Abstract
The invention relates to surgery and allows improved healing conditions for unformed intestinal fistulas. The method consists in abuting the fistula with a foam rubber obturator. The length of the rod, directed along the outlet loop, is 1.1–1.2 times the diameter of the intestine. At the same time, the diameter of the rod is 1.0–1.5 mm larger than the diameter of the fistulous opening, and the diameter of the stem is 2.0–2.5 mm larger than the diameter of the rod. As the epithemation proceeds, the obturator is replaced with a plug of a smaller diameter until the fistula is completely closed.
Description
The invention relates to medicine, in particular to surgery, and can be used in the treatment of unformed intestinal fistulas.
The aim of the invention is to improve the conditions for the healing of a fistula by providing a tight obturation and reducing trauma to the intestinal wall.
The goal is achieved by closing the intestinal fistula with an obturator — a nail made of foam sponge, followed by copiously filling the wound with ointment and successively replacing the obturator with an obturator with a smaller diameter as epithelialization of the skin around the fistula occurs.
The method is carried out as follows.
Using a foam foam sponge, they cut out an obturator in the shape of a nail with a rod length 1.1-1.2 times the diameter of the intestine and 1-1.5 mm in diameter more than the diameter of the fistula, while the diameter of the head is 2-2 larger than the diameter of the rod, 5 mm Shl pka is flat, 3-4 mm high
An obturator is inserted into the lumen of the intestine through the opening of the fistula, a nail previously moistened in furacillin solution. The obturator shaft closing the fistula is directed to the discharge loop of the intestine. The peristaltic contractions of the intestinal wall push the obturator in the distal direction, and the head prevents this. Thus, the obturator-nail is fixed in one position. During the first 2-3 days, the pores of the foam pipe are clogged with chyme and the obturator becomes airtight.
Transfer wounds 1-2 times a day. The wound is washed with an antiseptic solution, without shifting the position of the head of the obturator. Then the wound is abundantly filled with ointment, the intestinal loops are covered with ointment, and a gauze band is placed on top.

В заключение накладывают дав щую мазевую пов зку (можно мазь Вишневского) до окончательного закрыти кишечного свища .The fistula is performed by wound epithelialization to the extent that the skin edges begin to compress the obturator. Then the obturator is replaced with another one with a smaller rod diameter. And so on until the intestinal fistula turns into a point fistula. In conclusion, a pressing ointment band is applied (Vishnevsky ointment can be applied) before the final closure of the intestinal fistula.
Example. Patient P., 26 years old, entered 10 with a penetrating wound of the abdomen 2 days after the injury with a picture of peritonitis, the source of which was damaged by the injector 2 times. In satisfactory condition discharged home.
The method allows to reliably close the intestinal fistula, contributes to the favorable healing of the wound. An important advantage of the obturator is the possibility of applying this method in multiple fistulas.
The method has passed clinical trials and is recommended in practice.
