Abstract
一種認知障礙復健輔助系統,包含一音訊刺激單元、一穴位刺激單元、一光照射單元、一電子刺激單元、一顯示與光頻刺激單元,及一控制單元,該音訊刺激單元用於播送一拍頻樂曲,該穴位刺激單元包括複數穴位媒介,以對一使用者輸出物理刺激,該光照射單元包括二近紅外線光燈組,該電子刺激單元包括二電刺激媒介,以對該使用者輸出物理刺激,該顯示與光頻刺激單元可於一顯示模式及一光頻刺激模式之間切換,該控制單元可同時控制該音訊刺激單元播送該拍頻樂曲、該等穴位媒介發出物理刺激、該等近紅外線光燈組發射近紅外線光、該等電刺激媒介發出物理刺激及該顯示與光頻刺激單元切換模式,以提高認知障礙患者的復健效率。
Description
本發明是有關於一種復健輔助系統,特別是指一種認知障礙復健輔助系統。
從2001年開始,我國65歲以上人口持續成長,經統計2018年在台灣65歲以上高齡者已達331萬人,占總人口14%,正式邁入「高齡社會」(內政部統計處,2018),其中輕度認知障礙(Mild Cognitive Impairment,MCI)高齡者佔18.32%,失智症高齡者佔7.93%。隨著台灣高齡者逐年增加,預期輕度認知障礙高齡者的人口數也會快速上升。
輕度認知障礙的定義為個案認知下降程度遠超乎該階段的年齡和教育程度,通常介於正常高齡者轉變成失智症進程間的過渡期。但與失智症不同的是,輕度認知障礙高齡者在基本日常活動(Basic activities of daily living,BADL)上並不受影響,但認知缺損在工具性日常生活功能(instrumental activities of daily living,IADL)受到影響。
輕度認知障礙高齡者最先開始失去工具性日常生活功能,所出現的功能缺損常見如財務管理、使用電話等,工具性日常生活功能活動要比基本日常活動要求更複雜的認知能力,因此當認知功能快速衰退後,在未來有極高風險會轉變為失智症,例如失智症中的阿茲海默症。此外,過去研究指出工具性日常生活功能與認知能力如記憶力及處理複雜性任務有高度相關,換言之,有輕度認知障礙高齡者易出現認知與工具性日常生活功能之障礙。因此,找到一種介入方式去維持輕度認知障礙高齡者之認知與工具性日常生活功能,對於輕度認知障礙高齡者是極為重要的。
因此,本發明的目的,即在提供一種認知障礙復健輔助系統。
於是,本發明認知障礙復健輔助系統,用於供一使用者使用,該認知障礙復健輔助系統包含一支架單元、一音訊刺激單元、一穴位刺激單元、一光照射單元、一電子刺激單元、一顯示與光頻刺激單元,及一控制單元。
該支架單元對應於該使用者的頭型,且包括二對應於該使用者的雙耳的耳部、一連接於該等耳部之間且朝上跨過該使用者的頭頂的頂側部、一連接於該等耳部之間且朝後跨過該使用者的後
顱的後側部,及一連接於該等耳部之間且朝前跨過該使用者的前側部,該頂側部具有一位於該等耳部間的中心處的頂區,及二分別連接於該等耳部及該頂區間的側區,該音訊刺激單元包括二設置於該等耳部的揚聲器,該等揚聲器用於對該使用者的雙耳播送一拍頻樂曲,該拍頻樂曲具有一音頻頻率差,該穴位刺激單元包括一設置於該頂區的穴位設置座,及複數穴位媒介,該等穴位媒介用於對該使用者的頭部穴位輸出雷射光的物理刺激,該等穴位媒介的其中一者可調整位置地設置於該穴位設置座相對位於該頂區的中心處以對應該使用者的百會穴,該等穴位媒介的其中一者可調整位置地設置於該穴位設置座且相對位於該頂區的中心處朝前四點五倍的一指寸距離處以對應該使用者的神庭穴,該等穴位媒介的其中兩者可調整位置地設置於該後側部並位於該頂區的中心處朝後七倍的該指寸距離且分別朝該等耳部的方向間隔二點二五倍的該指寸距離處以分別對應該使用者的兩個風池穴,該指寸距離實質上為二點三公分,該光照射單元包括二用於發射近紅外線光的近紅外線光燈組,其中一近紅外線光燈組設置於該穴位設置座且環繞對應百會穴的穴位媒介,其中另一近紅外線光燈組設置於該穴位設置座且環繞對應神庭穴的穴位媒介,該電子刺激單元包括分別設置於該等側區的一第一設置座及一第二設置座,二分別設置於該第一設置座及該第二設置座的電刺激媒介,該等電刺激媒介用於對該使用者的頭部輸
