Description

可調式人工脊椎之椎體支架
本案係有關一種人工脊椎之椎體支架,尤指一種用於人體脊椎之頸、胸、腰椎的可調式人工脊椎之椎體支架,該椎體支架具有高(長)度調整,甚至傾斜角度調整之功能。
按椎體置換手術(Vertebral Body Replacement)為目前臨床上常使用可調式人工脊椎之椎體支架(spacer)來治療(替代)被破壞的椎體或腫瘤轉移至椎體的治療方法。藉由骨外科手術將病變的椎體切除,並以人工脊椎之椎體支架執行椎體置換手術,該椎體支架係利用後方椎弓足螺絲固定或側方固定骨板以增加穩定度,以便回復病患脊椎的功能,並減輕病患疼痛和幫助脊椎病變的復原。
目前常用的椎體支架在設計上,可概分為兩類:如圖1a所示係為一種固定式網格狀(mesh cage)椎體支架10,其依據所預置換椎體之長度裁剪,以便將該椎體支架10設置在兩椎體之間,而該椎體支架10顯著之缺失在於,其無法調整高度,故使用時,並需經由裁剪,而耗費手術時間,而且裁減後端面接近水平角的不規則尖突(spIke)與脊椎端板(endplate)接觸之角度(lordosIs angle)無法吻合,容易穿入脊椎端板加速下陷。
如圖1b所示則為一種可調式椎體支架20,其兩接觸端板20a間具有螺紋設計,以作為調整該可調式椎體支架之兩接觸端板20a與脊椎端板接觸截面積的高度,惟該接觸端板20a與人體原有椎體間之融合與固定的穩定性具有舉足輕重的關係,在臨床上的手術失敗問題計有:(1)可調式椎體支架的整組滑脫,而不在兩椎體間之預設位置上;或(2)人工脊椎之椎體支架凹陷或插入人體原有的上、下方椎體間,造成固定失敗與椎體破壞,誠為美中不足之處。
有鑑於此,本案申請人本於多年來從事醫療器材設計及產銷之經驗,潛心研究,期能克服前述固定式網格狀椎體支架與可調式椎體支架的諸多缺失,經再三實驗與測試,進而發展出本案之「可調式人工脊椎之椎體支架」。
本案之目的及所增益之功效在於:為克服原有可調式人工脊椎之椎體支架因定位不佳,而容易造成整組滑脫與椎體破壞之缺陷,進而導致椎體置換手術之失敗;希冀提供一種用於人體脊椎之頸、胸、腰椎的可調式人工脊椎之椎體支架,該椎體支架具有長度調整,甚至傾斜角度調整之功能。
為達成前述之目的,本案所採取之技術手段主要係提供一種可調式人工脊椎之椎體支架,其包括:一本體,其管壁外表面設有外螺紋,及頂面開口向下縱向開設至少二導槽,該本體內部具有一管內空間,底面設有一下接觸端板,該端板底面具有突出物之突伸及至少一骨融合孔;一調整環,其係螺合於本體之外螺紋;一活動件,其係套接於本體之管內空間,且管壁周緣側向延伸至少二導榫係容置於對應之導槽內,並被下方之調整環所支承,該活動件之管內空間上方設有一上接觸端板,該端板頂面具有突出物之突伸及至少一骨融合孔;一止動環,其係螺合於本體之外螺紋;俾藉由旋轉調整環,使其沿著本體移動,進而調整該活動件與本體之間的相對高度,接著旋轉止動環,使其朝向調整環移動,俾與調整環共同包夾活動件位於導槽側向伸出之導榫,從而獲致高度調整及固定者。
