Abstract

無功能性垂體腺瘤(NFPA)是良性腺垂體腫瘤,與激素分泌過多的臨床證據無關。NFPA主要是促性腺激素垂體腺瘤,約佔所有垂體腫瘤的35% (14-54%)。它們的患病率為7-41.3/100,000人,標準化發病率為0.65-2.34/100,000;發生高峰為從40歲至80歲

NFPA通常在出現「腫塊效應」的體徵和症狀時被診斷出來,例如頭痛、視力障礙及/或由壓迫和損傷延伸至海綿竇和鞍底引起的顱神經功能障礙。
3此外,一些病例是通過為其他目的進行的影像學研究偶然診斷出來的。由於正常前垂體的壓迫和垂體柄偏斜,也可能分別存在腦垂腺低能症和高乳促素血症。
目前大多數NFPA的標準第一線療法是內視鏡檢查或顯微鏡輔助經蝶骨手術和經顱手術,第二種主要用於蝶鞍上腫瘤。

手術治療後NFPA通常會進展,僅接受手術治療的NFPA患者的再生率為15-66%,而接受手術後放射療法的NFPA患者的再生率為2-28%。

已有三類主要的肽被研究用於治療NFPA:多巴胺受體2 (dopamine receptor 2,DR2)促效劑、體抑素促效劑(SSA)及促性腺激素釋放激素(gonadotropin-releasing hormone,GnRH)類似物。此外,一些中心已引入替莫唑胺(temozolomide)作為侵襲性腫瘤的治療方法。
DR2促效劑(卡麥角林(cabergoline)和溴麥角克普汀(bromocriptine))的功效與受體的表現相關,一些研究證明只有在手術後立即給藥時才有效。
並沒有證據證實GnRH類似物的療效。此外,在某些情況下,當GnRH促效劑用於治療同時患有促性腺激素腺瘤的轉移性前列腺癌患者時,會加劇腫瘤生長無變化之促性腺激素的分泌或誘發垂體中風。

如在實驗部分中詳細的揭露,本案發明人驚訝地發現,ITF2984是一種新穎環狀SSA泛促效劑六肽,與第一代SSA相比具有更高的SSTR3結合親和力及改良特性,可有效治療MENX(同基因型突變體)NFPA大鼠模型中的NFPA,其與人類對應物非常相似

在ITF2984的分子表徵過程中,發明人發現雖然在結構上類似於其他泛促效劑,例如帕瑞肽和奧曲肽,但與已知分子相比,ITF2984對SSTR3具有更高的親和力。分子模型顯示,ITF2984中較高的β-II'轉角機率與較高的SSTR3親和力相關,從而為該分子的獨特選擇性模式提供了結構原理。
特別是,獲得的活體外結果令人驚訝地顯示,在兩種不同的細胞株中,ITF2984比帕瑞肽或奧曲肽更有效地誘導SSTR3內化及磷酸化,以及藥理學上相關濃度範圍內的GIRK活化。基於這些資料,ITF2984因此可被認為是SSTR3受體的完全促效劑,可促進受體內化。
多項研究報告稱,SSTR3在促性腺激素腺瘤中頻繁且強烈地表現,而SSTR2僅在少數患者中表現,而SSTR5僅在例外情況下表現。

經體抑素(SST)和SSA的SSTR3活化藉由通過活化蛋白酪胺酸磷酸酶及隨後的Raf1及MAPK失活來干擾有絲分裂途徑,從而誘導細胞生長抑制及細胞毒性作用。此外,SSTR3參與被提議通過p53及凋亡蛋白酶活化來誘導細胞凋亡。SSTR3的靶向還抑制內皮細胞增生,從而抑制新血管生成。

