200940049 六、發明說明： 【發明所屬之技術領域】 本發明係關於式（I)之阿戈美拉汀或iV-[2-(7-曱氧基-1-萘 基）乙基]乙醯胺：
NHCOMe (I), ϋ 及其水合物、結晶形式及醫藥學上可接受之酸或鹼之加成 鹽單獨或組合於製得用於治療雙極性病症、尤其I及II型雙 極性病症且更尤其I型雙極性病症之藥物的用途。 【先前技術】 阿戈美拉汀或#-[2-(7-曱氧基-1-萘基）乙基]乙醯胺具有 雙重特徵，一方面其為褪黑激素能系統受體之促效劑，且 另一方面其為5-HT2C受體之拮抗劑。此等特性使其具有中 樞神經系統之活性，且更特定言之使其具有治療重性抑鬱 參 症、季節性情感障礙、睡眠障礙、心企管病狀、消化系統 病狀、時差引起之失眠症及疲勞、食慾障礙及肥胖症之活 性。 阿戈美拉汀、其製備及其治療用途已揭示於歐洲專利說 明書 EP 0 447 285及 EP 1 564 202 中。 本發明申請者現已發現單獨或組合使用之阿戈美拉汀或 iV-[2-(7-曱氧基-1-萘基）乙基]-乙醯胺，及其水合物、結晶 形式及醫藥學上可接受之酸或鹼的加成鹽具有頗具價值之 140681.doc 200940049 特性，允許其用於治療雙極性病症、尤其型雙極性病 症且更尤其I型雙極性病症❶ ' 雙極性病症係對於患者之生命、社會與家庭水平及職業 水平均具有極其嚴重影響之精神病理學病症。其特徵在於 由週期分開之抑鬱、躁狂、輕度躁狂或混合發作的通常反 覆出現，在此期間，患者先驗未受重性心理功能障礙影_ 響。換言之’罹患雙極性病症之患者的特徵在於易受週期 性出現之顯著情緒波動的影響。該等發作 之特徵使得若干種臨床形式得心分：1型雙極性病=〇 最典型且特徵在於通常伴有重性抑鬱發作之一或多次躁狂 或是合發作’就其而言’ 雙極性病症涉及至少一種重 性抑鬱發作及輕度躁狂發作、中度形式之躁狂的組合。雙 極性，V者中之自殺危險極高，25至50%之患者曾進行過至 少一次自殺嘗試。 目前尚無令人滿意、公認之雙極性病症治療法。首要治 療通常為情緒穩定劑或情感調㈣（thy歸egulat吵但此 等治療通常無法減輕抑鬱症狀(J。如ps—y，，❹ 65(4)，569·579)。儘管經常使用抗抑鬱劑之伴隨處方，但 由於k抑鬱劑可觸發或加重躁狂狀態及混合狀態，且當躁 狂發作出現時必須停止使用，故其為頗有爭議之習慣。 尤其認為抗抑鬱劑促進情感增盛週期之產生或加速，且 兩至三次促進躁狂或輕度躁狂之出現，且最終其之長期使 用似乎導致抑t及躁狂發作次數之增加。 "’、卜的疋本發明申請者現已發現阿戈美拉·;丁可單 140681.doc 200940049 獨或組合用於治療雙極性病症，尤其1及11型雙極性病症且 更尤其I型雙極性病症。 由於阿戈美拉汀對抑鬱具活性之事實，故使用阿戈美拉 汀對雙極性病症且更尤其〗型雙極性病症之預期作用與文 獻中所述之抗抑鬱劑，例如帕羅西灯（par〇xetine)，的作用 相同。 令人意外的是，如已在對罹患=型雙極性病症之患者所 進行之臨床研究過程中所觀察到的，阿戈美拉汀與習知抗 抑鬱劑表現不同。此等結果使得可能考慮其在治療雙極性 病症，尤其I及II型雙極性病症且更尤其〗型雙極性病症中 之使用’甚至其之長期使用。 【發明内容】 因此’本發明係關於阿戈美拉汀及其水合物、結晶形式 及j藥學上可接受之酸或驗的加成鹽單獨或組合於製得用 於治療雙極性病症，尤其I及π型雙極性病症且更尤其I型 雙極性病症之醫藥組合物的用途。 