本案所採取之另一技術手段係提供一種可調式人工脊椎之椎體支架,其包括:一本體,其管壁外表面設有外螺紋,及頂面開口向下縱向開設至少二導槽,該本體內部具有一管內空間,底面設有一下接觸端板,該端板底面具有突出物之突伸及至少一骨融合孔;一調整環,其係螺合於本體之外螺紋;一活動件,其係套接於本體之管內空間,且管壁周緣側向延伸至少二導榫係容置於對應之導槽內,並被下方之調整環所支承;一止動環,其係螺合於本體之外螺紋;一上接觸端板,其係樞接於活動件之管內空間頂部呈可擺動狀,活動件調整,且該端板頂面具有突出物之突伸及至少一骨融合孔;俾藉由旋轉調整環,使其沿著本體移動,進而調整該活動件與本體之間的相對高度,接著旋轉止動環,使其朝向調整環移動,俾與調整環共同包夾活動件位於導槽側向伸出之導榫,從而獲致高度調整及固定;而上接觸端板擺動調整後,從而獲致傾斜角度調整者。
為進一步揭示本案之具體技術內容,首先請參閱圖式,其中,圖1a及圖1b係習知兩款人工脊椎之椎體支架立體圖,圖2為本案可調式人工脊椎之椎體支架之立體分解圖,圖3為本案組立後之立體圖,圖4為本案組立後之側視圖,圖5為本案進行高度調整之使用狀態示意圖,圖6為本案進行傾斜角度調整之使用狀態示意圖,圖7為本案實際運用於兩椎體間之示意圖,圖8為本案可調式人工脊椎之椎體支架第二實施例之立體分解圖,圖9為本案第二實施例組立後之立體圖,圖10為本案第二實施例組立後之側視圖。
如圖2至圖4所示,基本上,本案之可調式人工脊椎之椎體支架係用於人體之頸、胸、腰椎失去支撐人體的功能,其係設於以手術切除病變的脊椎椎體與上下相鄰接的椎間盤後所清出的空間內,俾藉由該可調式人工脊椎之椎體支架替代切除之脊椎椎體,使其具有支撐人體的功能,其中該可調式人工脊椎之椎體支架係由一本體1,一調整環2,一活動件3,及一止動環4所組合而成。
其中,本體1係以金屬,例如鈦合金製成之管體,惟不以此為限,該金屬材料隨著生醫材料科學的進步,亦得以非金屬材料,例如PEEK(聚醚醚銅)材料製成管體。該本體1之管壁11外表面具有外螺紋111之設置,且間隔地環狀排列複數個透孔112,且本體1可依不同長度及管體尺寸增加透孔112之排列數及數量。該本體1底面設有一下接觸端板12,其底面得突伸突出物121,例如複數個尖刺,如圖4所示,各尖刺121以角錐形或圓錐形為佳,俾藉由該等尖刺121刺入下方脊椎椎體以增加穩定度。再者,該接觸端板12除具有複數個尖刺121設置之外,尚具有至少一骨融合孔(因投影角度未示,請參閱後敘之上接觸端板5)之配置,其可依實際需求形成尺寸較大之單一孔洞,或者尺寸較小之複數個孔洞。而本體1內之管狀空間13則可將容置經絞碎自體、異體移植骨漿或填充生醫材料,例如骨水泥或人造骨,使骨漿得以在管內空間13生長,或將生醫材料充斥於管內空間13內部,藉以提高支撐能力,並可使骨漿或生醫材料可通過下接觸端板12之骨融合孔,而與相鄰下方脊椎之椎體接觸,以方便骨融合之進行。
而該本體1之創作特點在於,該管壁11周緣由頂面開口向下縱向開設至少二導槽14,如圖2所示,該本體1係在管壁11等角開設四個導槽14,以利於後敘活動件3可在該管內空間13縱向升降,藉以調整該本體1與活動件3之高度。此外,該本體1管壁開設至少一比透孔112尺寸更大置入孔113。如圖1所示係為三角孔形,惟不以此為限,其亦得為四方孔或圓孔,俾當椎體支架植入脊椎之兩椎體之間,醫師調整高度至定位後,使管狀空間13、33加大後,醫師可透過該置入孔113補充骨漿或生醫材料。
調整環2係為一環體,其內徑對應於本體1之管壁11外徑,且為使該調整環2得以接裝於該本體1上,故於其內徑設有內螺紋21,以便可螺合於該本體1之外螺紋111。再者,為使前述之骨漿或生醫材料得以通過調整環2,而灌注於本體1之管內空間13,是以,其周緣等角預設複數個環孔22,以利灌注用具可通過環孔22及透孔112,而將骨漿或生醫材料灌注於管內空間13。此外,該調整環2頂面側向延伸一支撐面23,用以支撐該活動件3容置於各導槽14內之導榫32。
