Abstract
Устройство относится к медицине, а именно к травматологии и ортопедии. Цель работы устройства - одновременная дистракция и разработка движений в голеностопном суставе под дозированной нагрузкой. Устройство используется в послеоперационном периоде на этапе реабилитации у пациентов с повреждениями голеностопного сустава, а также после длительной иммобилизации (повреждения костей голени, в том числе лодыжек, стопы). На прооперированную нижнюю конечность надевается шерстяной носок, затем обувь со шнуровкой и с вставленной в нее специальной эбонитовой стелькой, имеющие отверстия в подошве, на уровне оси разрабатываемой конечности. Обувь плотно зашнуровывается. Нога укладывается на шину Беллера. Через отверстия, имеющиеся в стельке и подошве обуви, протягивается крепкая нить, за которую подвешивается груз. Вес груза подбирается индивидуально.The device relates to medicine, namely to traumatology and orthopedics. Известен «Реабилитационный подстопник», предназначенный для авторазработки сустава под дозированной нагрузкой (Заявка №92006111, Михайленко В.В., RU, от 1992.11.13). Подстопник с основанием в виде шины Беллера состоит из опор на рейках шины. На опорах, фиксируемых винтами, крепится рама с осью, проходящей сквозь подстопную платформу имеющей на резьбовых концах стопорные гайки. На стойке рамы укреплен кронштейн под пружину, которая другим концом крепится за петлю внизу платформы. На верху платформы подвижно крепится граблеобразный стопор, накинутый зубцами на шпинек, находящийся сверху рамы. При длительном отсутствии движений в голеностопном суставе (повреждения, длительная иммобилизация конечности) питание суставного хряща нарушается. При разработке сустава в нашем устройстве трение практически не оказывает повреждающего воздействия на суставные поверхности хрящей. Разработка происходит менее болезненно, в более короткие сроки, уменьшается риск возникновения деформирующего артроза. Больной X., 41 года, поступил в отделение травматологии ВОКБ №1 в плановом порядке 12 апреля 1999 года, история болезни №4338/3, с жалобами на боли в левом голеностопном суставе. Диагноз - застарелый закрытый перелом лодыжек, заднего края болыпеберцовой кости левой голени с вывихом стопы кзади. Травму получил 5 января 1999 года, лечился по месту жительства в гипсовой лонгете. Не исключает возможность вторичного смещения отломков. В раннем послеоперационном периоде больной X. прошел курс реабилитации, направленный на разработку голеностопного сустава с его разгрузкой, с применением устройства для реабилитации больных с повреждениями голеностопного сустава. 30 апреля выписан на амбулаторное лечение. В результате функция голеностопного сустава была сохранена.
