本發明係關於一種有抗沖刷能力之可塑型的骨科組合物,其包括:一粉末組合物,其包括半水硫酸鈣、三鈣磷酸鹽和羥丙基甲基纖維素;及一溶劑,其包括甘油和水。該骨科組合物具有良好的抗沖刷效果。還提供該骨科組合物的製備方法,以及包括該組合物之骨填物補套組。
Description
本發明係關於一種骨填補材料,特別係關於一種有抗沖刷能力之可塑型的骨科組合物。
硫酸鈣以長久使用於幫助骨修復,因其具有良好的骨傳導性與生物相容性,主要作為空腔的填補物,它能幫助恢復骨的形態輪廓,並阻止軟組織的入侵,其降解的速率可以配合新骨生成的速率,並且幫助血管的浸潤。當硫酸鈣置於缺損處時,表面解離出的鈣離子達到特定的濃度梯度會與體內正常存在的磷酸根離子結合,產生磷酸鈣的沉澱。在骨骼修復的過程中,磷酸根沉澱產生二次的骨引導性,使成骨細胞可以貼附。硫酸鈣開始降解時周圍組織的pH值會呈現弱酸性,會造成周為骨骼組織的去礦物化(demineralisation),而生長因子會伴隨著去礦物化的現象而釋放,產生骨誘導性,促使附近幹細胞分化,細胞開始生長後礦化,產生新骨[1]。
硫酸鈣依照結晶水多寡又可分為二水硫酸鈣 (Calcium Sulfate Dihydrate, CaSO4。2H2O)、半水硫酸鈣 (Calcium sulfate hemihydrate, CaSO4。0.5H2O)和無水硫酸鈣 (Calcium sulfate Dehydrate)三種,二水硫酸鈣可稱為生石膏,半水硫酸鈣又可稱為熟石膏。其中二水硫酸鈣和半水硫酸鈣皆可使用在骨填補材料中,但硫酸鈣若沒經過特殊處理或加入添加物,在體內很快就會被降解,無法提供缺損處初始支撐能力與細胞攀附,因此許多研究著重於開發硫酸鈣複合材料,以使硫酸鈣成為更良好的骨填補材。(1)硫酸鈣添加α-三鈣磷酸鹽(α-tricalcium phosphate, α-TCP):硫酸鈣和α-TCP混合後,可變成穩定的結構,使得降解時間長達1-2年,而混合物中的硫酸鈣降解後可產生孔洞,有助於新生骨長入[2-3]。(2)硫酸鈣加入氫氧基磷灰石(hydroxylapatite, HAp): 氫氧基磷灰石是人體骨骼中主要的無機成分,具有良好的生物相容性,其鈣磷比為1.67與骨骼的礦物組成非常相似,可與骨骼產生鍵結,成為骨的一部分,但限制是機械強度過低,故與硫酸鈣為複合材料時,硫酸鈣的比例提升會使植入物的機械強度增加[4],而硫酸鈣降解時,也會產生孔洞,使新成骨長入。 (3)半水硫酸鈣加入海藻酸鈉、甲殼素、甲基纖維素或透明質酸:加入這些添加物都可使半水硫酸鈣的降解時間增加,以及具有良好的操作性質,雖然有時機械強度會降低(例如添加海藻酸鈉或甲殼素時),但若比例調配得當,例如添加比例大於7.5%甲基纖維素或5%透明質酸,則可使抗壓強度上升,使材料更加穩定[5-6]。(4)半水硫酸鈣添加去細胞骨基質: 去細胞骨基質是可以提供生物相容性與骨傳導性外,還具有骨誘導性與生長因子[7],但其主要缺點是機械強度差、傳染風險與取得來源等問題,故添加半水硫酸鈣的複合骨材可以提供初始的機械強度,具有初期血管浸潤的效果[8]。
市售骨填補材料中,硫酸鈣產品多以注射型占大多數,磷酸鈣以錠狀、片狀、柱狀、注射型等劑型皆有,也有一些生物玻璃(矽酸鈣)、高分子材料(聚乳酸、聚羥基乙酸)被添加於骨填補材中。通常注射型或水合式的材料,在使用前需要將粉末與液體混合成糊狀後,添加至注射容器或較為稠狀像黏土的可以配合患部的形狀,塑型成想要的形狀後填入。
在混合的過程中,可能會遇到以下兩個問題[9]:
混合過程粉末與水沒有混均勻的現象,此時可能會造成材料性質變化,使原定的固化時間、注射性質、機械性質有所改變。其中,目前市售產品的混合方法又有所不同,如:A產品是需要在規定時間一分鐘內,將粉末與水混合均勻後,等待三分鐘,才能夠填充至患處。B產品需要將粉末與水浸泡三分鐘後,在進行混合一分鐘後,才能夠填充至患處。市售產品並沒有統一的混合規範,所以會造成容易醫生的混淆。
醫生必須在材料固化前植入病患體內,如果固化時間過快,會喪失注射性質;如果固化時間過慢,則會增加手術時間的進行,使病患添加手術時的風險。
故有預混合的劑型產生,預混合劑型泛指醫生在開起包裝時,不需進行煩雜的粉末與水混合,便可直接進行填補,可以省略粉末與水的混合時間,不僅可以方便醫生的使用,也可以降低病患的風險。
現今台灣市售骨填補材中,僅有吉萊骨粉有提供預混合的可塑型骨填補材,但在臨床上發現此產品會引起病患的發炎反應,甚至在植入處會形成發炎囊腫[10-11]。目前推測是當吉萊骨粉中的硫酸鈣降解後,會降低植入處的pH值,造成材料液化(liquefaction)後產生碎屑,進而引發發炎反應[11]。
因此開發出一種具有較長及較好的抗沖刷能力的可塑型的骨填補材料仍有其需要。
有鑑於上述的問題,本發明人係依據多年來的研究經驗,針對骨科組合物進行相關研究與改進;緣此,本發明之主要目的在於提供一種有抗沖刷能力之可塑型的骨科組合物及其製備方法。在本發明的部分實施例中,提供一種具有良好的抗沖刷性的可塑型的骨科組合物。
根據本發明的目的,本發明係關於提供一種可塑型的骨科組合物,其包括:一粉末組合物,其包括半水硫酸鈣(CaSO
4‧0.5H
2O)、三鈣磷酸鹽(β-tricalcium phosphate, β-TCP)和羥丙基甲基纖維素(Hydroxypropyl Methylcellulose, HPMC);及一溶劑,其包括甘油(Glycerol)和水。
較佳地,其中該半水硫酸鈣與該三鈣磷酸鹽之比例約為1:1(克/克)。
較佳地,其中該羥丙基甲基纖維素的重量百分比係為1~6%。
較佳地,其中該羥丙基甲基纖維素的重量百分比係為1~4%。
較佳地,其中該甘油的體積百分比係約為70~99%。
較佳地,其中該甘油的體積百分比係約為85%。
根據本發明的另一目的,進一步提供一種骨填補物套組,其包含儲存於單獨容器中如本發明所述之可塑型的骨科組合物。
根據本發明的另一目的,進一步提供一種可塑型的骨科組合物用於製備治療骨缺損之醫療用途。
本發明將藉由下列較佳實施例及配合之圖式,作進一步知詳細說明。需注意的是,以下各實施例所揭示之實驗數據,係為便於解釋本案技術特徵,並非用以限制其可實施之態樣。
本說明書全文中,術語「約」用於指明一數值包括材料比例的誤差、藥物濃度值的誤差、或者存在於實驗對象之間的變異。典型地該用語是指涵蓋大於或小於1%、2%、3%、4%、5%、6%、7%、8%、9%、10%、11%、12%、13%、14%、15%、16%、17%、18%、19%或20%的變異性,視情況而定。
本說明書全文中,所述「骨科組合物」的具體態樣例如但不限於關節植入物、脊柱植入物、顱頜面植入物、牙齒植入物、足踝植入物或創傷植入物(骨板、骨釘)等。
本說明書全文中,所述之「經注入(注射)」、「注入(注射)」或「可注射性」等用語包括任何組合物之投予,如注射、浸入或通過任何輸送裝置傳送至個體。
本發明係之目的係提供一種可塑型的骨科組合物,其包括:一粉末組合物,其包括半水硫酸鈣(CaSO