2)垂體腺瘤對SSA的反應取決於特定SSTR亞型的表現,如在GH分泌腺瘤中的SSTR2所見。

實驗章節中報導的活體內研究所獲得之結果證實了此點,清楚地表明ITF2984有效抑制小鼠腫瘤生長並誘使強烈減少NFPA細胞的增生。
與發現的受體親和力概貌及獲得的活體外資料一致,因此ITF2984顯示出選擇性抗腫瘤活性。
此抗腫瘤活性伴隨著經ITF2984處理之腫瘤的增生指數(KI67陽性)下降。此外,ITF2984在雌性大鼠中選擇性地誘導SSTR3 mRNA表現,表明存在代償性上調。
因此,獲得的資料與SSTR3的活體內參與及ITF2984在此模型中主要由SSTR3驅動的抗腫瘤活性相符,並為ITF2984在NFPA和其他經SSTR3驅動的疾病中潛在臨床用途提供了概念上活體內的證明,諸如表現SSTR3的腫瘤(即胰臟腫瘤、嗜鉻細胞瘤、副神經節瘤、肺癌和乳癌)及纖毛病變。
最近,報導了MENX模型中帕瑞肽及奧曲肽的活性
28,發明人將這些資料與本文報導的ITF2984的結果進行比較。在已發表的報告中,帕瑞肽顯示比奧曲肽具有更高的抗腫瘤活性,這在雌性和雄性大鼠中都很明顯,並且在雌性大鼠中具有更高活性的趨勢。這種增強的活性歸因於SSTR3受體的參與。
我們注意到這些數據不同於我們使用ITF2984的觀察結果。事實上,ITF2984僅在雌性大鼠中表現出顯著的放射學腫瘤反應和SSTR3 mRNA的代償性上調,而在SSTR3水平較低的雄性大鼠中未觀察到顯著的抗腫瘤作用。我們根據ITF2984的更具選擇性的活性來解釋這些數據,若不是完全,則主要是在測試的實驗條件下由SSTR3參與介導。
亦觀察到被治療的動物在治療結束時並未表現出不適的跡象。ITF2984的安全性亦在臨床前安全性研究、在正常健康志願者的兩項第一期臨床試驗及在肢端肥大症患者的第二期試驗中得到證實,顯示出在耐受劑量下的有效性(39,40)。

實驗部分 ITF2984 結構
ITF2984 (圖1(A)中所示之式),為一種新穎SST泛促效劑環狀六肽,其係在一個藥物化學項目中發現的,該項目旨在識別與第一代SSA相比具有改進特性的泛促效劑。相對於其他已知的體抑素類似物(如帕瑞肽及奧曲肽),該化合物對於SSTR1、2、3及5顯示出高結合親和力,對所有受體的IC
50值都在奈莫耳範圍內。IC
50值來自一項代表性實驗。來自3個獨立實驗的信賴區間(表1)。
當與奧曲肽相比,ITF2984對SSTR1、SSTR3及SSTR5顯示較高的親和力,對SSTR2顯示較低的親和力,而相對於帕瑞肽,其對人類SSTR1、SSTR2及SSTR3呈現較高的親和力(表1)。

使用分子模擬方法來合理化ITF2984對SSTR3具有更高親和力的結構基礎。為此,使用來自GPCRdb
28的SSTR結構,GPCRdb為G偶聯蛋白受體結構的開放存取庫(open access repository)。受體SSTR1-4模型主要基於κ-類鴉片受體(或KOP,序列相似性60-66%,pdb代碼6B73及6VI4分別表示活性和非活性構形異構物),而SSTR5更類似於δ-類鴉片受體。在此模擬訓練中使用類鴉片受體的相關性已藉由ITF2984及帕瑞肽單一劑量結合測定在此特定的GPCR家族中得到實驗證實,其中兩種六肽都能在10
-5M時完全取代促效劑(表2)。

為了解釋ITF2984與帕瑞肽對於SSTR2及SSTR3的不同行為,我們最初使用SST14構形異構物探索體抑素受體相互作用,該構形異構物源自SST14在5% D-甘露醇溶液中的NMR研究(十種幾何結構中的第一個構形異構物,PDB代碼2MI1)。SST14與SSTR3很好地對接,ZDOCK得分最高的姿勢與跨膜螺旋TM5、TM6及TM7相互作用,且與Phe7明顯接近連接TM4及TM5 (EL4-5)的細胞外環。不幸的是,由於TM4與TM5之間的細胞外環較長,因此無法對SRIF-14和SSTR2執行等效的對接計算:此環狀構形阻礙了由SSTR2中的跨膜螺旋所界定之內部空間的入口(圖2)。
SSTR3模型中的SST14結合幾何結構具有一系列感興趣的特徵:
i) Lys9錨定至Asp123,且當κ-類鴉片(PDB代碼6B73)的活性形式疊加至SSTR3 - SST14最佳對接姿勢複合物時,ζ氮原子佔據KOP促效劑MP1104的NH
4 +基團的相同區域,
ii) Trp8適合由Leu100、Val299、Tyr295及Phe273分隔的疏水性袋形結構,埋在局部非共價鍵網絡中,
iii) 配位體殘基Phe7-Trp8-Lys9-Thr10 產生扭曲的β-II'轉角。
在最有可能的SRIF-14結合構形異構物中發現的扭曲的β-II'轉角表明,顯示此類二級結構模體的更高穩定性的肽促效劑可能是較佳的。對帕瑞肽及L-363,301的NMR研究
29表明,相對於其他環狀六肽,前者具有更高的柔韌性,且更大的自由度被認為是帕瑞肽能夠更好地適應所有受體結合位點的原因。
ITF2984的類似實驗(圖3)顯示ITF2984相對於帕瑞肽的β-II'轉角機率更高,這是由於在軌跡分析中檢測到的分子動力學快照數量更多(30%,而不是12%)。化合物ITF2842中的β-II'轉角機率更高(58%)。比較所有三種分子在SSTR1-3及SSTR5上的結合結果(表3)顯示β-轉角的穩定性及效力(和選擇性)與SSTR3之間的相關性。該β-轉角的穩定性越高,SSTR3的選擇性就越高。
我們得出結論是有一個合理的結構假設可以解釋ITF2984對SSTR3的親和力增加。

