Description
The invention relates to medicine, in particular to maxillofacial surgery and can be used to treat adentia of the jaw.
According to statistics, of the total number of patients who turn to the dentist for lack of teeth, 18-20% have end defects in the dentition, more than half of them in the upper jaw. Due to the anatomical and topographic features of the structure of the upper jaw (the presence of the maxillary sinuses, pear-shaped holes) in most cases, it is possible to use only plate implants. Unfortunately, these implants have one significant drawback - low stability in the vestibulo-oral direction. To eliminate this drawback, implant designs were developed from materials with a shape memory effect.
A known dental implant with the method of its installation (RF patent No. 2135118, publication Bulletin of inventions No. 24 1999, p.176, M. class A 61 C 8/00), made of metal with a shape memory effect, consisting of a head, neck , plate body with simulators of periodontal ligaments, deforming in the lateral directions from the plane of the body after the introduction of the implant into the process of the jaw.
The disadvantages of this device are: the weak stability of the implant in the vestibulo-oral direction, due to the arising lever of forces, causing micro-movements of the fixing elements relative to the bone in the vertical plane, as well as the significant height of the intraosseous part, which complicates the use of this design in conditions of atrophy of bone tissue, especially in the location of the maxillary sinuses.
A known design of the intercortical maxillary implant (RF patent No. 2211004, M. class A 61 C 8/00, publication of the Bulletin of inventions No. 24, 2003), consisting of a head, neck and support plate with two locking elements, the first of which is located horizontally, and the second locking element is formed by twisting the height-reduced end of the plate by 90 ° relative to its longitudinal axis.
The disadvantages of this device are: the relatively large height of the intraosseous part, as well as the insufficient stability of the implant in the vestibulo-oral direction. This makes it difficult to use it on the upper jaw in the area of pre- and post-sinus bone masses, with a bone height under the sinus of less than 6-8 mm.
The closest to the claimed can be considered the design of the dental subperiosteal implant Patent RU 2094026, M. cl. 6 A 61 C 8/00, BIMP No. 30, 1997. The implant is a rectangular plate made of metal with a shape memory effect, with the implant head screwed in its center. The plate bends in the shape of the toothless section of the alveolar process. It is equipped with fixing legs, the pointed end parts of which are curved inward and towards each other.
This device has two main disadvantages - the need for double exposure of bone tissue over a large area, which subsequently causes its significant atrophy, as well as significant technological difficulties that arise in the process of shaping the plate of the alveolar process.
The objective of the utility model is to increase the stability of the implant, maximize its adaptation in shape to the installation site and, therefore, lengthen its life, expand indications for use.
The task is achieved in that the subperiosteal implant is made of an alloy with thermomechanical shape memory. The base plate is curved in the shape of the alveolar bone and is equipped with fixing

The support plate is made in the form of two tapes having mid sections located in parallel and end sections, which are the fixing elements. The tapes are connected by two transverse jumpers located at the points of transition of one section of the tape to another, with the formation of a rectangular window in the center. The end sections of the tapes are oriented in the horizontal plane towards each other and are perforated. The support heads are located in the center of the jumpers.
The novelty of the utility model.
Концевые участки лент ориентированы навстречу друг другу в горизонтальной плоскости, с целью использования естественных ретенционных пунктов верхней челюсти.• The support plate is made in the form of two tapes having mid sections located in parallel, and end sections of tapes, which are fixing elements. The end sections of the tapes are oriented towards each other in the horizontal plane, in order to use the natural retention points of the upper jaw.
• Ленты соединены двумя поперечными перемычками в местах перехода срединных участков лент в концевые участки с формированием прямоугольного окна для образования единой несущей конструкции и минимизации окклюзионного давления непосредственно на атрофированный гребень альвеолярного отростка• Tapes are connected by two transverse jumpers at the places of transition of the middle sections of the tapes to the end sections with the formation of a rectangular window to form a single supporting structure and minimize occlusal pressure directly on the atrophied ridge of the alveolar process
Этим достигается надежная и минимально травматичная фиксация имплантата.• The end sections are fixed to the natural retention sections of the alveolar process of the upper jaw (in the process of realizing the shape restoration effect) and are fixing elements. This ensures reliable and minimally traumatic fixation of the implant.
