Description
本發明係有關外科用卡匣。更具言之,本發明係有關使用於眼外科系統的手術卡匣。又更具言之,本發明係有關具有順應性夾持區的手術卡匣。
人類的眼睛可能遭受很多的病變,而造成緩和的劣化至完全喪失視力。雖隱形眼鏡和眼鏡能夠彌補某些輕症,但其它的則需要眼科手術。一般而言,眼科手術係分成後部程序,例如玻璃狀體手術;以及前部程序,例如白內障手術。在最近以來,結合前部與後部的程序已經被開發。
用於眼科手術的手術儀器可被分為專用於前部程序或後部程序者,或可二者兼用者。惟在任何情況下,該手術儀器通常需要使用附帶的耗材,譬如手術卡匣、流體袋、管線、手控器頭及其它耗材等。
一手術卡匣能提供許多的功能,乃視該手術和手術儀器而定。例如,用於白內障手術(如水晶體乳化程序)的手術卡匣可協助管理冲洗和抽取流體進出一手術部位。手術卡匣亦能提供流體袋的支撐,提供一歧管用以導引真空/壓力至手術儀器,以及其它的功能。
該等卡匣通常係在一卡匣承納處連結於該手術儀器。當該卡匣被插入該卡匣承納器時,一夾件會靠抵該卡匣上來將之固持定位。當操作時,該手術卡匣會在該夾持區中承受一相當大的力量。此力乃是該等夾件對抗於被一蠕動泵迫擠該卡匣中央附近所施之力或其它之力所產生的結果。
通常在一卡匣的夾持區中之力並非均勻地分佈。此乃因該夾件與卡匣之間的尺寸容差之差異,或其它會使該夾件與卡匣形成不平行的因素所造成的結果。該力的不均勻分佈會導致該卡匣的夾持區失效,且可能使該卡匣本身造成嚴重的損耗。
因此,乃有需要一種能配佈不均勻負載以防止故障的手術卡匣。
本發明的實施例提供一種外科手術用卡匣具有一夾持部會散佈一夾件所施負載。該夾持部可包含一組凸肋來分配該夾件的負載。在該組凸肋中的各凸肋可由一種材料來製成,其會在一彈性區中彈性地變形,並在達到一材料屈服點時塑性地變形。因此,依據一實施例,該夾持部能夠順應於施加其上的負載輪廓。
本發明的另一實施例可包括一手術卡匣具有一本體部份能容裝一用於眼科手術程序之流體管理系統的至少一部份,一第一夾持部附設於(例如形成一單件的一部份或連接於)該本體部份,及一第二夾持部附設於該本體部份。該等夾持部可被設在該手術卡匣的頂部、底部或側邊。該手術卡匣係可供插入一手術控制台中之一卡匣承納器內,且該等夾持部係被構製成可變形來配佈一將該手術卡匣固持定位於該手術控制台中之夾件所施加的負載。
本發明的實施例會提供一優點,即可協助在一卡匣上提供均勻的負載部份。
本發明的實施例可提供另一優點,即能吸收該夾持機構和卡匣的公差累積,尤其是該夾件與卡匣的不平行。
本發明的實施例會提供另一優點,即若該卡匣上之一流體通道被過度加壓時,將可協助防止該卡匣上的彈性體爆開。該等順應式夾持區可減少或免除以黏劑或任何其它接合方法來確保該等彈性物不會爆脫的需要。

本發明的實施例係提供一種外科手術用卡匣具有順應性夾持區以提供更均勻的負載。依據本發明之一實施例,一手術卡匣包含一夾持區係被構製成可變形而來順應施於該卡匣之不均勻的負載輪廓。該夾持部可包含多數凸肋係由一適當材料定寸製成,而使該等凸肋能在彈性區中可預知地偏轉,且當該材料的屈服點超過時會塑性地流變。使用該等凸肋會造成多數個別的小負載區域,它們將會可預知地偏轉和扭曲,並在高負載時非破壞性地變形。此乃能夠吸收該夾持機構和卡匣之間的累積公差(例如該夾持機構與卡匣之間的不平行),而同時仍可在該卡匣上提供一散佈的負載。
第1圖係為一眼科手術控制台100的實施例之概示圖。該手術控制台100可包含一旋轉監控器110具有一觸控螢幕115。該旋轉監控器110可針對任何需要看到觸控螢幕115的人來被定位在各種方位。旋轉監控器110能由一側擺向另側,以及旋轉和斜傾。觸控螢幕115會提供一圖表式使用者介面(“GUI”),其可容一使用者與該控制台100互動。
