Abstract
Description
The invention relates to medical activities in the field of maxillofacial surgery, and can be used to prevent the progression of postoperative edema in the middle and lower thirds of the maxillofacial region.
The problem of uncontrolled edema after orthognathic operations remains one of the most pressing in modern maxillofacial surgery. First of all, this is due to lymphostasis in the maxillofacial area after surgical interventions.
There is a known method of applying a pressure postoperative kinesiological bandage, which includes applying bandages made in the form of separate kinesiological tapes of various lengths and widths to the skin of the face (RU 2784288 C1, 01.09.2022). The method consists of sequential fixation of kinesiological tapes in the chin area, the area of the labiomental fold to the parotid-masticatory region, above the upper lip to the parotid-masticatory region and from the submental zone to the zygomatic areas on both sides, from the infraorbital region and the upper border of the mouth to the parotid region. masticatory region and the anterior border of the sternocleidomastoid region.
The disadvantage of this known method is the lack of fixation of kinesio tape in the direction of lymphatic drainage from the maxillofacial area to the area of the supraclavicular lymph nodes.
A closer invention is a method of healing the skin and muscles of the face by taping, described by E. A. Karkukli (RU 2772042 C1), in which the application of tapes for lymphatic drainage is carried out on the third visit. This method differs in that it is used for lymphatic drainage in patients with no postoperative swelling.
The complex of taping for lymphatic drainage includes taping the orbicularis oculi muscle, neck muscle and the effect according to the lymphatic drainage pattern in the area of the supraclavicular lymph nodes. In the complex for lymphatic drainage, gluing of the strip is carried out without fixing the points and without tensioning it.
The disadvantages of this method include: fixation of kinesio tapes without tension, thus reducing the cellular spaces that contain lymphatic vessels. Also, fixing tapes is divided into floors for lymphatic drainage.
The invention is aimed at creating and adapting a method of lymphatic drainage in the postoperative period of orthognathic operations into clinical practice.
The technical result of the invention is to prevent the occurrence and progression of postoperative edema in the postoperative area of the maxillofacial area.
1-5).The essence of the invention is expressed in a set of essential features, in which the method of applying a pressure postoperative kinesiological bandage to the maxillofacial area, including preparing the bandage for application to the skin and applying the bandage to key areas of the face, differs from the closest analogue in that the skin of the middle is degreased and lower areas of the face and side surfaces of the neck with a 70% solution of ethyl alcohol. Kinesiology tapes are fixed immediately after surgery. The bandage is applied in the form of separate kinesiology tapes. Kinesiology tapes are applied in the following sequence. The tape, 120 mm long and 45 mm wide, is cut lengthwise into three equal sections, leaving the base of the kinesio tape untouched. The base is 30x45 mm. The base is fixed in the supraclavicular fossa, from where the first piece of kinesio tape is passed and attached at the level of the ear tragus. The second piece of kinesio tape is installed in the middle of the cheek. The third piece of kinesio tape is attached in the area of the nasolabial fold. Thus, the distance between the pieces of kinzioteape is 15 mm. All segments are fixed with a tension of 25% of the maximum. After this, the contact of the kinesiological bandage with the skin surface is manually increased throughout its entire length by smoothing. This manipulation is performed on both sides of the face and neck. The essence of the invention is illustrated by photographs of the patient, sequentially illustrating all stages of the method (Fig. 1-5).
Using this method, the clinic rehabilitated 50 patients after orthognathic operations in the maxillofacial area.
Example: patient K., 22 years old, was admitted to the department of maxillofacial surgery with a diagnosis of anomaly in the development of the dentofacial system. Upper retro-, micrognathia. Inferior macrognathia. Mesial occlusion. III skeletal class according to Engle's classification. The following surgical intervention was performed: Osteotomy of the upper jaw according to Le-Fort I, bilateral intercortical osteotomy of the lower jaw with the jaws placed in an orthognathic relationship. Genioplasty. Immediately after the end of the surgical intervention, the skin of the middle and lower areas of the face and neck was degreased with a 70% ethyl alcohol solution. Next, the first kinesiology tape, 120 mm long and 45 mm wide, was cut lengthwise into three equal sections, leaving the base of the kinesiology tape untouched. The base is 30x45 mm. The base was fixed in the supraclavicular fossa (Fig. 1), from where the first piece of kinesio tape was passed and fixed at the level of the ear tragus (Fig. 2). The second piece of kinesio tape was installed in the middle of the cheek (Fig. 3). The third piece of kinesio tape was attached in the area of the nasolabial fold (Fig. 4). Thus, the distance between the pieces of kinzioteape was about 15 mm. All segments were fixed with a tension of 25% of the maximum. Upon completion of the manipulation, the kinesio tapes were manually smoothed along their entire length. This technique was performed on both sides of the face and neck (Fig. 5). The bandage was changed after 3 days and removed on the 7th day.
As a result of using this method of fixing kinesiology tapes, it is possible to prevent the occurrence and progression of significant postoperative edema in the area of the surgical intervention by accelerating lymphatic drainage through the regional lymph nodes to the supraclavicular lymph nodes.
