Abstract
Description
The invention relates to household diagnostic devices, in particular to devices that allow on an outpatient basis to support the stomach and measure the duration of periods of rest of the fetus during diagnostic apnea in the second half of pregnancy, diagnose the resistance of the fetus to hypoxia, choose the type of delivery and record the obtained values regardless of subjective sensations women, the presence of a paper carrier and a stopwatch.
Currently, on an outpatient basis, to measure the duration of the period of the immobile state of the fetus in the third trimester of pregnancy, pregnant women use the fingertips as a sensory sensor, and their subjective sensations as an information analyzer. In addition to this, they use a stopwatch, a writing device such as a ballpoint pen, as well as a paper medium to record the obtained values. The pregnant woman puts her fingers to her stomach, with the help of the pads of these fingers she tactilely diagnoses the upper body of the fetus, after which, using the pads of her fingers, she continuously monitors the motor activity of the fetus during diagnostic apnea and uses a stopwatch to record the duration of the fetal rest period (RU 2749637 ). The woman registers the obtained values on paper, for example, in a pregnant woman's exchange card.
However, the fingertips of a pregnant woman do not provide an objective and standardized diagnosis of fetal immobility. The fact is that the tactile sensitivity of the pads of each finger is changeable during the day due to the daily rhythm of metabolism and the function of the body of a woman, changes with a change in the local temperature of the pads of the fingers, depends on the degree of their blood supply, the thickness of the skin of the pads of the fingers and the presence of local inflammation, which can occur, in particular, due to accidental bruising of the soft tissues of the fingertip. In addition, the sensitivity of the pads depends on the manicure, genetic predisposition, experience, intensity of playing musical instruments and their type (plucked strings and others), and also changes when the skin of the fingers is treated with creams and / or ointments, when the diet changes, the development of dermatitis, allergies, hypovitaminosis and / or changes in the hormonal background in a woman.
In addition, the duration of the period of immobility of the fetus, determined using a handy stopwatch, is not accurate, since this does not exclude the use of any stopwatch, including poor quality, defective, very old stopwatch and / or a stopwatch that has expired, as well as a stopwatch that requires a certain the skill of holding it in your hand and the skill of pressing the power button. Moreover, the use of a stopwatch that happened to be at hand does not ensure its timely switching on and off by a pregnant woman, since it depends on the morphofunctional state of her upper limb, the degree of reactivity of the woman, her dexterity and her experience of switching on and off the selected stopwatch. In addition, the accuracy of determining the values depends on the psycho-emotional state of the woman, the ability to use a mechanical and / or electronic stopwatch, as well as the correspondence of the size of the stopwatch to the size of the woman's hand and fingertips. In addition, the use of an occasional "manual" or stationary, mechanical or electronic stopwatch does not exclude the sudden stop of the stopwatch due to its unavailability for measurement. And, finally, the use of well-known stopwatches does not exclude the woman's visual observation of the changing value and the influence on the final value of the desire of the woman herself.
Therefore, the use of the fingertips of a pregnant woman and a known stopwatch of indeterminate type, shape and quality to measure the duration of the period of fetal immobility during diagnostic apnea does not standardize the acquisition of values, their objectivity and the repeatability of the values obtained, which reduces efficiency, accuracy and safety.
In addition, the pads of the fingers and known stopwatches do not provide high accuracy and efficiency due to the fact that they do not provide measurement in any outpatient setting. The fact is that in some cases, a pregnant woman may temporarily lack a stopwatch, but at the same time, there may suddenly be a need for an urgent diagnosis of the condition of the fetus. In addition, not every woman, not every time and not under any conditions, can determine the upper body of her fetus inside the uterus from the first time with her fingers, and also distinguish the motor activity of her stomach, intestines, abdominal muscles from the motor activity of the fetus.
All this allows untimely diagnosis of the sudden onset of low fetal resistance to intrauterine hypoxia and a belated decision on the need for urgent permission for delivery by Caesarean section to save the health and life of the fetus.

The disadvantage of the known device is that it has a narrow range of application, low accuracy, efficiency, accuracy and safety, since it is not intended for diagnosing the condition of the fetus in the second half of pregnancy, since it does not have a touch sensor, a stopwatch, a block for receiving, processing and transmitting information at a distance to visualize the obtained value using a smartphone.
The disadvantage of the known device is that it has a narrow range of application, low accuracy, efficiency and safety, since it is not intended for diagnosing the condition of the fetus in a pregnant woman in the second half of pregnancy. In addition, this device is not applicable to a pregnant woman at home, since the device is not transportable, since the known device requires connection to an additional battery and to a home desktop computer.
All this limits the dynamics of measurements and reduces the effectiveness of diagnosing the health of the fetus during critical periods of its physiological and anatomical development, as well as in assessing the role of the characteristics of course therapeutic nutrition and / or exercise regimen in a pregnant woman and the quality of the environment.
