При преобладании в клинической картине больного симптомов тревоги, иав зчивостей, страхов, суицидальных мыслей терапевтически значимыми дл данного больного вл ютс левосторонние воздействи . Они в большинстве случаев вызывают отчетливое психическое и мышечное расслабление, успокоение, улучшение ночного сна, а также (в первые 2-3 сеанса) сопровождаютс некоторым повышением фона настроени и психической активности. При этом левосторонние воздействи следует планировать более частыми (например, 4 процедуры из 7) и более длительными (5 мин вместо 2-3), чем правосторонние. Кроме того, желательно с них начать и ими заканчивать курс воздействий, а также они должны преобладать среди процедур, проводимых дл профилактики последующих обострений. Следует учитывать, что повтор емые левостонние воздействи за счет возможного углублени В1ыраженной релаксации могут, сн в тревогу, углубить тоску, печаль, вызнать слабодушие, обильные слезы. При преобладании в клинической картине у больного симптомов снижени .настроени от печали до тоски, идеаторно моторной заторможенности, апатии, в лости и т. д. следует начинать и заканчивать курс процедур с правосторонних воздействий. Они редко провоцируют релаксацию, чаще вызывают стимул цию психики, что сопровозждаетс повышением активности, бодрости , работоспособности, а также вызывают под-Ьем настроени . Если первые процедуры привод т к ожидаемому эффекту, то удлинение времени воздействи в последующем нецелесообразно. В других случа х можно удлин ть терапевтически значимые латеральные воздействи и Укорачивать противоположные по эффекту и стороне воздействи . Наконец, в тех случа х, когда ожидаема ремисси не наступает, латеральными воздействи ми можно видоизменить клиническую картину за счет усилени одних и ослаблени других компонентов заболевани . Левосторонние воздействи , например, целесообразны в тех случа х, когда в лечебном арсенале врача имеетс набор различных антидепрессантов и психостимул торов , но нет эффективных анксиолитиков и транквилизаторов. Релаксировав и погасив тревогу с помощью данного способа, можно продолжить лечение антидепрессантами и психостимул торами. Клиническими показани ми дл проведени лечени предлагаемым способом вл ютс прежде всего- патологи эмоциональной сферы при пограничных состо ни х (неврозы и психопатии) и при эндогенных психозах (шизофрени и маниакально-депрессивный психоз), в частности депрессии различной глубины, тревога, ипохондричность, суицидальные тенденции, различные страхи н нав зчивости, моторна заторможенность, астено-апатические влени , а также цсихич еские нарушени , возникающие в результате применени психофармакологических препаратов и т. п. «госпитализма на этапе становлени терапевтической ремиссии. Данный способ полностью совместим с другими биологическими способами лечени (психофармакологические средства, инсулиновые шоки) или могут пользоватьс в разных сочетани х. Имеютс противопоказани дл той или иной латеральности воздействий в зависимости от конкретного сопутствующего патологического фона. Так, дл левосторонних воздействий противопоказанием вл ютс различные заболевани сердечно-сосудистой системы, очаги хронических инфекций, дл правосторонних воздействий относительным противопоказанием вл ютс заболевани правосторонних париых и непарных органов и систем. Первые леВые и все правосторонние воздействи могут способствовать некоторому повышению артериального давлени (систолического и диастолического ), поэтому гипертоиическа болезнь вл етс относительным противопоказанием дл способа. С осторожностью следует проводить лечение предлагаемым способом лиц с сопутствующими латеральными ирриатативными эпнлептифорн ыми очагами в головном мозгу . Лиц с эпилептической болезнью лечить им не следует. Дл лечени детей и подростков , у которых еще не завершилось развитие функциональной асимметрии головного мозга , этот способ непригоден. , Предлагае мый способ обеспечивает положительный эффект при депресси х с зат жным течением, плохо купируемых с помощью обычных психофармакологических средств, легко переноситс , а также восстанавливает терапевтическую чувствительность больных к психотропным препаратам и предупреждает рецидивы депрессий.Prior to stimulation, sensory excitability of the hands is studied.