活動件3係為一套管,其管壁31外徑係對應於本體1內徑,以利於套接於管內空間13,其中,該管壁31亦可間隔地環狀排列複數個穿孔311,以利於通過該等穿孔311灌注骨漿或生醫材料於管壁31內所圍組之管內空間33。而該活動件3之創作特點在於,該管壁31周緣對應本體1之導槽14位置各橫向延伸一導榫32,其係容置於對應之導槽14內,並被其下方調整環2之支撐面23所支承。如欲調整該活動件3與本體1間之高度,僅需旋轉該調整環2,使其支撐面23沿著本體1縱向升降,從而令位於各導槽14內之導榫32縱向位移,使該活動件3可由圖4所示之最短狀態伸展置如圖5所示之最高狀態。
止動環4亦為一環體,其內徑對應於本體1之管壁11外徑,且為使該止動環4得以接裝於該本體1上,故於其內徑設有內螺紋41,以便可螺合於該本體1之外螺紋111,並縱向移動。該止動環4設置之目的,係在於與調整環2共同包夾該導榫32,使該活動件3藉由該調整環2調整至所欲的高度後,然後反向旋轉該止動環4,使其朝向該等導榫32移動,直到與調整環2共同包夾該等導榫32,即可將該活動件3固定在所欲之高度位置。其中,該止動環4之管壁11外周緣設有彼此相鄰接之複數個銑面42,以利使用工具旋轉該止動環4。
為提供該活動件3與上方脊椎之椎體形成接觸,藉以增加穩定度,是以,該活動件3上方設有一上接觸端板5,如圖2所示,該上接觸端板5頂面得突伸突出物51,例如複數個尖刺,各尖刺51以角錐形或圓錐形為佳,俾藉由該等尖刺51刺入相鄰上方脊椎之椎體以增加穩定度。再者,該接觸端板5除具有複數個尖刺51設置之外,尚具有至少一骨融合孔52之配置,其可依實際需求形成尺寸較大之單一孔洞,或者尺寸較小之複數個孔洞。理應瞭解的是,該活動件3之管內空間33係容置骨漿或生醫材料,並可將骨漿或生醫材料通過上接觸端板5之骨融合孔52,而與相鄰上方脊椎之椎體接觸,以方便骨融合之進行。
尤有進者,由於吾人各脊椎之椎體傾斜角度因人而異,所以,本案進一步提供具有傾斜角度調整功能之上接觸端板5,該端板5係為一蓋體,俾可套接及樞接於活動件3頂部,呈可擺動狀。俾當活動件調整定位,該上接觸端板5自動調整角度配合上方脊椎椎體底部的角度相吻合。如圖2所示,該端板5側壁相對開設一對軸孔53,而活動件3頂部亦相對開設一對接孔34,並將一對樞軸54穿越對應之軸孔53而樞設及固定於接孔34,使該上接觸端板5相對於活動件3為可擺動狀態,亦即如圖6所示之調整傾斜角度狀態。
如圖7所示,係在病患脊椎之腰椎的第三椎體因病變被手術切除,其第二、三椎體與第三、四椎體間的椎間盤也一併切除之。接著,以椎體置換手術方式植入本案可調式人工脊椎之椎體支架,接著旋轉調整環2,使其支撐面23沿著本體1縱向升降,從而令位於各導槽14內之導榫32縱向位移,使該活動件3於本體1頂部開口縱向移動,而調整至所欲高度後,然後反向旋轉止動環4,使其朝向該等導榫32移動,直到與調整環2共同包夾該等導榫32,即可將該活動件3固定在所欲之高度位置,使上、下接觸端板5、12分別抵靠於相鄰之椎體,且該上接觸端板5可因應相鄰錐體之傾斜角度而自動調整因應,並使突出物51、12,例如複數個刺狀結構可局部刺入相鄰之椎體內,即可令該椎體支架替代所切除之椎體,而使其具有支撐人體的功能。是以,使用本案之可調式人工脊椎之椎體支架除了可避免脊椎之椎體下陷外,且具防止整組椎體支架滑脫之優點,又可達到骨融合的效果。
