autodermoplasty in the shortest possible time and thereby carry out the first stage of treatment of deep burns.