2O)、三鈣磷酸鹽(β-tricalcium phosphate, β-TCP)和羥丙基甲基纖維素(Hydroxypropyl Methylcellulose, HPMC);及一溶劑,其包括甘油(Glycerol)和水。
根據本發明之一具體實施例,其中半水硫酸鈣與三鈣磷酸鹽之比例約為1:1(克/克)。
根據本發明之一具體實施例,其中羥丙基甲基纖維素的重量百分比在粉末組合物中係約為1~6%,較佳約為1~4%,最佳約為2~4%。當羥丙基甲基纖維素的重量百分比超過6%時,本發明之骨科組合物會過度黏膩,造成操作性差,無法成為可塑型之骨科組合物。
根據本發明之一具體實施例,本發明之粉末組合物與溶劑的液粉比最佳約為0.35 毫升/克。本發明之粉末組合物之半水硫酸鈣之重量百分比係約為47、47.5、48、48.5、49、49.5%,較佳約為48、48.5、49、49.5%、最佳約為48、48.5、49%;三鈣磷酸鹽之重量百分比係約為47、47.5、48、48.5、49、49.5%,較佳約為48、48.5、49、49.5%、最佳約為48、48.5、49%;羥丙基甲基纖維素之重量百分比係約為1、2、3、4、5、6%、較佳約為1、2、3、4%、最佳約為2、3、4%。
根據本發明之一具體實施例,其中甘油的體積百分比在溶劑中係約為70~99%,較佳約為85%。當甘油的體積百分比低於70%時,本發明之骨科組合物易於崩解無法有效成型,一天內會崩解無法成型。
本發明所述之骨科組合物,可是需要進一步加入用以使粉劑以均勻分散之分散劑、用以維持粉劑粒子懸浮狀態之懸浮安定劑、藥劑(例如,骨細胞成/分化促進劑、血管新生促進劑等)、營養劑、抗菌劑、抗生素或添加劑(例如固化促進劑、固化遲滯劑等)。
本發明之另一目的係提供一種骨填補物套組,其包含儲存於單獨容器中如本發明所述之可塑型的骨科組合物。習知目前常用之骨填補注射器,其注射口之內徑約3mm,故而上述半水硫酸鈣、三鈣磷酸鹽和羥丙基甲基纖維素之粒徑大小,亦會影響骨科組合物於注射時之流動性。為使本發明之骨科組合物具有較佳之流動性,使其易於被擠出。可應用於本發明之半水硫酸鈣較佳為具有20~40μm之平均粒徑、三鈣磷酸鹽較佳為具有20~40μm之平均粒徑和羥丙基甲基纖維素較佳為具有100~200μm之平均粒徑。
本發明之另一目的係提供一種可塑型的骨科組合物用於製備治療骨缺損之醫療用途。
本發明將藉由參考下列的實施例做進一步的說明,這些實施例並不限制本發明前面所揭示之內容。熟習本發明之技藝者,可作些許之改良與修飾,但仍不脫離本發明之範疇。