出電流以進行經顱電刺激及輸出電磁脈衝以進行經顱磁刺激的其中一者的物理刺激,該等電刺激媒介分別對應於國際10-20制腦波電極位置中的F3位置及F4位置,該顯示與光頻刺激單元設置於該支架單元並可於一顯示模式及一光頻刺激模式之間切換,於該顯示模式時,該顯示與光頻刺激單元顯示一虛擬影像供該使用者的雙眼觀看,於該光頻刺激模式時,該顯示與光頻刺激單元以閃爍畫面刺激該使用者的雙眼,該控制單元電連接該等揚聲器、該等穴位媒介、該等近紅外線光燈組、該等電刺激媒介及該顯示與光頻刺激單元,且儲存有該拍頻樂曲的數位資訊,並可根據一預設命令同時控制該等揚聲器播送該拍頻樂曲、該等穴位媒介發出物理刺激、該等近紅外線光燈組發射近紅外線光、該等電刺激媒介發出物理刺激,及該顯示與光頻刺激單元切換模式。
本發明的功效在於:藉由設置該等揚聲器、該等穴位媒介、該等近紅外線光燈組、該等電刺激媒介、該顯示與光頻刺激單元及該運動單元,使該使用者能於單次療程中同時獲得多重刺激,提高認知障礙患者的復健效率。

參閱圖1、2、3,本發明認知障礙復健輔助系統的一實施例,用於供一使用者使用,該認知障礙復健輔助系統包含一支架單元2、一音訊刺激單元3、一穴位刺激單元4、一光照射單元5、一電子刺激單元6、一顯示與光頻刺激單元7、一運動單元8,及一控制單元9。
參閱圖3、4、5,該支架單元2對應於該使用者的頭型,且包括二對應於該使用者的雙耳的耳部21、一連接於該等耳部21之間且朝上跨過該使用者的頭頂的頂側部22、一連接於該等耳部
21之間且朝後跨過該使用者的後顱的後側部23、一連接於該等耳部21之間且朝前跨過該使用者的前側部24、二安裝槽25,及複數設置於該頂側部22底部的導電條26(見圖6),該頂側部22具有一位於該等耳部間21的中心處的頂區221、二分別連接於該等耳部21及該頂區221間的側區222,該等安裝槽25分別位於該等側區222。
該音訊刺激單元3包括二設置於該等耳部21的揚聲器31,該等揚聲器31用於對該使用者的雙耳播送一拍頻樂曲(Frequency Following Response Music),該拍頻樂曲具有一音頻頻率差。於本實施例中,該拍頻樂曲的音頻頻率差介於0.1Hz~45Hz。
該穴位刺激單元4包括一設置於該頂區221的穴位設置座41,及複數穴位媒介42,該等穴位媒介42用於對該使用者的頭部穴位輸出雷射光的物理刺激。於本實施例中,該等穴位媒介42的雷射光波長介於500nm~900nm,輸出功率介於100mW~200mW。
該等穴位媒介42的其中一者可調整位置地設置於該穴位設置座41相對位於該頂區221的中心處以對應該使用者的百會穴(國際穴位代碼GV20),圖式中以編號A1表示。
該等穴位媒介42的其中一者可調整位置地設置於該穴位設置座41且相對位於該頂區221的中心處朝前四點五倍的一指寸
距離處以對應該使用者的神庭穴(國際穴位代碼GV24),圖式中以編號A2表示。於本實施例中,該指寸距離實質上為二點三公分。