如圖8至圖10所示,係本案可調式人工脊椎之椎體支架第二實施例之示意圖,其中,本實施例之一本體1,一調整環2,一活動件3,及一止動環4的大部分元件均與第一實施例相同,故相同之元件名稱均以相同標號代表之,是以,各元件間之結合關係在此不擬贅述。而本實施例之差異在於,該上接觸端板5係與活動件3採固定方式結合,而非以樞接方式連接。因此,該上接觸端板5具有一傾斜面55之設計。
該傾斜面55之創作特點在於:當脊椎角度越大時,尤其是第五節腰椎,在臨床的手術後,習知的可調式人工脊椎之椎體支架容易滑脫失效。因此,如圖10所示,若該上接觸端板5朝向脊椎前後方向的傾斜面55角度越大,則該等突出物51,例如線形斜向棘齒刺入椎體深度越深,俾藉由前述特殊的傾斜面設計來提高穩定度。經由實測該傾斜角度以不大於16度為佳,以提供不同病患的脊椎角度之選擇。
此外,本案之調整環2之支撐面23係採用棘輪231設計,其可與活動件3之導榫32形成單向抵住功能,據以提供該調整環2防滑的效果。而該棘輪231設計當然可運用於前述之第一實施例之調整環2。由於本實施例之上接觸端板5係固定於活動件3一端,因此,其並無法提供傾斜角度調整之功能,而僅能提供高度調整的功能。
所以,經由本案之實施,其相較於先前技術,藉由旋轉該調整環,使其沿著本體移動,進而調整該活動件與本體之間的相對高度,接著旋轉該止動環,使其朝向該調整環移動,俾與該調整環共同包夾該活動件位於導槽側向伸出之導榫,從而獲致高度調整及固定之功能;另兩端之突出物,例如複數個刺狀結構有利於醫師於手術時植入兩脊椎端板之間,除了可防止可調式人工脊椎之椎體支架滑脫外,並可有效避免椎體支架之凹陷或坍塌,又可達到骨融合的效果。再者,本案之活動件頂部可樞接一上接觸端板,其擺動調整後,可因應相鄰錐體之傾斜角度,從而與之吻合地密接。尤有進者,該本體之管狀空間係用來填充自體、異體移植骨漿或生醫材料,且周圍之圓孔設計則方便骨融合及骨骼生長,誠為同類物品前所未見之一大佳構。
本案所揭示者,乃較佳實施例之一種,舉凡局部之變更或修飾而源於本案之技術思想而為熟習該項技藝之人所易於推知者,俱不脫本案之專利權範疇。
綜上所陳,本案無論就目的、手段與功效,在在顯示其迥異於習知之技術特徵,且其首先創作合於實用,亦在在符合發明之專利要件,懇請 貴審查委員明察,並祈早日賜予專利,俾嘉惠社會,實感德便。

Claims (18)

  1. 一種可調式人工脊椎之椎體支架,其包括:一本體,其管壁外表面設有外螺紋,及頂面開口向下縱向開設至少二導槽,該本體內部具有一管內空間,底面設有一下接觸端板,該端板底面具有突出物之突伸及至少一骨融合孔;一調整環,其係螺合於本體之外螺紋;一活動件,其係套接於本體之管內空間,且管壁周緣側向延伸至少二導榫係容置於對應之導槽內,並被下方之調整環所支承,該活動件之管內空間上方設有一上接觸端板,該端板頂面具有突出物之突伸及至少一骨融合孔;一止動環,其係螺合於本體之外螺紋;俾藉由旋轉調整環,使其沿著本體移動,進而調整該活動件與本體之間的相對高度,接著旋轉止動環,使其朝向調整環移動,俾與調整環共同包夾活動件位於導槽側向伸出之導榫,從而獲致高度調整及固定者。