我們進一步表徵了ITF2984的生物活性:此分子有效抑制大鼠垂體前葉初代細胞培養物中的GH釋放,與帕瑞肽相比沒有統計學差異(圖4)。ITF2984進一步抑制戊巴比妥(pentobarbital)誘導的大鼠GH釋放並降低大鼠和犬的IGF1程度。
為了突顯ITF2984與其他SSTR促效劑之間的機制差異,我們使用兩種不同的實驗模型研究SSTR內化:
以人類SSTR轉染的HEK293細胞及以人類GFP標記受體(SSTR2-tGFP、SSTR3-tGFP、SSTR5-tGFP)轉染的U2OS細胞。SST14及SST28分別作為參考化合物被包括在第一個和第二個實驗中。
在轉染的HEK293細胞中,ITF2984誘導SSTR3的強烈內化,這與SST14誘導的內化相當,並且顯著高於奧曲肽或帕瑞肽誘導的內化(圖5A,5B)。相反地,僅觀察到SSTR2及SSTR5的部分內化。
在U2OS細胞中,與未處理的對照相比,所有化合物都以劑量依賴性方式增加SSTR2-tGFP的內化,奧曲肽是最有效的內化誘導劑,其次是帕瑞肽和ITF2984。在SSTR5-tGFP轉染的細胞中,由於以SSA處理而導致的受體內化增加與SST的效果相似,儘管較低(約為SST28的百分之五十)(表4,圖6)。

最後,在SSTR3-tGFP
-轉染細胞中,ITF2984是最有效的受體內化誘導劑,在1 μM時顯示出與SST28相當的增量。在相同濃度下,奧曲肽及帕瑞肽的作用顯然不太顯著。
我們得出結論是在兩種不同的細胞株中,ITF2984比帕瑞肽或奧曲肽更有效地誘導SSTR3內化。
使用磷酸化位點特異性抗體評估 SSA 誘導的人類 SSTR2 、 SSTR3 及 SSTR5 受體活化
其次,使用針對以下殘基的磷酸化位點特異性抗體藉由西方墨點法測試SSA誘導的人類SSTR的活化:對於SSTR2:S341/pS343、T353/T354、T356/T359;對於SSTR3:S337/T341、T348;對於SSTR5:T333。
30ITF2984以劑量依賴性方式(圖7B,7C)和藥理學相關的奈莫耳劑量範圍誘導SSTR3的完全磷酸化,而帕瑞肽僅在超藥理學、微莫耳劑量範圍內誘導微弱作用。在SSTR2中,兩種環狀六肽都誘導S341/S343的選擇性磷酸化,而SST14及奧曲肽導致所有研究位點的SSTR2完全磷酸化(圖8B、8C)。最後,SSTR5 pT333磷酸化位點僅部分受到所有測試化合物的影響,帕瑞肽顯示出最顯著的影響(圖9B、9C)。

因此,在以hSSTR3轉染的HEK293細胞和在AtT20野生型以及hSSTR3轉染的細胞中,使用基於GIRK的螢光膜電位測定來研究SSTR促效劑介導的G蛋白訊號傳導。

