Abstract
Description
The invention relates to the field of plastic surgery, in particular, to a method for changing the shape of the larynx and thyroid cartilage to form aesthetic contours of the neck and eliminate psychological discomfort associated with dissatisfaction with its appearance and shape. Such changes lead to improved social adaptation of patients, including as part of surgical gender reassignment. The method can be used in specialized plastic surgery departments.
Endoscopically Facilitated Reduction Laryngochondroplasty. Archives of Facial Plastic Surgery, 5(4), 345); (K. Altman (2012). Facial feminization surgery: current state of the art., 41(8))The following existing methods for correcting the shape of the larynx and thyroid cartilage, chondrolaryngoplasty, can serve as analogues of the proposed method: closed endoscopic method of chondrolaryngoplasty through the sublingual space (Conrad, Krzysztof; Yoskovitch, Adi (2003). Endoscopically Facilitated Reduction Laryngochondroplasty. Archives of Facial Plastic Surgery, 5( 4), 345); external access with resection in the area of the superior notch of the thyroid cartilage (K. Altman (2012). Facial feminization surgery: current state of the art., 41(8))
The closed endoscopic method of chondrolaryngoplasty through the sublingual space by Conrad et al (2003) is performed through a horizontal incision in the mental fold. The thyroid cartilage is identified and a Keith needle is inserted along the midline from the external approach into the lumen of the larynx as a marker a few millimeters above the midpoint of the anterior vertical height of the cartilage to avoid cutting the true vocal cords. Under direct visualization through a laryngoscope, the position of the marker needle in relation to the anterior commissure of the true vocal cords is checked. This position should be visible 2-3 mm above the anterior commissure. External markings for excision of the thyroid cartilage are made in the shape of the letter V, the apex of which corresponds to the point of insertion of the marker needle. Excision is performed using a Stryker saw or scalpel.
The closest analogue to the claimed invention is external access with resection in the area of the superior notch of the thyroid cartilage by K. Altman (2012). A 2 cm long incision is made in the upper skin fold of the neck. The perichondrium is incised to expose the notch of the thyroid cartilage. Bone cutters are used to remove the thyroid cartilage protrusion until satisfactory reduction of the thyroid cartilage is achieved.
However, the known methods have a number of significant disadvantages:
The closed endoscopic technique of chondrolaryngoplasty is difficult to use, as it requires endoscopic equipment and involves possible intraoperative complications, such as damage to the anatomical structures of the submandibular region and the upper third of the neck.
External approach with resection in the area of the superior notch of the thyroid cartilage has a risk of intraoperative damage to the thyrohyoid membrane when the perichondrium is torn off in the area of the superior notch of the thyroid cartilage and, as a consequence, the risk of dysfunction of the superior laryngeal nerve, which can lead to dysphonia.
The purpose of the proposed method is to form aesthetic contours of the neck by changing the shape of the thyroid cartilage with minimal risks of complications and the use of low-traumatic techniques.
The stated goal in the chondrolaryngoplasty method for correcting the shape of the larynx and thyroid cartilage is different in that the plates of the thyroid cartilage are resected from the superior thyroid notch to the point of projection of the vocal cords and epiglottis fixation onto the outer surface of the thyroid cartilage, the remaining edges of the plates are sutured together, tucked in, forming the anterior contour of the thyroid cartilage in the form of a smooth arc.
The proposed method is not found in the medical literature, which allows us to conclude that it meets the “novelty” criterion.
The essence of the proposed invention is that the plates of the thyroid cartilage are resected from the superior thyroid notch to the point of projection of the vocal cords and epiglottis onto the outer surface of the thyroid cartilage, the remaining edges of the plates are sutured together, tucked in, forming the anterior contour of the thyroid cartilage in the form of a smooth arc .
Thus, the combination of all the distinctive features allows you to achieve your goal and get a positive result.

The method is carried out as follows: a transverse incision up to 1.5 cm long is made along the upper cervical fold. Next, a blunt and sharp method is used to separate the soft tissues and expose the angle of the thyroid cartilage. In the area of intended cartilage resection, perichondrium detachment is performed. Using a scalpel, the plates of the thyroid cartilage are resected from the superior thyroid notch to the point of projection of the vocal cords and epiglottis onto the outer surface of the thyroid cartilage, the remaining edges of the plates are sutured together, tucked in, forming the anterior contour of the thyroid cartilage in the form of a smooth arc. The wound is sutured in layers with absorbable suture material 5/0: subcutaneous neck muscle, subcutaneous fat and skin.
The use of the proposed method of chondrolaryngoplasty when correcting the shape of the larynx and thyroid cartilage allows:
