The goal is to reduce the morbidity during obturation of fistulas having a narrow, tortuous and long stroke. A conductor is inserted through the external fistulous opening into the intestinal lumen, captured by an endoscope inserted retrogradely, brought out through the anus, attaching an obturating element to the conductor, which, securing it to the internal fistula, closes it and fixes it on the second obturating element, previously laid on the external fistulous opening. The method allows for occlusion of long, crimped passages, which promotes faster healing. The method is recommended for practical use. w Ё
Description
The invention relates to medicine, namely to surgery, and can be used for the treatment of external fistulas of the colon.
The purpose of the invention is to reduce the trauma during obstruction of fistulas that have a narrow, tortuous and long stroke.
The method is carried out as follows.
Pre-fistulografia of the external fistula of the colon is determined. in the colon, the localization of the external and internal fistulous openings, respectively (which may be located in the right or left half of the intestine), communicating with a long, twisted passage, the diameter of the fistula, the direction and length of the fistulous passage, the presence of cavities.
If there is a cavity in the course of the fistula, it is preliminarily drained. Depending on the location of the fistula, a fibrocolonoscope or recto-manoscope is used in the right or left half of the large intestine.
A conductor in the form of a fishing line is inserted into the lumen of the intestine through the external fistulous opening along the long, crimped passage and the internal fistulous opening. Into the large intestine is injected retrogradely, through the anus, with a retoromanoscope tube with a biopsyner to the internal fistulous opening and the line drawn out through it. A line is drawn through the - fistulous opening of the fistula / with a biopsator and it is pulled out of the fistula through the anus to the outside. To the line brought out

Attach the suture with the occluding element in the form of a petal or plate and pull it with a fishing line to the internal fistulous opening. Thus, the installed obturating element, continuing to push it to the external fistula opening, is fixed with suture material on the second obturating element in the form of a spongy pad and plate, which is pre-laid on the external fistulous opening on the abdominal wall. When the inner occluding element is tightened to the outer, the long, crimped fistulous course is shortened, and the fibrously changed walls of the fistulous course are sutured with suture material, followed by the development of granulations and healing of the fistulous course along with secondary tension, which contributes to faster healing fistulous course and, consequently, the treatment time is reduced. As a suture material, you can use any suture material. Depending on the goal pursued — the preoperative preparation of the patient and the operative floor — silk is used; in the radical conservative treatment of the fistula, catgut is preferred because the fistulous wall is traumatized through the fistula outside the abdominal wall and the granulation develops the wall of the stroke, followed by healing of the stroke and the fistulous opening. The occluding element emerges from the colon during an act of defecation, the external occluding element covering the opening of the fistulous passage also disappears when the fistulous passage is closed and the catgut is resorbed.
Example. B-on T .. 15 years, was treated with a diagnosis of: external tubular fistula cecum. When performing fistulography, a fistula of the cecum was found with a diameter of 0.5 cm and a length of 12 cm. An endoscopic retrograde obstruction of the external tubular fistula of the cecum with a long, twisted stroke was made using the proposed technique. Within 12 days, the fistula of the cecum was closed, as a result of the healing of the fistulous course during an act of defecation, a lamina emerged covering the internal fistulous opening and detached the lid covering the external opening of the fistula.
Использование способа обтурации свищей толстой кишки позвол ет производить окклюзию длинных, извитых ходов, способствующую быстрейшему заживлению их и сокращению сроков лечени .Using the method of obstructing the fistula of the large intestine allows for occlusion of long, tortuous passages, contributing to their faster healing and shortening the treatment time.