In addition, the known device does not have a stopwatch and paper information carrier, so it is not intended to measure the exact duration of the period of immobility of the fetus during diagnostic apnea and record the obtained values manually.
(Ahyeon Koh, Daeshik Kang, Yeguang Xue, Seungmin Lee, Rafal M. Pielak et al.. Science Translational Medicine. 2016;8(366): 366ra165.DOI10.1126/scitranslmed.aaf2593).A flexible wearable analytical system is known for studying the dynamics and state of sweat in an adult (preferably an athlete) in the form of a skin application, which is a device with a diameter of 3 centimeters and a thickness of about 700 micrometers, consisting of several layers. A biocompatible acrylic adhesive layer with a high degree of adhesion (about 5 Newtons, which is about 5 times higher than the characteristics of a medical patch) is in contact with the skin, which ensures a snug fit even on thick, vellus hair and heavily sweaty skin. It has 5 holes with a diameter of 3 mm for separate collection of sweat. The collected sweat enters a microfluidic device made of polydimethylsiloxane, consisting of five separate containers connected by microchannels with their own collecting holes. Closer to the center are four round 4 mm reservoirs for the analysis of acidity, chloride ions, lactate and glucose. The reservoirs are surrounded by a serpentine channel to assess the volume and rate of perspiration. Excess sweat is removed through microchannels that open on the side of the device. An annular copper antenna with a chip for short-range non-contact communication is mounted in the outer layer, transmitting information to reading devices, for example, to a smartphone. All qualitative and quantitative analyzes in the microfluidic device are performed by enzymatic or chromogenic chemical reactions on a cellulose matrix and are evaluated colorimetrically (by reagent color change and its intensity). One white and four black reference markers are applied to the outside of the system for automatic readout calibration. The sensitivity of the device is 0.5 units for pH and 0.2, 0.3 and 0.1 millimoles per liter for chloride, lactate and glucose, respectively. The mechanical characteristics of the device, such as extensibility and flexibility, match the parameters of the skin, which ensures a snug fit and no discomfort when worn. The device can hold up to 50 microliters of sweat, allowing it to operate for one to six hours, depending on the area of skin and the level of sweating (in humans, it ranges from 12 to 120 microliters per hour per square centimeter of skin). Sweat samples from reservoirs can be used for subsequent laboratory analysis. (Ahyeon Koh, Daeshik Kang, Yeguang Xue, Seungmin Lee, Rafal M. Pielak et al.. Датчик частоты биения сердца матери и размещенный на бандаже датчик регистрации частоты биения сердца плода подключены к внешнему блоку индикации (RU 2232607 С2).An acoustic stimulator of the development of a child in the prenatal period is known, equipped with a bandage covering the anterior abdominal wall, the surface of which, facing the abdominal wall, is divided into separate cells, each of which contains a system for generating a stimulating sound signal, including a fetal movement registration sensor connected via vibration shaper to the sound signal generator. The mother's heart rate sensor and the fetal heart rate sensor located on the bandage are connected to an external display unit (RU 2232607 C2).
The disadvantage of the known device is the narrow range of application, low efficiency, accuracy and safety, since it is not intended for self-diagnosis of the health of the fetus in a pregnant woman on an outpatient basis. Moreover, the known device itself increases the motor activity of the fetus, regardless of the presence of intrauterine hypoxia and the size of the adaptive reserves of the fetus to it. Therefore, the device accelerates the depletion of the remaining reserves of fetal resistance to hypoxia and contributes to increased hypoxic damage to the fetal brain. The known device does not have a stopwatch and paper media. Therefore, it is not intended to independently measure the duration of the fetal immobility period during diagnostic apnea and record the obtained values in an outpatient setting.

In a bandage made in the form of a belt, consisting of 2 end tapes with an expansion between them in the middle part, the placement of a fetal movement sensor in the upper edge in the middle of the length of each end tape ensures high efficiency of recording the motor activity of the fetus in the womb from the anterolateral parts the surface of her abdomen, to which the bandage fits snugly when worn by a pregnant woman. The fact is that the indicated placement of sensors increases the efficiency and accuracy of diagnosing fetal motor activity in the working state of the bandage, since it provides information from the right and left sides of the pregnant woman's abdomen and the body of her fetus inside the uterus simultaneously. Therefore, if the fetus is located with its back to one of the sensors, the fetus will not be with its back to the second sensor, and based on this, it is this sensor that will more reliably register the end of the period of the immobile state of the fetus by the sudden appearance of motor activity of the fetal limbs, since they are in crossed position on the front surface of his torso.
The implementation of an electronic device with the function of an electronic stopwatch, with a switch with a button for manual activation and the implementation of a stopwatch with the function of automatically measuring the duration of the immobile state of the fetus during diagnostic apnea expands the range of application, increases efficiency, accuracy and safety. The fact is that the presence of a switch of an electronic device with a button for manual switching on ensures the savings in the consumption of energy by it and the timeliness of switching on and off the electronic device, namely, during the period chosen by a pregnant woman to diagnose the state of health of her fetus. This, in turn, ensures the economy of energy consumption in the device, a long period of its operation and diagnostics of the health of the fetus using a diagnostic bandage. The addition of an electronic device with the function of an electronic stopwatch with automatic measurement of the duration of the immobile state of the fetus during diagnostic apnea provides objectivity, standardization and repeatability of the results.