此外,在小鼠激腎上腺皮質素細胞腫瘤AtT-20細胞中分析藉由奧曲肽及ITF2984介導之G蛋白訊號傳導,該細胞內源性表現GIRK1/2通道以及SSTR2及SSTR5 受體。SSTR3受體的外源性表現導致ITF2984介導的劑量反應曲線向左偏移,但以奧曲肽獲得之劑量反應曲線則無,此表明在這些條件下,ITF2984參與SSTR3,而奧曲肽則無(圖11)。這些資料與以hSSTR3轉染之HEK293細胞株所獲得的資料一致。
ITF2984 在活體內對內源性 NFPA 的療效
受到活體外研究結果的鼓舞,我們決定在MENX (同基因型組合突變體)大鼠模型中研究ITF2984的活體內抗腫瘤活性,這是唯一一種自發的內源性模型,其中NFPA與其人類對應物非常相似,並且具有完全的基因表現率。
16,17發現在此模型中開發的NFPA亦概括了其人類對應物的SSTR表現模式,顯示出高SSTR3表現。有趣的是,發現MEX大鼠垂體瘤中的SSTR3水平具有性別特異性,在女性中觀察到更高的表現。這種模式亦可擴展至人類NFPA。兩種性別的大鼠都用以ITF2984或安慰劑治療56天,並使用高解析度磁振造影(MRI)縱向監測腫瘤生長。
以ITF2984治療之雄性大鼠和對照組成員顯示出相對腫瘤體積的快速增加,(圖12)呈現出最適合具有二次時間效應的線性混合效應模型(LME)之對數值。相比之下,以ITF2984治療之雌性大鼠顯示治療期間腫瘤體積僅略有增加,表明呈線性生長。經ITF2984治療之大鼠性別之間的時間斜率差異顯著(p值=0.0251)。這表明與雄性相比,雌性突變大鼠對ITF2984的反應明顯更好。值得注意的是,雖然在研究開始時兩隻雌性大鼠的腫瘤已經相對較大,但ITF2984控制了腫瘤的生長,並且這兩隻動物在治療結束時沒有表現出任何不適的跡象。當結合兩種性別時,經ITF2984治療的大鼠(用作藥物反應的代表)與對照大鼠相比,腫瘤生長的總體減少,如藉由最合適的LME評估,是適度的。然而,當分別分析性別時,與安慰劑治療的雌性大鼠相比,經藥物治療的雌性大鼠表現出腫瘤生長受到抑制,表明對ITF2984有反應。
ITF2984 對 NFPA 增生率的影響
在治療結束時,收集垂體組織用於離體分析。將以安慰劑或ITF2984治療的大鼠的所有腫瘤進行Ki67染色,並計算每100.000 μm
2中Ki67陽性細胞的數量。經安慰劑處理的(對照)NFPA顯示每區域平均有375個(雄性)及312個(雌性)Ki67陽性細胞,如所報告的(圖13)。並不驚訝的是,在對照組中,雄性與雌性的Ki67陽性細胞數量與絕對腫瘤體積之間存在正向趨勢。在以ITF2984處理的大鼠腫瘤中,Ki-67陽性細胞的數量下降至雄性大鼠的平均250 (-33.5%)及雌性大鼠的平均134 (-57%) (圖13)。經安慰劑處理及經ITF2984處理的雌性大鼠之間,Ki67陽性細胞數量的差異是顯著的(p= 0.031)。腫瘤細胞增生的變化與MRI確定的腫瘤體積變化相關:ITF2984在雌性中比在雄性中更有效地抑制腫瘤生長,這與前者中NFPA細胞增生的強烈減少一起發生。
大鼠 NFPA 中 SSTR 的表現
經由定量RT-PCR測量每個轉錄本的拷貝數(絕對定量),在治療結束時評估大鼠腫瘤中各種
Sstr基因的表現程度。還使用免疫組織化學(IHC)以針對SSTR1、2、3及5的抗體對來自2個動物組別的腫瘤進行染色,而結果證實雌性大鼠中SSTR3的高表現
31。ITF2984的投予導致兩種性別中
Sstr5表現的下調,以及
Sstr3mRNA的增加,但僅在雌性中(圖14)。在雌性大鼠中,SSTR3的這種更高的表現解釋了ITF2984在雌性中的更大作用。
尚未證實SSTR3在人類女性中的表現在統計學上也顯著更高。
與奧曲肽及帕瑞肽相比, ITF2984 對 NT-3 細胞株球體的活性 方法 -NT-3 細胞株球體的生成和處理
NT-3分化良好的胰腺神經內分泌(PanNET)細胞株(32)是胰臟腫瘤的活體外模型。
該細胞在含有10% FBS、1%青黴素/鏈黴素、20 ng/ml EGF (AF-100-15,Peprotech)、10 ng/ml FGF (100-18B,Peprotech)的RPMI 1640 Medium GlutaMAX™中培養。
藉由在96孔ULA 盤(Corning)中接種2000個細胞/孔來生成3D球體。對於每種條件複製四次接種。
球體形成需要4天,之後開始處理。
ITF2984、帕瑞肽及奧曲肽的儲備溶液已在100% DMSO中製備,工作溶液在含有0.5% DMSO的完全生長培養基中以2560、1280、640、320、160、80、40、20 nM製備。
藉由含有2X所需藥物濃度的新培養基更換50%的培養基(以避免去除球體),每3天重複一次處理。
在處理期間每天監測球體形態,並在第0、4、7、11、14、18天拍攝照片。第0天對應於處理的第一天。每個時間點都從同一孔中拍攝照片。
結果 -ITF2984 對 NT-3 球體的細胞毒性活性
最近開發的NT-3細胞株是一種分化良好的PanNET,表現高程度的SSTR,尤其是SSTR3 (32)。
與奧曲肽和帕瑞肽相類似,ITF2984在劑量反應中測試其對NT-3球體的細胞毒性活性。追踪球體生長/存活18天。
如圖15所示,從第7天開始,ITF2984在濃度≥640 nM時減少了球體的體積。在2.56 µM,球體被完全破壞。同樣的現像在經奧曲肽及帕瑞肽處理過的球體中並不太明顯,它們在最高濃度下仍保持其形態(圖16及圖17)。
因此,考慮在NT-3球體中所觀察到的細胞毒性活性對應於抗腫瘤活性,該結果可證實ITF2984可用作治療胰臟癌更有效的藥物。