該等穴位媒介42的其中兩者可調整位置地設置於該後側部23並位於該頂區221的中心處朝後七倍的該指寸距離且分別朝該等耳部21的方向間隔二點二五倍的該指寸距離處以分別對應該使用者的兩個風池穴(國際穴位代碼GB20),圖式中以編號A3表示。
該等穴位媒介42的其中四者可調整位置地設置於該穴位設置座41且相對位於該頂區221的中心處間隔該指寸距離處以分別對應該使用者的四個四神聰穴(國際穴位代碼EX-HN1),圖式中以編號A4表示。
要說明的是,前述對應使用者穴位的方式,可以在該穴位附近量測人體皮膚的電阻,並以最低電阻處作為該穴位的位置,再微調使相對應的該穴位媒介42對應該處而完成穴位的定位。
該光照射單元5包括二用於發射低能量的近紅外線光的近紅外線光燈組51,其中一近紅外線光燈組51設置於該穴位設置座41且環繞對應百會穴A1的穴位媒介42,其中另一近紅外線光燈組51設置於該穴位設置座41且環繞對應神庭穴A2的穴位媒介42。於本實施例中,該等近紅外線光燈組51所發射的光波長介於760~860nm,較佳為810nm,脈衝頻率為10Hz。
參閱圖3、5、6,該電子刺激單元6包括分別設置於該等側區222的一第一設置座61及一第二設置座62,二分別設置於該第一設置座61及該第二設置座62的電刺激媒介63,該等電刺激媒介63用於對該使用者的頭部輸出物理刺激,該等電刺激媒介63分別對應於國際10-20制腦波電極位置中的F3位置及F4位置。於本實施例中,所述的物理刺激為進行經顱直流電刺激(transcranial direct current stimulation,tDCS)的直流電,電流大小介於0.5mA~2mA,電流密度介於0.03mA/cm2~0.09mA/cm2,但不限於此,也能夠是進行經顱磁刺激(transcranial magnetic stimulation,TMS)的電磁脈衝,電磁頻率介於1-20Hz,刺激強度為90% aMT(active motor threshold),120% rMT(rest motor threshold),其中aMT及rMT為經顱磁刺激儀器的經常性強度定義,因此本說明書不再進一步進行說明。
該第一設置座61具有設置於該等側區222的其中一者及該等導電條26下方的第一滑動件611、一自該頂側部22相反該第一滑動件611的一側穿過各自的該安裝槽25並螺鎖於該第一滑動件611以用於固定該第一滑動件611的第一螺鎖件612、一自該第一螺鎖件612相反該第一滑動件611的一側可滑動地穿過該第一螺鎖件612的第一定位件613、一設置於該第一滑動件611相反該頂側部22的一側並螺接於該第一定位件613的第一安裝台614,及複數可
伸縮地穿過該第一滑動件611及該第一安裝台614且分別電連接該等導電條26的第一導電件615,該等電刺激媒介63的其中一者是設置於該第一安裝台614相反該頂側部22的一側並電連接該等第一導電件615。
該第二設置座62具有設置於該等側區222的其中另一者及該等導電條26下方的第二滑動件621、一自該頂側部22相反該第二滑動件621的一側穿過各自的該安裝槽25並螺鎖於該第二滑動件621以用於固定該第二滑動件621的第二螺鎖件622、一自該第二螺鎖件622相反該第二滑動件621的一側可滑動地穿過該第二螺鎖件622的第二定位件623、一設置於該第二滑動件621相反該頂側部22的一側並螺接於該第二定位件623的第二安裝台624,及複數可伸縮地穿過該第二滑動件621及該第二安裝台624且分別電連接該等導電條26的第二導電件625,該等電刺激媒介63的其中另一者是設置於該第二安裝台624相反該頂側部22的一側並電連接該等第二導電件625。
