Abstract
Description
The invention relates to medicine and can be used in maxillofacial surgery, traumatology and plastic surgery.
The problem of uncontrolled postoperative edema still remains one of the most urgent in modern medicine. First of all, this is due to the use of such methods of applying pressure bandages, in which it is impossible to influence key areas of the face during surgical interventions.
Известен способ наложения давящей послеоперационной кинезиологической повязки, включающий накладывание на кожные покровы повязок, выполненных в виде отдельных кинезиологических лент (US 10517767 В2, 31.12.2019).A known method of applying a pressure postoperative kinesiological bandage, including applying dressings made in the form of separate kinesiological tapes to the skin (US 10517767 B2, 12/31/2019).
The disadvantage of the known method is the shape and size of kinesiology tapes, which make it impossible to use such tapes for application to the maxillofacial region.
A closer analogue of the invention (prototype) is a method of applying bandages on the head, described by I.V. Mikhin, V.A. Blue (Tutorial "Desmurgy". - Volgograd, "Volgograd State Medical University" 2012. - 17-20 p.), in which for bandaging the head they use: the returning bandage "Hippocrates' hat", the bandage "cap", the bandage " frenulum", a cruciform bandage on the back of the head and the back of the neck, a sling bandage on the jaw.
The material is cut longitudinally at the ends of the bandage, the central part of the fabric remains uncut. Such a bandage allows you to fix the dressing without resorting to hard fixing devices, glue or adhesive tape. The bandage is fastened with the help of the ends of the fixing material cut into two parts.
The disadvantages of the method include: uneven pressure when applying bandages with a simple medical bandage, exerting uncontrolled pressure on the head, the need for daily bandage replacement, the possibility of bandages slipping when the patient moves his head.
The invention is aimed at solving a technical problem, which consists in eliminating the shortcomings of the known methods of applying a pressure bandage.
The technical result of the invention is to prevent the occurrence and progression of postoperative edema in the postoperative zone of the maxillofacial region.
The essence of the invention is expressed in a set of essential features, in which the method of applying a pressure postoperative kinesiological bandage to the maxillofacial region, including preparing the bandage for application to the skin and applying the bandage to key areas of the face, differs from the closest analogue in that the skin is degreased and lower areas of the face with 70% ethanol solution, on which a bandage is applied, made in the form of separate kinesiology tapes, and the application of kinesiological tapes is carried out in the following sequence: a tape with dimensions of 50 x 25 mm is applied vertically in the chin area, in the center, a tape with dimensions 150 x 25 mm are applied horizontally in the region of the labiomental fold to the parotid-masticatory region; a tape with dimensions of 150 x 25 mm with lateral recesses made in the middle under the nose and upper lip is applied horizontally above the upper lip to the parotid-chewing area, a tape with dimensions of 200 x 50 mm is applied from the submental zone to the zygomatic areas on both sides, while two tapes with dimensions of 150 x 50 mm, made with end recesses under the wings of the nose, are applied diagonally from the infraorbital region and the upper border of the mouth region to the parotid-chewing region and the anterior border of the sternocleidomastoid region on the right and left.
The essence of the invention is illustrated by photographs of the patient, sequentially illustrating all the steps of the method, where:
1- tapes used; fig. 2-8 - steps of the method.
According to this method, 23 patients were bandaged under clinical conditions after surgical interventions in the maxillofacial region.
Example: Patient A., aged 25, was admitted to the Department of Maxillofacial Surgery with a diagnosis of Combined deformity of the jaws. Lower retromicrognathia. Upper promacrognathia. Distal bite. II skeletal class according to Angle's classification. Conducted surgical intervention in the volume: Orthognathic surgery by osteotomy of the upper and lower jaws. Genioplasty. Immediately after the end of the surgical intervention, the skin of the middle and lower zones of the face is degreased with a 70% solution of ethyl alcohol. Further, kinesiology tapes are applied in the sequence: 50 x 25 mm - vertically in the chin area, in the center; 150 x 25 mm - horizontally in the region of the labiomental fold to the parotid-masticatory region; 150 x 25 mm - horizontally above the upper lip to the parotid-chewing area; 200 x 50 mm - from the submental area to the cheekbones on both sides; two strips 150 x 50 mm - diagonally from the infraorbital region and the upper border of the mouth region to the parotid-chewing region and the anterior border of the sternocleidomastoid region on the right and left. The dressing is removed after 4-6 days. Due to the application of this technique of applying a pressure bandage, it is possible to prevent the occurrence and progression of postoperative edema in the area of the surgical intervention.
