The purpose of the invention is to reduce operative and postoperative complications in partially resolved cataracts with local adhesions of the anterior and posterior capsules. The anterior capsule is dissected in places of the lens thickening, the anterior capsule flaps are formed, a sequence of aspiration of the lens masses from the intracapsular synechiotomy is successively performed, then the intracapsular lens is installed into the capsular bag formed, rotating and centering it around the optical axis.
Description
The invention relates to the field of medicine, namely to ophthalmology, and can be used for the surgical treatment of cataract with the implantation of an intraocular lens.
A known method of traumatic cataract with implantation of an artificial lens, which consists in making a cataract incision, dissection of the anterior capsule, removal of the contents of the capsule bag, implantation of the artificial lens of the eye.
In this method, an artificial lens of the eye is first implanted and fixed in the ciliary sulcus in the posterior chamber, and then the anterior and posterior lens capsule is dissected. However, this method has significant drawbacks:
- наличие операционных и послеоперационных осложнений в виде выпадени стекловидного тела, которое приводит к- the presence of operative and postoperative complications in the form of loss of the vitreous body, which leads to
Кроме того расположение опорных элементов в цилиарной борозде в р де случаев при несоответствии диаметра борозды диаметру опорной части искусственного хрусталика глаза может вызывать пролежни и кровотечени в области контакта с тканью цилиар- ного тела,the development in the postoperative period of such complications as Irving-Gass syndrome or retinal detachment. In addition, the location of the supporting elements in the ciliary sulcus in some cases, if the diameter of the sulcus does not match the diameter of the supporting part of the artificial lens of the eye, can cause pressure sores and bleeding in the area of contact with the tissue of the ciliary body,
The aim of the invention is to reduce operative and postoperative complications in partially resolved cataracts with curved fusion of the anterior and posterior capsules.
Carry out the method as follows.
After the formation of an incomplete cataract incision 2.0-3.0 mm long with a microcystic or diamond blade, the anterior lens capsule is opened in

In the places of thickening of the latter, if possible in the upper third, the sequence of work with a spatula, viscous substances (healon or vizitil or their substitutes), a special flat cannula with a pointed 5 working edge, intracapsular synechiae are sequentially separated and the contents of the capsular bag are aspirated as it is released from synechia. A capsule pocket is gradually formed. Ka-10 t. The total incision is expanded to 5.0 mm. The artificial lens of the eye is inserted into the anterior chamber with tweezers. In this case, the lower support element is immersed in the formed capsule pocket. The upper support is a 15th element with the help of micro-hooks in vbd Gza remnants of the anterior capsule. A suture is placed on the cataract incision and the anterior chamber is filled to a weak hypertension, 4.0 mm depth for the hydraulic movement of the artificial lens into the capsular bag. A micro-hook inserted into the hole in the optical part of the artificial lens of the eye and under the action of the pressure of chamber moisture 25 makes rotational movements of the lens before the supporting elements are completely immersed in the capsule pocket and the lens reaches the center position relative to the optical axis of the eye. Sealing the wound, 30 forming the anterior chamber.
Example 1. Patient H., 23 g., Operated on according to the proposed method on April 16, 1988 regarding a traumatic membranous cataract of the intercapsular synexus of the left eye. An intracapsular lens was implanted with fixation in the newly formed capsule bag.

The cornea retains its transparency. The pupil was kept round, the artificial lens of the eye was located relative to the optical axis of the eye. Complications in the postoperative period is not marked. As a result of the intervention, a high functional effect was achieved: sharpness of sight 0.5 s cyl. - 1.5D, axis 43 ° 1.0. The effect achieved is maintained throughout the entire observation period of 1.5 years.
Contraindications to surgery are extensive defects in the lens capsule or its absence.
The application of the proposed method allows to reduce operative and postoperative complications by 60%. This effect is due to the fact that intracapsular correction eliminates the possibility of the formation of bedsores with the tissue of the ciliary body. In addition, the likelihood of loss of the vitreous body is reduced.