每一電刺激媒介63具有複數平行設置以輸出物理刺激的導電柱631。
該顯示與光頻刺激單元7設置於該支架單元2並可於一顯示模式及一光頻刺激模式之間切換,於該顯示模式時,該顯示與光頻刺激單元顯示一虛擬影像(圖未示)供該使用者的雙眼觀看,於
該光頻刺激模式時,該顯示與光頻刺激單7元以閃爍畫面刺激該使用者的雙眼。於本實施例中,該顯示與光頻刺激單元7為一種虛擬影像操作裝置,且包括一顯示模組71及一操作按鈕72。
該顯示與光頻刺激單元7於該光頻刺激模式時,該顯示與光頻刺激單元7顯示的閃爍畫面是能以一預定頻率閃爍或以一光頻頻率差分別刺激該使用者的雙眼。於本實施例中,該預定頻率是介於35~45Hz之間,使整個畫面以相同的頻率閃爍,而以該光頻頻率差分別刺激該使用者的雙眼時,即該使用者的雙眼是分別受到不同頻率的光刺激。
參閱圖1、2、5,該運動單元8訊號連接該控制單元9,供該使用者操作以進行運動,該運動單元8包括一健身車81,該健身車81具有一車架82、一可轉動地設置於該車架82的曲柄83、二設置於該曲柄83的兩相反側的踏板84、設置於該車架82的一把手架85及一椅座86,及一設置於該曲柄83並訊號連接該控制單元9以輸出一對應該曲柄83的扭力的量測訊號的扭力感測器87。
該控制單元9電連接該等揚聲器31、該等穴位媒介42、該等近紅外線光燈組51、該等電刺激媒介63及該顯示與光頻刺激單元7,且儲存有該拍頻樂曲的數位資訊,並可根據一預設命令同時控制該等揚聲器31播送該拍頻樂曲、該等穴位媒介42發出物理刺激、該等近紅外線光燈組51發射近紅外線光、該等電刺激媒介63
發出物理刺激,及該顯示與光頻刺激單元7切換模式。於本實施例中,該控制單元9為一個人電腦。
參閱圖2、3、7,該控制單元9可執行一評量程式,於該評量程式中,該控制單元9控制該顯示模組71依序顯示複數用於評估認知障礙的認知自評題目711,每一認知自評題目711具有複數自評選項712,並可透過改變該顯示模組71的方向後改變所選擇的自評選項712,並於透過該操作按鈕72的確認後產生對應所選擇的該自評選項712的一認知自評分數進行儲存,並於全部的認知自評題目711皆作答完畢後加總所有的認知自評分數後產生一認知結果分數進行儲存。於本實施例中,該顯示模組71是以一次顯示一組認知自評題目711,並於對應的自評選項712選擇完畢後,再進行下一組認知自評題目711,但並不以此為限,在其他的實施態樣中,也可以同時顯示多組認知自評題目711,而該等憂鬱自評題目711的來源,可以是來自如圖8、圖9的現有的評量表,但並不以此為限。
參閱圖1、2、5,該控制單元9可於該運動單元8運作一預定時間後,透過該顯示與光頻刺激單元7於該虛擬影像中顯示提示訊息。於本實施例中,該虛擬影像為VR影像,該數位資訊及該預設命令是經由雲端網路10下載後預存在該控制單元9中。
使用時,該使用者先配戴該認知障礙復健輔助系統,接著該控制單元9會控制使該等揚聲器31播送該拍頻樂曲,並使該等
穴位媒介42對相對應的穴位發出物理刺激,使該等近紅外線光燈組51進行照射,以及使該等電刺激媒介63對相對應的腦區發出物理刺激,如此就能使該使用者同時受到該拍頻樂曲、該等穴位、低能量照射及該等腦區的刺激。
另外,該使用者可於使用前先執行評量程式以獲得操作前的認知結果分數,再於整個操作過程結束後再進行一次評量程式以獲得操作後的認知結果分數,藉由兩次的認知結果分數可以看出認知障礙的改善程度,而可作為紀錄以及作為驅使該使用者進一步使用的動力。
