This invention relates to medicine, in particular, to methods for the surgical treatment of congenital radioulary synostosis.
1.There is a known method for the treatment of congenital radiosolar synostosis, including resection of the ulnar and radius areas and the creation of neoarthrosis between them 1.
However, this method does not ensure full restoration of rotational movements in the forearm, since consolidation of the bones is possible in the postoperative period.
The purpose of the invention is the full restoration of rotational movements in the forearm.
м внед рени .The goal is achieved by the method of treating congenital radiosolar synostosis, including resection of the ulnar and radial bone fragments and the creation of neoarthrosis between them, after resection of the radius bone portion, its distal fragment is connected to the allograft of the radial bone head by inserting it.
The method is carried out as follows.
Kocher incision in the upper third of the forearm cut through the skin, subcutaneous tissue, superficial fascia. The branch of the radial nerve is isolated and moved on a rubber holder. Then, the periosteum is dissected and the radius is extracted directly under the synostosis. An acute osteotome resects a fragment of the radial bone at a distance of 2-3 cm, together with the periosteum and partially the interosseous membrane. After that, movements in the forearm are checked. A longitudinal incision is then made in the lower third of the forearm right above the head.
The subperiosteal strip of the ulnar bone is resected and 0.5 cm of the diaphysis is resected. Rotational movements in the forearm are checked and intramedullary osteosynthesis of the ulnar bone is performed with a Kirschner needle.
На ночь одевают гипсовый лонгет в положении гиперкоррекции супинации кисти ипредплечь .After that, they return to the incision in the upper third of the forearm, process it with cylindrical mills, sawdust and allograft of the radial head, and connect them with an insertion method. Wounds protect layers tightly. Impose the rear hipseys longget in the position of operational correction of pathological pronation of the hand. On the 5-7th day after the operation, the correction of pronation by 15-20 ° is performed, and then the gypsum longt is changed every other day before the hypercorrection of the rotational movements in the hand and forearm is changed. By the 10-11th day after the operation, the stitches are removed and they are occupied with active correcting gymnastics. At night, they put on a plaster splint in the position of hypercorrection of the brush supination and an upper arm.
In the postoperative period, the allograft is fully consolidated, and the articular allograft carcinoma prevents the restoration of the radial bone integrity, and functions as an arthroplastic material. Consolidation of the ulna occurs within 5-6 months after the operation, although it is active on exercise therapy. By this time, the needle is removed and it is possible to fully restore the rotational movements in the forearm.
Предлагаемый способ операции позвол ет восстановить анатомические соотнощени костей и ротационные движени в предплечье .The proposed method of operation allows the restoration of anatomical bone ratios and rotational movements in the forearm.
