Description
The invention relates to medicine, namely to traumatology.
The aim of the invention is to increase the strength of osteosynthesis in oblique and perioral articular fractures of the long tubular bones.
FIG. 1.2 shows a device for compression osteosynthesis, a general view. in fig. 3.4 - the same. chilled; in fig. 5. 6 - the same. in working condition.
The device for osteosynthesis is made in the form of plate 1. which has on one side two pointed legs 2, on the other - pointed clamps 3. Except for. In the central part of the plate 1, holes 4 are made under the screws 5. Moreover, the holes 4 in the central part of the plate 1 are connected by a longitudinal slot 6. The edge 7 of which is curved in opposite directions, the end of the gripper 3 is in the form of an annular hook 8, the pointed point of which is directed to opposed sharpening of the legs 2. The device is made of a material with a memory effect, for example, of an alloy of nickel-titanium-TH 20, which ensures the restoration of the initially specified shape in the working position while heating the plate 1 d o temperatures above 35 ° С (after its preliminary deformations at temperatures above 25 ° С).
The device is applied as follows.
е. При этомWith comminuted hypothalamic fracture of the humerus during the operation, the fracture site is exposed and the ItGCTHbid fragments are compared. The distance from the large tubercle of the head in the distal direction by a distance greater than 0.8-1 cm in length of the outer plate part of the plate 1, in the diaphyseal bone fragment, for example, with a shila, a hole is drilled (see FIG. 5). Then the device is irrigated with chloroethyl for 10-15 s, cooling it to a temperature below 10 ° С (in principle, a device made of an alloy of TN 20 can be deformed without chlorostil, i.e. at room temperature 20-25 С, however pre-cooling with chlorostil provides the elongation of the time of contact heating of the plate, which is necessary for the installation of screws). Then, using, for example, pliers, the longitudinal edges 7 are squeezed at the level of slot b, as a result of which the plate 1 lengthens by 0.8-1.0 cm. See fig. 3) and legs 2 are straightened to give them a shape suitable for insertion into bone (see Fig. 4). The device is transferred to the surgical wound, the ring-shaped hook 8 of the grip 3 of the plate 1 is inserted into the previously drilled hole, and the pointed legs 2 are driven into the spongy substance of the large tubercle of the humeral head. After that, install the screws 5 through the openings 4 of the plate 1, thus achieving additional fixation of the device in the head, the distal fragment and the middle fragment. The wound is washed with warm (40-50 ° C) sterile furatsillin solution, as a result of which the device is heated to a temperature above 35 ° C and the memory effect of titanium nickelide appears. Plate 1 tends to take a given (i.e., initial) shape.
