Abstract
Description
- М.: 2000. - 111 с.).The invention relates to medicine, namely to endocrinology, and can be used to predict the progression of abdominal obesity in patients with metabolic syndrome, which is characterized by abdominal obesity, insulin resistance and impaired carbohydrate metabolism, arterial hypertension, atherogenic dyslipidemia (Dedov I.I. Obesity. Metabolic syndrome Type 2 diabetes mellitus. - M .: 2000. - 111 p.).
A method for predicting the progression of abdominal obesity in patients with metabolic syndrome is not known and is not described in literature.
The invention is aimed at solving the problem: the ability to predict the progression of abdominal obesity in patients with metabolic syndrome.
Effect: prevention of cardiovascular and other complications of obesity.
J. Physiol. Heart Circ. - 2000. - V.279. - №4. - P.1453-1459; Wahren J., Ekberg K., Johansson J. et al. Am. J. Physiol. Endocrinol. Metab. - 2000. - V.278. - №5. - P.759-768).The essence of the method consists in the use of determining the level of C-peptide in the blood. With a C-peptide level of more than 3.5 ng / ml, the progression of abdominal obesity in patients with metabolic syndrome is predicted. C-peptide, as it became known in recent years, is a biologically active substance, the excess of which contributes to the development of abdominal obesity, increases the risk of coronary heart disease, strokes, cardiac arrhythmias, steatohepatitis (Young LH, Ikeda Y., Scalia R. et al. Am. J Physiol. Heart Circ. - 2000. - V.279. - No. 4. - P.1453-1459; Wahren J., Ekberg K., Johansson J. et al. Am. J. Physiol. Endocrinol. Metab. - 2000. - V.278. - No. 5. - P.759-768).
The method is as follows.
After a 10-hour fasting on an empty stomach, in a patient with metabolic syndrome, blood sampling is performed in the morning and the level of C-peptide in the blood is determined. With C-peptide values greater than 3.5 ng / ml, the progression of abdominal obesity is predicted. When the content of the C-peptide in the blood is up to 3.5 ng / ml, the absence of progression of abdominal obesity in a patient with metabolic syndrome is predicted.
И. - М.: 1998).The study of the C-peptide is carried out by the enzyme immunoassay (Balabolkin M.I. Endocrinology. - M .: 1998).
The novelty of the method lies in the ability to predict the progression of abdominal obesity in a patient with metabolic syndrome, which will allow to take therapeutic and preventive measures that prevent the increase in visceral (hormone-active) fat.
Examples of specific performance.
М.Г. Example 1. M.G.G., 32 years old, suffers from metabolic syndrome from 25 years old. Height 184 cm, body weight 101 kg; BMI 30 kg / m 2 . The predominant deposition of fat in the region of the greater omentum. The patient in the last 5-6 months followed a sub-calorie diet, had regular significant physical exertion. Weight remained stable. Checked the level of C-peptide in the blood. It turned out to be 6.1 ng / ml with an upper limit of normal of 3.5 ng / ml. The patient was informed that his abdominal obesity would progress and that he was offered treatment with biguanides, which lower the level of C-peptide in the blood (glucofage 1000 mg at night). However, the patient refused to take glucophage. After 6 months, with the same lifestyle and against the background of a sub-calorie diet, body weight increased by 15 kg. The patient began to take glucophage. Upon repeated examination of the C-peptide, its level decreased to 4.3 ng / ml. Gradually, the progression of abdominal obesity stopped.
Н. Example 2. Patient P.N.A., 39 years old, suffers from metabolic syndrome and abdominal obesity of the III degree for 8 years. Plump during pregnancy and in the first 6 months after birth. Then the increase in body weight spontaneously stopped. Height 166 cm, body weight 124 kg; BMI 45 kg / m 2 , OT / OB index 1.03. The level of C-peptide in the blood was 5.87 ng / ml. Over 12 months, body weight increased by 9 kg, blood pressure increased to 160/100 mm Hg, opsomenorrhea appeared. After complex treatment, the level of C-peptide in the blood returned to normal and amounted to 2.8 ng / ml. Dynamic observation of the patient for 1.5 years showed that the progression of abdominal obesity has stopped.
Г. Example 3. Patient M.G.N., 45 years old, has been suffering from metabolic syndrome and abdominal obesity for 11 years. Height 170 cm, weight 98 kg, BMI 34 kg / m 2 , OT / OB index 1.1. Over the past six months, the menstrual cycle has been violated, menopause has been diagnosed. The patient began to worry that menopause would cause the progression of obesity. C-peptide level tested: 2.67 ng / ml. Over 12 months of follow-up, body weight did not increase.
Using the proposed method allows to predict the progression of abdominal obesity in patients with metabolic syndrome and to conduct timely treatment and prevention.