補充說明的是,本案中對應該等穴位媒介42、該拍頻樂曲、該等近紅外線光燈組51與該等電刺激媒介63的物理刺激所對認知障礙患者帶來的影響的試驗結果,已由以下的參考文獻得到證明,因此本說明書中不再進一步進行說明。

疫系統的功能障礙的證明。
該顯示與光頻刺激單元7能根據該扭力感測器87的回饋,而顯示運動狀況,且能透過顯示提示訊息,以及透過該等揚聲器31發出提示音而得知運動的進度,能提高該使用者使用的便利性。
綜上所述,藉由設置該等揚聲器31、該等穴位媒介42、該等近紅外線光燈組51及該等電刺激媒介63、該顯示與光頻刺激單元7及該運動單元8,使該使用者能於單次療程中同時獲得多重刺激,提高認知障礙患者的復健效率,故確實能達成本發明的目的。
惟以上所述者,僅為本發明的實施例而已,當不能以此限定本發明實施的範圍,凡是依本發明申請專利範圍及專利說明書內容所作的簡單的等效變化與修飾,皆仍屬本發明專利涵蓋的範圍內。

Claims (13)
- 一種認知障礙復健輔助系統,用於供一使用者使用,該認知障礙復健輔助系統包含: 一支架單元,對應於該使用者的頭型,且包括二對應於該使用者的雙耳的耳部、一連接於該等耳部之間且朝上跨過該使用者的頭頂的頂側部、一連接於該等耳部之間且朝後跨過該使用者的後顱的後側部,及一連接於該等耳部之間且朝前跨過該使用者的前側部,該頂側部具有一位於該等耳部間的中心處的頂區,及二分別連接於該等耳部及該頂區間的側區; 一音訊刺激單元,包括二設置於該等耳部的揚聲器,該等揚聲器用於對該使用者的雙耳播送一拍頻樂曲,該拍頻樂曲具有一音頻頻率差; 一穴位刺激單元,包括一設置於該頂區的穴位設置座,及複數穴位媒介,該等穴位媒介用於對該使用者的頭部穴位輸出雷射光的物理刺激,該等穴位媒介的其中一者可調整位置地設置於該穴位設置座相對位於該頂區的中心處以對應該使用者的百會穴,該等穴位媒介的其中一者可調整位置地設置於該穴位設置座且相對位於該頂區的中心處朝前四點五倍的一指寸距離處以對應該使用者的神庭穴,該等穴位媒介的其中兩者可調整位置地設置於該後側部並位於該頂區的中心處朝後七倍的該指寸距離且分別朝該等耳部的方向間隔二點二五倍的該指寸距離處以分別對應該使用者的兩個風池穴,該指寸距離實質上為二點三公分; 一光照射單元,包括二用於發射近紅外線光的近紅外線光燈組,其中一近紅外線光燈組設置於該穴位設置座且環繞對應百會穴的穴位媒介,其中另一近紅外線光燈組設置於該穴位設置座且環繞對應神庭穴的穴位媒介; 一電子刺激單元,包括分別設置於該等側區的一第一設置座及一第二設置座,二分別設置於該第一設置座及該第二設置座的電刺激媒介,該等電刺激媒介用於對該使用者的頭部輸出電流以進行經顱電刺激及輸出電磁脈衝以進行經顱磁刺激的其中一者的物理刺激,該等電刺激媒介分別對應於國際10-20制腦波電極位置中的F3位置及F4位置; 一顯示與光頻刺激單元,設置於該支架單元並可於一顯示模式及一光頻刺激模式之間切換,於該顯示模式時,該顯示與光頻刺激單元顯示一虛擬影像供該使用者的雙眼觀看,於該光頻刺激模式時,該顯示與光頻刺激單元以閃爍畫面刺激該使用者的雙眼;及 一控制單元,電連接該等揚聲器、該等穴位媒介、該等近紅外線光燈組、該等電刺激媒介及該顯示與光頻刺激單元,且儲存有該拍頻樂曲的數位資訊,並可根據一預設命令同時控制該等揚聲器播送該拍頻樂曲、該等穴位媒介發出物理刺激、該等近紅外線光燈組發射近紅外線光、該等電刺激媒介發出物理刺激,及該顯示與光頻刺激單元切換模式。
- 如請求項1所述的認知障礙復健輔助系統,其中,該等穴位媒介的其中四者可調整位置地設置於該穴位設置座且相對位於該頂區的中心處間隔該指寸距離處以分別對應該使用者的四個四神聰穴。
