Хирургический метод лечения является самым эффективным способом в борьбе с морбидным ожирением, особенно на фоне осложнений заболевания, требует особого подхода, связанного с выбором метода операции. Все огромное количество выполняемых сегодня бариатрических вмешательств можно разделить на 3 типа: рестриктивные (подразумевающие уменьшение объема желудка); шунтирующие (предусматривающие ускорение пассажа по кишечнику); смешанные. Между тем, применение достаточно простых и безопасных рестриктивных вмешательств не всегда позволяет достичь желаемого эффекта снижения массы тела. Известны способы прогнозирования течения послеоперационного периода у больных после операций на органах брюшной полости - исследование анализов крови (лейкоцитоз с нейтрофильньгм сдвигом, ускорение СОЭ), распространенные интегральные шкалы (APACHE II и другие). Предлагаемый способ прогнозирования результатов хирургического лечения морбидного ожирения при рестриктивном типе бариатрической операции осуществляется следующим образом. Всем готовящимся к выполнению бариатрической операции определяют исходный уровень (концентрацию) грелина в крови пациента. Исследование проводят натощак в процедурном кабинете. Забирают кровь при помощи венепункции, кровь центрифугируют, получают сыворотку. Содержание грелина в сыворотке крови определяют методом конкурентного иммуноферментного анализа с использованием набора ELISA Kit for Ghrelin (GHRL) для человека согласно протоколу фирмы производителя реагентов «Cloud-Clone Corp.» (КНР). При значении в сыворотке крови уровня грелина 350 пкг/мл и более прогнозируют благоприятное течение послеоперационного периода при рестриктивном типе операции, при значении уровня грелина менее 350 пкг/мл прогнозируют рецидив ожирения при рестриктивном типе, бариатрическую операцию дополняют шунтирующим компонентом. В настоящее время наша клиника располагает опытом выполнения разнообразных инвазивных бариатрические процедур у 230 пациентов, у которых ИМТ превышал 35 кг/м2. В последние годы предпочтение отдавалось применению рестриктивных методик, и в частности рукавной («sleeve») резекции желудка. Для определения роли исходного уровня грелина на прогноз послеоперационного исхода мы провели исследование у 23 пациентов с морбидным ожирением, перенесших бариартрические операции в сроки от 1 года до 6 лет (в среднем 3,5±1,5 года). 18 пациентов перенесли рестриктивные вмешательства, из них у 13 получен стойкий эффект снижения массы тела, у 5 наступил рецидив ожирения. Кроме того, обследованы 5 пациентов, перенесших шунтирующие процедуры: билиопанкреатическое шунтирование (БПШ), гастрошунтирование (ГШ). In recent years, preference has been given to the use of restrictive techniques, and in particular sleeve gastrectomy. To determine the role of initial ghrelin levels on the prognosis of postoperative outcome, we conducted a study in 23 patients with morbid obesity who underwent bariatric surgery within a period of 1 to 6 years (average 3.5 ± 1.5 years). 18 patients underwent restrictive interventions, of which 13 achieved a lasting effect of weight loss, and 5 experienced a relapse of obesity. In addition, 5 patients who underwent bypass procedures were examined: biliopancreatic bypass (BPB), gastric bypass (GS). All patients were comparable in terms of gender, age, and initial BMI data. Research has been conducted on the recurrence of obesity. The clinical ethics committee approved all procedures and protocols, and written informed consent was obtained from all subjects before inclusion in the study. Пример 1. Больной Р., 37 л. поступил в клинику с диагнозом МО. Индекс массы тела 35 кг/м2. Исходный уровень грелина сыворотки крови 453 пкг/мл. Спрогнозировано благоприятное течение послеоперационного периода при рестриктивном типе бариатрической операции. Выполнена лапароскопическая рукавная резекция желудка. Послеоперационное течение гладкое. В отдаленный период до 1 года после операции самочувствие отличное, снижение массы тела на 48 кг.Example 1. Прогнозирован рецидив ожирения при рестриктивном типе бариатрической операции. Было принято решение бариатрическую операцию дополнить шунтирующим компонентом. Выполнена операция SADI (дуоденоилеошунтирование). Послеоперационное течение гладкое. Спрогнозирован высокий риск рецидива ожирения при рестриктивном типе бариатрической операции. Предложен смешанный вариант бариатрической процедуры, включающий шунтирующий компонент. Больной от него отказался. Выполнена лапароскопическая рукавная резекция желудка. Послеоперационное течение гладкое. Поначалу отмечалось снижение массы тела на 22 кг до полутора лет. Через 2 года прибавил массу тела на 12 кг. Вновь обратился в клинику. Уровень грелина сыворотки крови составил 237 пкг/мл. Спрогнозирован высокий риск рецидива ожирения при рестриктивном типе бариатрической операции. Было принято решение бариатрическую операцию дополнять шунтирующим компонентом. Выполнена операция SADI (дуоденоилеошунтирование). Послеоперационное течение гладкое. В ближайшем периоде после операции, самочувствие отличное, каких-либо признаков рецидива заболевания не отмечено, снижение массы тела на 49 кг.Example 3.
