Додатково призначають курс місцевої озонотерапії у вигляді ротових полоскань, ванночок та іригацій озонованою дистильованою водою з концентрацією озону 1,5-7 мг/л по 400-450 мл тривалістю 5-10 хвилин один раз на добу, а також фармакологічної композиції "Цитогексизол", до складу якої входить протимікробний препарат Хлоргексидин, протипротозойний препарат Метронідазол та антигіпоксант метаболічного типу дії Цитофлавін®, та яку використовують для місцевого аплікаційного лікування у формі пасти тривалістю аплікації 5-10 хвилин 1-2 рази на день.A method of treating patients with generalized periodontitis includes the use of ozone therapy and the pharmacological composition "Cytogeksisol". Приклад. Проведено комплексне клінічне і лабораторне обстеження та лікування 31 пацієнта з діагнозом: генералізований пародонтит, початкового ступеня, хронічного перебігу.Example. Серед яких 13 чоловіків та 18 жінок. Вік пацієнтів 18-35 років. Діагноз захворювань пародонта встановлювали за класифікацією М.Ф. Данилевського (1994). Клінічне дослідження проводилось у відповідності з протоколом МЗ України для хворих на генералізований пародонтит. ОНІ-5 (Стееп-Мептіїоп, 1964). Ступінь запальних процесів визначали за допомогою папілярно- маргінально-альвеолярного індексу РМА (С. Рапта, 1960). Кровоточивість ясен оцінювали індексом кровоточивості ясеневих сосочків (РВІ, по Н.А. Мийіетапп, 5. оп, 1971). Лабораторні методи включали проведення цитології вмісту пародонтальних карманів та визначення реакції адсорбції мікроорганізмів клітинами епітелія ясен (РАМ).ONI-5 (Steep-Meptiope, 1964). Основна група: середній показник індексу ОНІ-5 складав 2,075-0,15, індексу РМА - 24,82 905-1,42, індексу РВІ - 0,82:20,07, показник РАМ - 53,79 952-1,05. Контрольна група: середній показник індексу ОНІ-5 складав 2,13:20,16, індексу РМА - 25,29 90:-1,62, індексу РВІ - 0,81:0,09, показник РАМ - 54,42 965-1,22.The following clinical and laboratory examination data were obtained before the start of treatment of patients in groups. Всім пацієнтам після усунення місцевих подразнюючих чинників та проведення професійної гігієни ротової порожнини призначалось місцеве лікування. В 1-й основній групі (19 чол. лікування проводили за вище запропонованим способом, в 2-й контрольній групі (12 чол.) - за традиційною загальноприйнятою методикою. Через 3-4 сеанси лікування всі пацієнти відмічали значне покращення загального стану та стану тканин ротової порожнини: значно зменшилась кровоточивість та припухлість ясен, болі та неприємні відчуття. На 7-8-ий день спостереження всі скарги у пацієнтів першої групи практично повністю зникли, а у пацієнтів другої групи скарги значно зменшились, але ще тривали 3-4 дні.All patients were prescribed local treatment after elimination of local irritants and professional oral hygiene. Контрольна група: середній показник індексу ОНІ-5 складав 0,86:0,11, індексу РМА - 15,71 95:20,43, індексу РВІ - 0,570,06, показник РАМ - 69,33 9550,80.We have the following data from a clinical and laboratory examination conducted after 1 month by patient group. Отже, включення до комплексної терапії вище запропонованого способу лікування значно скорочує терміни лікування, більш ефективно пригнічує дистрофічно-запальний процес, дозволяє знизити лікарське фармакологічне навантаження та спрогнозувати збільшення термінів ремісії генералізованого пародонтиту.Therefore, the inclusion of the above proposed method of treatment in the complex therapy significantly shortens the duration of treatment, more effectively inhibits the dystrophic-inflammatory process, allows to reduce the drug pharmacological burden and predict an increase in the remission period of generalized periodontitis.
Таким чином, можна зробити висновок, що запропонований спосіб лікування хворих на генералізований пародонтит характеризується протимікробними, протипротозойними та протианаеробними спектрами дії, сприяє покращенню мікроциркуляції та оксигенації крові, поліпшенню реологічних та киснево-транспортних властивостей крові, оптимізації антиоксидантних механізмів захисту, кисеньзалежних біохімічних, метаболічних та енергетичних реакцій, нормалізації окисно-відновних реакцій та тканинного дихання, ліквідації кисневої недостатності в тканинах пародонта.Thus, it can be concluded that the proposed method of treatment of patients with generalized periodontitis is characterized by antimicrobial, antiprotozoal and antianaerobic spectrums of action, contributes to the improvement of microcirculation and oxygenation of blood, improvement of rheological and oxygen transport properties of blood, optimization of antioxidant defense mechanisms, oxygen-dependent biochemical, metabolic and energy reactions, normalization of redox reactions and tissue respiration, elimination of oxygen deficiency in periodontal tissues.