骨科組合物的配製方法為依據表1所示之重量百分比,將半水硫酸鈣(平均粒徑20~40μm)、三鈣磷酸鹽(平均粒徑20~40μm)和羥丙基甲基纖維素(平均粒徑100~200μm)的粉末充分混和後,依表1所示之溶劑的體積百分比,將甘油與水充分混和,並於室溫下加入於粉末中,用藥匙充分攪拌均勻,以形成一具有抗沖刷能力之可塑形的骨科組合物。
沖刷實驗方法:
可塑型骨科組合物放置在304不鏽鋼模具(直徑6公釐,高度3公釐)中,以製成圓柱體狀並秤重。之後浸泡在含模擬體液的圓筒底部中心,圓筒直徑為20公釐,高度為20公釐,並放置在攝氏37度的環境。骨科組合物和模擬體液的比例為0.2克骨填補材/1毫升模擬體液,使用模擬體液的原因是因其離子濃度和人類血漿相似[12]。骨科組合物浸泡了15分鐘,1小時或4小時之後,移除模擬體液,加入1毫升之99.5%酒精,並放置在室溫30秒。接著移除酒精,將未崩解之骨填補材主體(直徑6公釐之圓柱體),以及崩解之碎屑放入攝氏50度之烘箱3天,秤量未崩解之骨填補材主體,以實驗前的重量扣除未崩解骨填補材主體之重量取得重量流失,再除以實驗前的重量得到沖刷百分比,因此沖刷百分比越低,代表該配方的抗沖刷能力越好。