本發明亦係關於阿戈美拉汀與情緒穩定劑或情感調節劑 之組合用於製得用於治療雙極性病症，尤其1及π型雙極性 病症且更尤其I型雙極性病症之醫藥組合物。 【實施方式】 根據本發明之情緒穩定劑或情感調節劑（thymoregulator) 係關於鐘及諸如痛痙寧（carbamazepine)、丙戊酸鹽 (valproate)及拉莫三嗓（iani〇trigine)等之抗癲癇藥。 更特定言之’根據本發明之組合的情緒穩定劑或情感調 140681.doc 200940049 節劑將為鐘或丙戊酸納。 當阿戈美拉汀與情緒穩定劑組合時， 之前、同時或之後投藥，其限制條件為兩:=:=: 允許在中_經系統中獲得_之協同效應。在同 時投與根據本發明之組合之兩種組份㈣Μ 1 與與-或多種醫藥學上可接受之賦形劑組合之包含阿戈美 拉>丁及情緒穩定劑的醫藥組合物。 、 該等醫藥組合物將以適於經口、非經腸、經皮、鼻、直 腸或經舌途經投藥之形式且尤其以可注射製劑、錠劑、舌 7劑、細ette、明膠膠囊、膠囊、口含劑、检劑、乳 骨、油膏、皮膚凝膠等之形式存在。 :阿戈美拉汀及視情況與其組合之情緒敎劑外，根據 明之醫藥組合物包含選自以下物質之一或多種赋形劑 或載劑：稀釋劑、潤滑劑、黏合劑、崩解劑、吸收劑、著 色劑、甜味劑等。 藉助非限制性實例可提及： ♦作為稀釋劑：乳糖、右旋糖ϋ甘露醇、山梨糖 醇、纖維素' 甘油， H潤滑劑：石夕石、滑石、硬脂酸及其鎮鹽與辦鹽、聚 乙二醇， 作為黏合劑：矽酸鋁及矽酸鎂、澱粉'明膠、黃耆膠、 甲基纖維素、羧曱基纖維素鈉及聚乙烯咄咯啶酮， 作為崩解劑：瓊脂、褐藻酸及其鈉鹽、發泡混合物。 適用之劑量將根據患者之性別、年齡及重量，投藥途 I40681.doc 200940049 經、病症特性及任何組合療法而變化，且在每24小時1坩 至5 0 mg阿戈美拉、;丁之範圍内變化。 阿戈美拉汀之日劑量將較佳為每天25 mg。 醫藥組合物： 製備1000粒各含有25 mg活性成份之錠劑的配方。 曱乳基-1-萘基）乙基1乙醯脸 乳糖單水合物..................... ..................25 g 硬月旨酸鎂...................... 62 g 聚乙烯吡咯酮(Povidone).. 1·3 g 無水矽膠......................... ..................9g 羥乙酸纖維素鈉.................. ..................0.3 g 硬脂酸.............................. ^ _ Mg
此研究係在21名惟患I型雙極性病症之患者中進行丄专 等患者經鋰（n=14)或丙戊酸鈉（n=7)治療至少6個月。以f 戈美拉汀（25毫克/天）進行治療6週。在此等患者中，有B 名在急性治療6週後繼續治療，且其中之丨1名持續八療長 達1年。所獲得之結果展示81%之患者對該治療作出積極 反應-約50%在治療恰好一週後作出反應。治療6週後僅觀 察到三次躁狂或輕度躁狂發作，與用情緒穩定劑所遇到之 彼等水平完全一致，此清楚地表明甚至在長時間之治療 後，阿戈美拉汀對I型雙極性患者之積極作用亦與躁狂及/ 或輕度躁狂發作之發生率的增加無關。 140681.doc