該手術控制台100亦包括一連接面板120,係用來供各種工具和耗材連接於手術控制台100。連接面板120可包括,例如,一凝結物連接器,平衡的鹽溶液承接器,各種手控器之連接器,及一流體管理系統(“FMS”)或卡匣承納器125。手術控制台100亦可包含各種方便於使用者的細構,譬如一腳踏板控制器(例收存在面板130後面),及其它的細構等。
在操作時,一卡匣(未示出)可被置入卡匣承納器125中。在手術控制台100內的夾件會將該卡匣夾固定位,以使該卡匣在使用時的移動最小化。該夾件能夠夾住該卡匣的頂面和底面,該卡匣的側面,或夾住該卡匣。
手術控制台100係舉例來提供,且本發明的實施例能被應用於許多手術系統。能夠使用本發明各種不同實施例之卡匣的手術系統例乃包括,例如:Series 2000Legacy白內障手術系統,Accurus400VS手術系統,及InfinitiTM

第2圖係為一卡匣承納器125實施例未裝設一卡匣的概示圖。該卡匣承納器125可具有不同的流體輸入及輸出孔口(概示為135)來由該手術卡匣接收流體(即液體和氣體)。卡匣承納器125可更包含一開孔,以容蠕動泵滾輪140在操作時接觸該手術卡匣。一蠕動泵與對應卡匣的實施例曾被揭述於Sorensen的No.6293926美國專利申請案中,其內容併此附送參考。
在第2圖的實施例中,該手術卡匣會被一具有一底軌142和一頂軌(未示出)的夾件固持於定位。該各軌條可具有數夾指(例如夾指144),它們會在對應的夾持區中接觸該卡匣。一手術卡匣夾件的實施例曾被揭述於No.2003/0202894美國專利公開案中(即Leukanech的No.10/132797申請案),其內容亦併此附送。一釋放按鈕145會被按壓來促發該卡匣由該夾件釋放。視該手術控制台100而定,該卡匣釋放程序可包含數個步驟,包括壓力或流體的排放,夾件的釋脫,或其它步驟等等。第2圖中的構造係舉例提供者。該卡匣承納器125的形成因素,輸入/輸出孔口的設置和數目,及該承納器125的其它特徵細構等,乃可取決於該手術控制台100、要被進行的手術程序、或其它的因素等。
第3圖係為一手術卡匣150實施例的概示圖。卡匣150能提供一封閉的系統流體裝置,其可在一手術程序之後被拋棄。卡匣150可包含一匣體155和與該夾件交接的部份(例如概示於夾持區160和165)會由該匣體155突出。在所示實施例中,該卡匣150是由三個主要區部所形成:一內部或手術控制台介面區部170,其在當卡匣150被插入手術控制台100內時將會面向該手術控制台,及一中間區部175,和一背板180。該卡匣150之各區部可藉由壓裝、互扣凸片、化學接合、熱接合、機械固緊物,或其它在該領域中習知的固接機構來連結在一起。在其它實施例中,卡匣150可由一單件或多數件來形成。
該手術控制台介面部170能提供一介面以供流體流道(例如由一彈性泵膜所提供之蠕動泵的流道177)、閥(例如冲注/抽取閥等)、壓力感測器和其它的特徵細構來管理流體流。該卡匣150亦可附接於一流體袋(未示出)而在一程序中來收集流體。
當操作時,卡匣150會被該等在夾持區中接觸卡匣150的夾軌來定位夾固於卡匣承納器125內。例如,該上夾軌會在夾持區160和165中接觸卡匣150,而底夾軌(例如142)會在類似的底夾持區來接觸卡匣150。
第4圖係為一插入卡匣承納器125中之卡匣150實施例的概示圖。如由第4圖中可見,背板180可包含一手柄181以供單手插入及卸除卡匣150。
第5圖為一插入卡匣承納器125中之卡匣150實施例的截面圖。卡匣150係被一夾件固持於定位。在第5圖的實施例中,該夾件包括下夾軌142和上夾軌182,惟在其它實施例中該等夾件亦能在其它區域接觸卡匣150。當卡匣150最初被插入時,夾軌142/182會旋轉而使各夾指(例如144和184)在該等夾持區中接觸卡匣150。例如,夾指144會在夾持部190接觸卡匣150,而夾指184會在夾持部195接觸卡匣150。該等夾軌142/182可藉以一馬達、以一氣缸,或任何其組合來插入的力量而被促使轉動。要釋放該卡匣時,該等夾軌142/182則會以相反方向旋轉。當插入時,手術控制台介面區部170會接觸手術控制台100,而使例如蠕動泵滾輪140能夠壓擠流道177。