由表1及表2的數據中可以看出,當加入羥丙基甲基纖維素(HPMC)後,在每個骨科組合物中皆能夠有效降低骨科組合物的百分比,且隨著羥丙基甲基纖維素的百分比增加,骨科組合物的抗沖刷能力越好。如圖1、2、3所示,羥丙基甲基纖維素能夠顯著的提升骨科組合物長時間的抗沖刷能力,又以85%的甘油的組別中最具有顯著的提升效果。

由表4中可以看出即使沖刷時間長達4小時,羥丙基甲基纖維素都能夠有效的提升每個骨科組合物的抗沖刷能力,且隨著甘油的比例的降低,更能有效的提升骨科組合物的抗沖刷能力。

如表5所示,當甘油的比例低於70%時,即甘油為50%或30%時,骨科組合物沒辦法保存超過1天,即1天內會崩解而無法成型。而當甘油比例高於70%時,骨科組合物具有可塑型的能力,且能夠保存於常溫中至少21天以上。
不同濃度的羥丙基甲基纖維素對骨科組合物之塑型能力的影響如表6所示。

如表6所示,隨著羥丙基甲基纖維素的重量百分比的提升,骨科組合物的黏度也逐漸上升,造成可操作性下降。當羥丙基甲基纖維素的濃度達到6%時,骨科組合物會像口香糖一樣黏著,造成骨科組合物不易塑型。
實施例4
本發明之骨科組合物與市售之骨科組合物的比較如表7所示。

如表7所示,與市售之古萊骨粉(GeneX
®Bone Graft Substitute)相比,本發明之骨科組合物在抗沖刷能力上略高於古萊骨粉。顯示本發明之骨科組合物具有良好的抗沖刷能力及可塑性。
綜上可知,硫酸鈣已廣泛使用於骨填補材料,其優點是有良好的生物相容性、無毒性、可被生物吸收與優良的骨傳導特性。但硫酸鈣在植入體內後,容易因血水沖刷而無法停留在需填補之處,則無法達到修復功能。因此本發明之一有抗沖刷能力之可塑型的骨科組合物,以硫酸鈣與磷酸鈣組合物(四鈣磷酸鹽與二鈣磷酸鹽)為主體,其添加檸檬酸與羥丙基甲基纖維素後,可達到有效的抗沖刷效果,具備更良好的修復功能。

圖1所示為皆含70%甘油(Glycerol)的骨科組合物,但含不同比例的羥丙基甲基纖維素(HPMC),浸泡在模擬體液中15分鐘或1小時之沖刷百分比。使用t檢定統計,**P<0.01為同樣配方,浸泡15分鐘和1小時相比的差異。
圖2所示為皆含85%甘油(Glycerol)的骨科組合物,但含不同比例的羥丙基甲基纖維素(HPMC),浸泡在模擬體液中15分鐘或1小時之沖刷百分比。使用t檢定統計,*P<0.05或**P<0.01為同樣配方,浸泡15分鐘和1小時相比的差異。
圖3所示為骨填補材皆含99%甘油(Glycerol),但含不同比例的羥丙基甲基纖維素(HPMC),浸泡在模擬體液中15分鐘或1小時之沖刷百分比。使用t檢定統計,浸泡15分鐘和1小時相比皆無統計上的差異。

Claims (9)
- 一種有抗沖刷能力之可塑型的骨科組合物,其包括: 一粉末組合物,其包括半水硫酸鈣、三鈣磷酸鹽和羥丙基甲基纖維素;及 一溶劑,其包括甘油和水。
- 如請求項1所述之可塑型的骨科組合物,其中該半水硫酸鈣與該三鈣磷酸鹽之比例約為1:1(克/克)。
- 如請求項1所述之可塑型的骨科組合物,其中該羥丙基甲基纖維素的重量百分比係為1~6%。
- 如請求項1所述之可塑型的骨科組合物,其中該羥丙基甲基纖維素的重量百分比係約為1~4%。
- 如請求項1所述之可塑型的骨科組合物,其中該甘油的體積百分比係約為70~99%。
- 如請求項1所述之可塑型的骨科組合物,其中該甘油的體積百分比係約為85%。
- 如請求項1所述之可塑型的骨科組合物,其中該粉末組合物及溶劑的液粉比約為0.35 毫升/克。
- 一種骨填補物套組,其包含儲存於單獨容器中如請求項1~7所述之可塑型的骨科組合物。
- 一種如請求項1~6所述之可塑型的骨科組合物用於製備治療骨缺損之醫療用途。