  2. 如申請專利範圍第1項所述之可調式人工脊椎之椎體支架,其中該本體與活動件之管壁周緣開具複數個透孔及穿孔,以便與本體與活動件之管內空間相連通,而調整環周緣亦開具複數個環孔。
  3. 如申請專利範圍第1項所述之可調式人工脊椎之椎體支架,其中該上、下接觸端板之突出物係為一尖刺結構。
  4. 如申請專利範圍第3項所述之可調式人工脊椎之椎體支架,其中該尖刺結構係為角錐形或圓錐形。
  5. 如申請專利範圍第1項所述之可調式人工脊椎之椎體支架,其中該本體及活動件之管狀空間係放置經絞碎自體、異體移植骨漿或填充生醫材料。
  6. 如申請專利範圍第1項所述之可調式人工脊椎之椎體支架,其中該調整環頂面側向延伸一支撐面,用以支撐該活動件容置於各導槽內之導榫。
  7. 如申請專利範圍第6項所述之可調式人工脊椎之椎體支架,其中該支撐面之頂面係為一棘輪。
  8. 如申請專利範圍第1項所述之可調式人工脊椎之椎體支架,其中該上接觸端板頂面具有一不大於16度之傾斜面。
  9. 如申請專利範圍第2項所述之可調式人工脊椎之椎體支架,其中該本體之適當位置開設至少一尺寸大於透孔及穿孔之置入孔,以利補充骨漿或生醫材料。
  10. 一種可調式人工脊椎之椎體支架,其包括:一本體,其管壁外表面設有外螺紋,及頂面開口向下縱向開設至少二導槽,該本體內部具有一管內空間,底面設有一下接觸端板,該端板底面具有突出物之突伸及至少一骨融合孔;一調整環,其係螺合於本體之外螺紋;一活動件,其係套接於本體之管內空間,且管壁周緣側向延伸至少二導榫係容置於對應之導槽內,並被下方之調整環所支承;一止動環,其係螺合於本體之外螺紋;一上接觸端板,其係樞接於活動件之管內空間頂部呈可擺動狀,且該端板頂面具有突出物之突伸及至少一骨融合孔;俾藉由旋轉調整環,使其沿著本體移動,進而調整該活動件與本體之間的相對高度,接著旋轉止動環,使其朝向調整環移動,俾與調整環共同包夾活動件位於導槽側向 伸出之導榫,從而獲致高度調整及固定;而上接觸端板擺動調整後,從而獲致傾斜角度調整者。
  11. 如申請專利範圍第10項所述之可調式人工脊椎之椎體支架,其中該本體與活動件之管壁周緣開具複數個透孔及穿孔,以便與本體與活動件之管內空間相連通,而調整環周緣亦開具複數個環孔。
  12. 如申請專利範圍第10項所述之可調式人工脊椎之椎體支架,其中該上、下接觸端板之突出物係為一尖刺結構。
  13. 如申請專利範圍第12項所述之可調式人工脊椎之椎體支架,其中該尖刺結構係為角錐形或圓錐形。
  14. 如申請專利範圍第10項所述之可調式人工脊椎之椎體支架,其中該本體及活動件之管狀空間係放置經絞碎自體、異體移植骨漿或填充生醫材料。
  15. 如申請專利範圍第10項所述之可調式人工脊椎之椎體支架,其中該調整環頂面側向延伸一支撐面,用以支撐該活動件容置於各導槽內之導榫。
  16. 如申請專利範圍第15項所述之可調式人工脊椎之椎體支架,其中該支撐面之頂面係為一棘輪。
  17. 如申請專利範圍第10項所述之可調式人工脊椎之椎體支架,其中該上接觸端板側壁相對開設一對軸孔,而活動件頂部亦相對開設一對接孔,並將一對樞軸穿越對應之軸孔而樞設及固定於接孔,使該上接觸端板相對於活動件為可擺動之傾斜角度調整狀態。
  18. 如申請專利範圍第11項所述之可調式人工脊椎之椎體支架,其中該本體之適當位置開設至少一尺寸大於透孔及穿孔之置入孔,以利補充骨漿或生醫材料。