在第5圖的實施例中,該等夾指會將卡匣150推向手術控制台100來將卡匣150固持定位。當在一手術程序中,蠕動泵滾輪140會施加一力於卡匣150上(例如用以壓擠流道177),而在夾持部190和195處造成一更大之力。因為接觸該夾件的夾持部表面可能並不與該夾件平行,故該夾件所施之力可能不會均勻地分佈。例如,若該夾持部195的朝後表面並未與夾指184平行,則該夾指184所施加之力將不會有一均勻的力分佈輪廓。依據本發明的實施例,夾持部195可包含一組凸肋來分配該力,如於後參照第7圖所示。
第6圖係為卡匣150被夾軌182夾住的詳示圖。在第6圖的實施例中,中間區部175會界定匣體155的一部份。夾持部195會由匣體155突出(如第3圖所示)來卡抵夾軌182的夾指184。有一組凸肋會作用來分佈該夾指184所施之力。在第6圖的實施例中,該等凸肋的端面會接觸夾件。但在其它實施例中,該等凸肋可被設在一會接觸該夾件的表面後方。此外,控制台介面區部170亦可包含一夾持部200由該介面區部170的本體部份突出。雖該夾持部200並未接觸夾指184,但夾持部200亦可包含一組凸肋來分佈被施於卡匣150之力。
第7圖為該卡匣150的中間區部175之一實施例的概示圖。中間區部175可包含一本體部份205來界定該匣體155(如第3圖所示)的一部份。本體部份205可包含外壁210和215。在此例中,該本體外壁210係為一頂壁,而本體外壁215係為一底壁。夾持部190會由本體部份205突出,並包含一組凸肋220橫向地列設於外壁215和端壁225之間。同樣地,夾持部195會由本體部份205突出,並包含一組凸肋230橫向地列設於外壁210和端壁235之間。該等凸肋的端面(例如240)在使用時能接觸夾件(例如夾指等)。依據其它實施例,該等凸肋亦可設在一夾件交接壁後方,該交接壁係被構製成可在使用時接觸該夾件。
雖第7圖示出該等凸肋係為均勻間隔分開的凸肋並具有呈矩形的截面,但該等凸肋亦可為不同的佈設和形狀。通常,該等凸肋可由一種材料製成並被成型,而使其當承壓時係呈穩定的。各凸肋可由一種塑膠製成,使得該凸肋在彈性區中將會可預知地變形,且當達到該材料的屈服點時將會塑性地流變。若一凸肋在彈性區中變形,則其它的凸肋將會接抵該夾件(或若已接觸該夾件則會進一步變形)而來分配該負載。同樣地,若一凸肋塑性地變形時,相鄰的凸肋將會觸抵該夾件(或進一步變形)來分配該負載。故本發明的實施例會提供個別的小負載區域,其能可預知地偏轉和扭曲,並在高負載時非破壞地變形。此乃可供吸收累積的公差,而仍可在該卡匣上提供一均佈的負載,因該卡匣會順應於施加其上的負載輪廓。
換而言之,因為該夾件與卡匣可能會由於例如加工和組合的公差而不平行,以致該夾件所施加的負載輪廓可能會不均勻。該卡匣150的夾持部能順應於該負載輪廓(例如藉在較高負載區域中的凸肋更多地變形)來配佈該負載。因此,一不均勻分佈的高負載將能在該夾持部中被均配,而不會使卡匣150造成破壞性的故障。該等凸肋可依據該卡匣將會遭遇的預期負載來被定寸和成型。
雖第7圖示出一中間區部175之一實施例,但類似的夾持部亦可被包含於控制台介面區部170或者卡匣150中。該本體部份205、夾持部190和195等係可為一單件的射出成型塑膠,或被連接在一起或者附設的分開組合物。

Claims (10)
- 一種可供使用於外科手術系統的手術卡匣,其包含:一本體部份,該本體包含一形成該手術卡匣外表面的外壁,以及一夾持部,該夾持部係從該本體部份的周圍突出且被構製成在使用時用以接觸一夾件,該夾持部包含:一端壁,以及一組凸肋,該組凸肋被橫向地設置於該本體部份的外壁與該夾持部的端壁之間,用以分散該夾件所施加的負載。
- 如申請專利範圍第1項之手術卡匣,其中該組凸肋之各凸肋包含一端面,且該夾持部係被構製成可在一或多數的該等端面處接觸該夾件。
