N9 Изобретение относитс к медицине, а именно к хирургической стоматологии. Известен способ пластики подбородка при его тотальном отсутствии с помощью кожного лоскута с волосистой части головы 1. Однако при использовании этого способа возникают послеоперационные осложнени : рубцовые изменени , некроз внутренней выстилки . Цель изобретени - уменьшение послеоперационных осложнений. Указанна цель достигаетс тем, что согласно способу пластики подбородка при его тотальном отсутствии с помощью лоскута на передней поверхности грудной, клетки выкраивают кожно-жировой лоскут с включением больщой грудной мышцы, который поднимают вдоль ветвей питающей ее артерии , перекидывают через ключицу и через тоннель под кожей нижних отделов шеи укладывают на дно полости рта кожей внутрь, образу внутреннюю выстилку. Пример. После освежени краев раны на передней грудной стенке с центром на уровне п того ребра выкраивают овальной или пр моугольной формы лоскут кожи с жировой клетчаткой необходимых размеров (8X10, 12X14 см). Подъем лоскута выполн ют снизу, под больщой грудной мышцей. Через 8-10 см обнаруживают сосудистый пучок мышцы, располагающийс на ее внутренней поверхности. Рассекают кожу передней грудной стенки от ключицы до верхнего кра лоскута, под визуальным контролем за сосудистым пучком сквозными разрезами иссекают большую грудную мышцу вдоль сосудистого пучка, формируют мышечнососудистую ножку. Кожно-жировой лоскут с подлежащим участком мышцы поднимают на мышечной сосудистой ножке и через тоннель под кожей шеи перевод т в полость рта. Рану на грудной клетке ушивают после распрепаровки ткани. Кожную часть лоскута подшивают к остаткам слизистой дна полости рта кожей внутрь, мыщечную ножку располагают вдоль сосудистого пучка щей. На волосистой части головы выкраивают над апоневрозом кожный лоскут с учетом хода ветвей поверхностной височной артерии, освобождают от подлежащих тканей и на двух питающих ножках, наход щихс у скуловых дуг, смещают на подбородок, формиру наружную выстилку. Лоскуты послойно сшивают между собой. Через 4-5 недель ножки отсекают и возвращают на место. Дефект ткани в теменной области закрывают расщепленным кожным лоскутом. Предлагаемым способом можно полностью устранить обширный дефект подбородочной области с хорошим функциональным и косметическим результатом и таким образом формировать полноценный подбородок, резко уменьшить веро тность некроза внутренней выстилки.N9 The invention relates to medicine, namely to surgical dentistry.