• Опорные головки расположены в центре перемычек, что позволяет оптимизировать распределение окклюзионного давления на альвеолярный отросток верхней челюсти.• The support heads are located in the center of the lintels, which allows optimizing the distribution of occlusal pressure on the alveolar process of the upper jaw.
• Концевые (фиксирующие) участки лент перфорированы для возможности прорастания костной ткани сквозь тело имплантата, что повышает надежность закрепления.• The end (fixing) sections of the tapes are perforated to allow bone tissue to grow through the body of the implant, which increases the reliability of fixation.

6.The subperiosteal implant is made in the form of a plate curved in the shape of the alveolar ridge and contains two ribbons 1, which are connected by two transverse jumpers 2 to form a rectangular window in the center, with the middle sections 3 of the ribbons 1 parallel to each other, and the fixing elements are formed due to the counter the orientation in the horizontal plane of the perforated end sections 4 of the tape 1. In the center of the jumpers 2 are supporting heads 5. Sections 4 have perforations 6.
The manufacture of a jaw implant from a metal with a thermomechanical memory, for example, from titanium nickelide, ensures the restoration of the initially given implant shape under the influence of heating after preliminary deformation of the fixing elements at a temperature below 0 ° C.
When installing the implant on the upper jaw, one tape 1 having two fixing elements at the ends 4 is located subperiosteally with
В процессе реализации эффекта формовосстановления концевые участки 4, являющиеся фиксирующими элементами, изгибаясь в горизонтальной плоскости, плотно охватывают альвеолярный отросток, надежно фиксируя имплантат и исключая его смещение в вестибуло-оральном и вертикальном направлениях.the buccal side of the alveolar process of the upper jaw, covering its tubercle and going into the region of the incisal fossa, and the second is subperiosteal from the palatine side of the alveolar process, due to which the existing bone mass is used to the maximum and the distribution of the masticatory load is optimized. In the process of realizing the shape restoration effect, the end sections 4, which are the fixing elements, bending in the horizontal plane, tightly cover the alveolar process, reliably fixing the implant and excluding its displacement in the vestibulo-oral and vertical directions.
The implant is used as follows: for a patient with adentia, a mucosal incision is made along the crest of the alveolar ridge of the jaw, two trapezoid mucosal-periosteal flaps (palatine and vestibular) are exfoliated, a bone is exposed, two grooves are formed in the bone of the crest of the alveolar ridge along the width of the 2 transverse grooves. The implant is placed in the refrigerant, cooling it to a temperature below 0 ° C, with the help of the tool, the end sections of 4 tapes 1 are unbent (Fig. 4), then the implant is placed in the formed bed. After 20-30 seconds, due to the implementation of the shape-restoration effect that occurs when the implant is contacted heated to 36 ° C, the design tends to take a predetermined (i.e., initial) form. The end sections 4 of the tapes 1, which are the fixing elements, bending in the horizontal plane by 10-15 ° cover and securely fix the implant on the alveolar process (Fig.5, 6). The mucoperiosteal flap is put in place, the wound is sutured, the heads of the implant 5 rise above the level of the mucosa, protruding into the oral cavity.
Example: Patient M., 52 years old, medical history No. 269, contacted the dental department of the Clinical Perinatal Center, MPLU for the absence of 14, 15, 16, 17 and 18 teeth.
The patient underwent an operation to install a subperiosteal implant of the described design in place of the missing teeth. During the above operation, the following techniques were performed:
1. Infiltration anesthesia.
2. Dissection of soft tissues and exposure of the bone of the alveolar process of the upper jaw at the site of implant placement.
3. The formation of the bone bed.
4. Cooling and deformation of the implant at a temperature below 0 ° C.
5. The introduction of the implant into the bone bed.
6. Suturing of the surgical wound with interrupted sutures with polyester. The postoperative period was uneventful. After 7 days, the sutures were removed from the mucosa, and after 3 days a temporary plastic bridge was made and installed with support for 13, 12 teeth and the heads of the installed implant. After 4 months, the temporary prosthesis was replaced by a permanent ceramic-metal one. During the follow-up examinations after 6 and 12 months, the patient does not show complaints. The wound was epithelized, the mucosa was of a normal color, the implant was stable, there were no signs of inflammation, or rejection. Cosmetic and functional effect is good.
Thus, the proposed design provides reliable fixation of the implant on the alveolar process of the upper jaw, prolongs its functioning due to the optimal load distribution and reduces the time interval from installation of the device to prosthetics.