- 如請求項1所述的認知障礙復健輔助系統,其中,該等近紅外線光燈組所發射的光波長介於為760~860nm。
- 如請求項1所述的認知障礙復健輔助系統,其中,該顯示與光頻刺激單元於該光頻刺激模式時,該顯示與光頻刺激單元顯示的閃爍畫面是以一預定頻率閃爍,該預定頻率介於35Hz~45Hz。
- 如請求項1所述的認知障礙復健輔助系統,其中,該顯示與光頻刺激單元於該光頻刺激模式時,該顯示與光頻刺激單元顯示的閃爍畫面是以一光頻頻率差分別刺激該使用者的雙眼。
- 如請求項1所述的認知障礙復健輔助系統,其中,該拍頻樂曲的音頻頻率差介於0.1Hz~45Hz,該等穴位媒介的雷射光波長介於500nm~900nm,輸出功率介於100mW~200mW。
- 如請求項1所述的認知障礙復健輔助系統,其中,該顯示與光頻刺激單元為虛擬影像操作裝置,且包括一顯示模組及一操作按鈕,該控制單元可執行一評量程式,於該評量程式中,該控制單元控制該顯示模組依序顯示複數用於評估認知障礙的認知自評題目,每一認知自評題目具有複數自評選項,並可透過改變該顯示模組的方向後改變所選擇的自評選項,並於透過該操作按鈕的確認後產生對應所選擇的該自評選項的一認知自評分數進行儲存,並於全部的認知自評題目皆作答完畢後加總所有的認知自評分數後產生一認知結果分數。
- 如請求項1所述的認知障礙復健輔助系統,其中,該等電刺激媒介為輸出直流電的型式時,電流大小介於0.5mA~2mA,電流密度介於0.03 mA/cm 2~0.09 mA/cm 2,該等電刺激媒介為輸出電磁脈衝的型式時,電磁頻率介於1-20Hz。
- 如請求項1所述的認知障礙復健輔助系統,其中,每一電刺激媒介具有複數平行設置以輸出物理刺激的導電柱。
- 如請求項1所述的認知障礙復健輔助系統,其中,該支架單元還包括二分別位於該等側區的安裝槽,及複數設置於該頂側部底部的導電條,該第一設置座具有設置於該等側區的其中一者及該等導電條下方的第一滑動件、一自該頂側部相反該第一滑動件的一側穿過各自的該安裝槽並螺鎖於該第一滑動件以用於固定該第一滑動件的第一螺鎖件、一自該第一螺鎖件相反該第一滑動件的一側可滑動地穿過該第一螺鎖件的第一定位件、一設置於該第一滑動件相反該頂側部的一側並螺接於該第一定位件的第一安裝台,及複數可伸縮地穿過該第一滑動件及該第一安裝台且分別電連接該等導電條的第一導電件,該等電刺激媒介的其中一者是設置於該第一安裝台相反該頂側部的一側並電連接該等第一導電件,該第二設置座具有設置於該等側區的其中另一者及該等導電條下方的第二滑動件、一自該頂側部相反該第二滑動件的一側穿過各自的該安裝槽並螺鎖於該第二滑動件以用於固定該第二滑動件的第二螺鎖件、一自該第二螺鎖件相反該第二滑動件的一側可滑動地穿過該第二螺鎖件的第二定位件、一設置於該第二滑動件相反該頂側部的一側並螺接於該第二定位件的第二安裝台,及複數可伸縮地穿過該第二滑動件及該第二安裝台且分別電連接該等導電條的第二導電件,該等電刺激媒介的其中另一者是設置於該第二安裝台相反該頂側部的一側並電連接該等第二導電件。
- 如請求項1所述的認知障礙復健輔助系統,還包含一運動單元,該運動單元訊號連接該控制單元,以供該使用者操作以進行運動。
- 如請求項11所述的認知障礙復健輔助系統,其中,該運動單元包括一健身車,該健身車具有一車架、一可轉動地設置於該車架的曲柄、二設置於該曲柄的兩相反側的踏板、設置於該車架的一把手架及一椅座,及一設置於該曲柄並訊號連接該控制單元以輸出一對應該曲柄的扭力的量測訊號的扭力感測器。
- 如請求項1所述的認知障礙復健輔助系統,其中,該數位資訊及該預設命令是經由雲端網路下載後預存在該控制單元中。
