Восстанавливают целость анатомического ложа. Таким же образом выдел ют дл пересадки и удлин ют дистальный конец сухожили полусухожильной или нежной мыш с внутренней стороны бедра. Через линейный разрез кожи по передней поверхности коленного сустава обнажают надколенник, распиливают его во фронтальной плоскости, в этой же плоскости производ т расщеп сзаожшш четырехглавой мышцы бедра на 4-6 см вверх в задней створке надколенника формируют костный способ. В подкожной клетчатке из первой и второй рай в направлении третьей формируют . В них провод т сухожили пересаживаемых мышц. Голень сгибают до угла 130-150°, проведенные сухожили нат гивают до тех пор, пока рассто ние между контролыа1ми метками на мышце и коже не cTaiieT равным 1-1,5 см. Затем пересаживаемые сухожили ушивают кетгутовыми швами в расщеп С5осожили четьфехглавной мышцы, надколенника и между собой. После этого голень выпр мл ют, надколенник смеша ют максимально вниз, дистальные концы пересаживаемых сухожилий провод т через толщу собственной св зки надколенника. Последнюю укрепл ют путем гофрировшш живой сухожильной тканью. Раны послойно ушивают наглухо. Накладьтают большую тазобедренную пов зку на 6 недель. После сн ти гипсовой пов зки провод т курс консервативного лечени (массаж мышц, ЛФК, теплые ванны, меди каментозные препараты), направленаого на выработку новой функции пересаженных мыш и улучШешм функции оперированной конечности . П р и м е р 1. Больной Я. С., 14 лет, диагноз: детский церебральный паралич, Ш1рамидна форма, сгибательно-привод щие контрактуры в суставах нижних конечностей. Вьшолнена операци двухсторонней адцукто ропластики, закрытой ахшшоп астики, пересадки длинной головки двуглавой и нежной мыищ слева. Линейный разрез длиной 12 см по задненаружной поверхности левого бедра д головки малоберцовой кости. Выделена и мобилизована длинйа головка двуглавой мьшдхы бедра, дистальна часть ее сухож1ти удтшнена на 6 см за счет фасциального раст жени голени. На верхний край операционной раны на кожу прошита цветна шелкова лигатура. Подобна лигатура из кетгута прошита на мы шечное брюш1со на одном уровне. Конец сухожили отсечен. Восстановлено анатомическое ложе. Подобный разрез кожи по зашгевнутрен ней поверхности левого бедра. Вьщелено сухожилие полусухожильной мышцы, дистальный конец его удлинен на 5 см за счет сухожильного раст жени поверхностей гусиной лапки . Аналогичным образом наложены контрол ные нитки на кожу и мышцы. Сухожилие 4 отсечено. Третий- разрез кожи по передней по верхности коленного сустава длиной 10 см. Мобилизован надколенник, распилен во фронтальной плоскости, в этой же плоскости расшеплено сухожилие квадрицепса вверх на . 8 см. В задней створке надколенника сформирован костный желоб, в который проведены и уложены вз тые дл пересадки сухожили длинной головки двуглавой и полусухожильной мышц. Последние подшиты кетгутом в расшеп квадрицепса и надколенника при согнутой до 130° голени. Контрольна метка на мышце не дот нута до таковой на коже на 1,5 см. Голень полностью вьшр млена, надколенник максимально смешен вниз. Дистальные концы пересаженных сухожилий проведены через толшу собственной св зки надколенника с ее гофрированием и укреплены в костный канал бугристости большеберцовой кости. Раны послойно ушиты наглухо. Через 4 недели сн та гипсова пов зка, продолжена выработка новой функции пересаженных мышц. В насто шее врем ребенок ходит самосто тельно , контрактуры в суставах нижних конечностей устранены, функци оперированных конечностей хороша . Результатами операции больной и родители довольны. Пример 2. Больна К. И., 22 лет диагноз: детский церебральный паралич, спастический нижний парапарез, сгибательно-привод щие контрактуры в суставах нижних конечностей . Произведена операци : двухсторонн аддукторопластика, закрыта двухсторонн ахиллопластика, пересадка длинной головки двуглавой-и полусухожильной мышц к бугристости большеберцовой кости справа. Во врем операции использован предложенный способ пересадки сухожилий мышц при детском церебральном параличе. Послеоперационный период протекал гладко. Результатами операции больна довольна. Пример 3. Больной А. А., 16 лет, диагноз: детский церебральный паралич, спастический тетрапарез, слибательно-привод шие контрактуры в суставах нижних конечностей. Произведена операци : двухсторошш закрыта ахиллолластика, двухсторонн аддукторопластика , пересадка полу сухожильной длинной головки двуглавой мышц к бугристости большеберцовой кости слева. Во врем операции применен предложенный способ пересад си сухоукклкЛ мышц. Результаты операции хорошие . Таким образом, предложенный способ поэвол ет увеличить объем активного .сгибани в коленном суставе за счет создани необходимого запаса длины пересаживаемых сухожилий и восстановить конгруентность суставных поверх1юстей надколенника и бедренной кости.A linear incision is made along the posterior posterior thigh surface from the border of the middle and lower third to the head of the fibula 14 cm long. Patient A. A., 16 years old, diagnosed with cerebral palsy, spastic tetraparesis, positive-limiting contractures in the joints of the lower extremities. The operation was carried out: the double-sided closed achillo-plastic, double-sided adductoplasty, transplantation to the floor of the tendonal head of the biceps to the tibial tuberosity on the left. Thus, the proposed method makes it possible to increase the amount of active flexion in the knee joint by creating the necessary reserve of length of the transplanted tendons and to restore the congruence of the articular surfaces of the patella and femur.
