Description of the invention
The useful model refers to medicine, namely to surgery, and it can be used in the treatment of patients 2 with damage to the peripheral segment of the arterial bed of the limb in the stage of critical ischemia.
(Еиг Мазс Епдомавзс Зиго 25, р.276-278, 2003).There is a known method of indirect revascularization of the lower limb, which includes automyelotransplantation in the tissue of the lower leg in the area of ischemia. At the same time, the stem cells contained in the bone marrow contribute to the restoration of blood vessels in the ischemia zone. (Eig Mazs Epdomavzs Zygo 25, p. 276-278, 2003).
The disadvantage of this method is the long term of revascularization of the limb. 70 The task of a useful model is the development of such a method of indirect revascularization of the lower extremity, which, due to the use of rotational osteotrepanation of the tibia, would ensure a reduction in the terms of revascularization of the lower extremity.
The task is solved by the fact that in the method of indirect revascularization of the lower extremity, which includes automyelotransplantation in the tissue of the lower leg in the ischemia zone, according to a useful model, in the ischemia zone, 12 additional rotational osteotrepanations of the tibia are performed.
Performing a rotational osteotrepanation of the tibia ensures an additional supply of bone marrow, which contains stem cells, to the tissues of the leg in the ischemia zone, which contributes to faster revascularization of the lower limb.
The method is performed as follows. The first stage of three separate accesses on the anteromedial surface and two on the anterolateral surface, measuring up to 1.5 cm, on the tibia, performs rotational osteotrepanation of the tibia using a modified technique with a bmm drill up to 15 points. At the same time, bone marrow is taken from the wing of the pubic bone in the amount of 10 ml to 200 ml. This suspension is heparinized with 0.5-1.0 ml of heparin. In the second stage, a 0.595 lidocaine solution is injected subfascially into the tissues of the lower leg distally and proximally through rotary accesses using a blunt cannula, and then automyelotransplantation is performed using the same cannula. Fasciotomy is performed from the same approaches distally and proximally to -ptv) 5 cm, the wounds are sutured tightly. They are treated with iodine, an aseptic bandage is applied.
Example
ЗPatient Yu., medical history Mo564/2005, was admitted with a diagnosis: obliterating endarteritis of the vessels of the lower extremities PI-MI. Occlusion of the leg segment on the left side. WITH
After the examination and confirmation of the diagnosis, operative treatment was performed. After treatment of the operating field (Ge) by the traditional method of three separate accesses on the anteromedial surface and two on the anterolateral surface, up to 1.5 cm in size, rotational osteotrepanization of the tibia was performed on the left tibia using a modified technique with a 15-point BMM drill. At the same time, 200 ml of bone marrow was collected from the wing of the pubic bone. One ml of heparin was added to this suspension. At the next stage, using a 3o blunt cannula, through rotational access subfascial into the tissues of the lower leg distally and proximally, 0.590 - lidocaine solution of 5Oml was injected, respectively, on the anteromedial and anterolateral surfaces, and then with the help of the same cannula, the harvested bone marrow was injected, respectively, 100ml into each area.
«Fasciotomy on the lower leg was performed from the same approaches distally and proximally up to 1 cm. "
Wounds on the shin and in the area of the wing of the pubic bone were sutured without leaving drainage, treated with iodine, and a Z 70 aseptic bandage was applied. c Beginning of revascularization on the 10th day.
оFrom" According to the proposed method, 12 patients were treated; the average time of the start of revascularization was observed at 10 days, while in the group of 12 patients treated according to the analogue method, the average time of the start of revascularization was 24 days. of 45 Thus, the use of the proposed method allows to shorten the term of limb revascularization. at