Claims (14)
- 一種治療表現SSTR3的腫瘤使用的式(I)化合物或其醫藥上可接受的鹽及/或溶劑合物, 。
- 如請求項1使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其用於治療無功能性垂體腺瘤、及選自胰臟腫瘤、嗜鉻細胞瘤、副神經節瘤、肺癌或乳癌之神經內分泌相關惡性腫瘤。
- 如請求項1或2使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其被每日投予至患者。
- 如前述請求項中任一項使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其被投予至人類。
- 如請求項1或2使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其以0.5至5 mg/die,較佳為0.2至2.5 mg/die,更佳為0.1至2 mg/die之範圍的量投予至患者。
- 如前述請求項中任一項使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其以含有其與至少一種生理學上可接受的賦形劑的醫藥組成物之形式投予至患者。
- 如前述請求項中任一項使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其中該醫藥組成物藉由經口、舌下、直腸、血管內、靜脈內、皮下途徑投予,較佳為藉由靜脈內途徑投予。
- 如前述請求項中任一項使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其中該醫藥組成物為固體或液體形式。
- 如請求項8使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其中該固體形式選自粉末、錠劑、顆粒劑、凝聚物、壓製丸劑或包衣丸劑、硬膠囊或明膠膠囊。
- 如請求項8使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其中該液體形式為懸浮液、糖漿或注射液。
- 如前述請求項中任一項使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其與至少一種活性成分組合投予至患者。
- 如前述請求項中任一項使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其中該至少一種活性成分選自胰島素促分泌物、胰島素分泌促進劑、胰島素增敏劑、低胰島素劑量、具有多巴胺受體2促效作用之藥劑、具有抗血管生成作用的藥劑、或化學治療劑。
- 如請求項12使用的化合物或其醫藥上可接受的鹽及/或溶劑合物,其中該具有胰島素增敏劑活性之藥劑為二甲雙胍(metformin);該具有胰島素促分泌物之藥劑為磺醯脲類或腸泌素類藥物,選自維格列汀(vindagliptin)及那格列奈(nataglinide);該具有胰島素分泌促進劑之藥劑為GLP-1促效劑,該藥劑較佳為利拉魯肽(liraglutide)或艾塞那肽(exenatide);該具有多巴胺受體2促效作用之藥劑為卡麥角林(cabergoline)或溴麥角克普汀(bromocriptine);且該化學治療劑為替莫唑胺(temozolomide)。