Execution of the inner surface of tapes from oilcloth fabric of white color with a table placed on it with four longitudinal rows of cells placed one below the other and simultaneously forming transverse columns, with cells of the upper row having a sequential designation of gestational age in the range from 20 to 42 weeks, with cells the lower rows, made empty with the possibility of written manual recording of the measured values of the diagnostic apnea test in them in the range of up to 10 seconds in the upper row, in the range of 11-29 seconds in the middle row, in the range of more than 30 seconds in the lower row, with cells stained in the bottom, middle and top rows respectively in green, yellow and red color expands the application range, improves efficiency, accuracy and safety. The fact is that the oilcloth fabric of white color ensures the registration of the data received by the pregnant woman using a conventional writing device such as a ballpoint pen, and also ensures the storage of these records until the birth of the child. At the same time, indicating the gestational age in weeks from 20 to 42 allows you to register the obtained values in the cells of the column corresponding to this period, from the beginning of the second half of pregnancy until its end and the birth of a child. Moreover, red staining of the cells of a row designed to record the duration of the immobile state of the fetus up to 10 seconds increases efficiency and accuracy, since the red color is associated in society with danger and prohibition. Therefore, placing the obtained values less than 10 seconds in a row of red color helps a pregnant woman to conclude that the health of her fetus is unsatisfactory, that an urgent medical examination is needed, and that a natural delivery is contraindicated. In turn, placing the measured values in the cells with green color allows the woman to conclude that her fetus is in a satisfactory state of health, that there are no indications for additional medical examination and that there is no contraindication for natural delivery. And, finally, placing the found values in the cells of the yellow row allows a woman to conclude that her fetus is in a satisfactory state of health, but requires careful monitoring by obstetrician-gynecologists and about the desirable hospitalization in order to maintain pregnancy.
In addition, the implementation of the bandage with tables placed on the inner surface of both tapes, on the one hand, provides a high aesthetic result, on the other hand, it provides convenience for registering values for right-handers and left-handers, respectively, in a table placed in the left and right end tape, and also provides the possibility of additional registration of some additional values in the free table, if necessary.
Thus, the implementation of a bandage for a pregnant woman with a fetal motor activity sensor located in the upper edge of each tape in the middle of its length, with an electronic device for receiving, processing and transmitting electronic information, made with the function of an electronic stopwatch and with a switch with a button for manual activation, with a stopwatch designed to automatically measure the duration of the immobile state of the fetus during diagnostic apnea, with the inner surface of the tapes made of white oilcloth fabric with a table placed on it with four longitudinal rows of identical cells placed one below the other and forming transverse columns, made of in which the cells of the upper row have a sequential designation of gestational age in the range from 20 to 42 weeks, the lower rows of cells are empty with the possibility of written manual registration in them of the measured values of the diagnostic apnea test in the range of up to 10 seconds in the upper row, in in the range of 11-29 seconds in the middle row, in the range of 30 or more seconds in the bottom row, indicating these designations in the cells of the leftmost column of the table, with the coloring of the lower, middle and upper rows of cells, respectively, in green, yellow and red, provides a pregnant woman with a woman on an outpatient basis to independently diagnose the state of health of her fetus. At the same time, such a device provides high reliability, efficiency and accuracy of supporting the abdomen of a pregnant woman. The applied technical solutions keep the mass and size of the device, as well as its safety within the limits allowed for storage and operation of the bandage by pregnant women at home.

The device is used as follows.
Currently, on an outpatient basis, the diagnosis of the health of the fetus is carried out by a pregnant woman with low accuracy, since there is no special diagnostic bandage for a pregnant woman. The resistance of the fetus to intrauterine hypoxia is measured using a diagnostic apnea test, in which a pregnant woman determines the duration of the immobile state of the fetus using her fingers and a handy stopwatch. At the same time, the duration of the period of the immobile state of the fetus is determined subjectively and with low accuracy, which reduces the efficiency, accuracy and safety of diagnosing the state of fetal health.
In this regard, in order to expand the range of application, increase the efficiency, accuracy and safety of diagnosing the state of fetal health on an outpatient basis, pregnant women choose a diagnostic bandage that includes sensors for fetal motor activity, an electronic device for collecting, processing and transmitting electronic information with the function of an electronic stopwatch that automatically measures the duration of the immobile state of the fetus during diagnostic apnea, and with a switch with a button for manual activation, as well as a table on the inner surface of both end bands of the bandage for recording the obtained values.
To apply the diagnostic bandage, a pregnant woman opens the box in which her diagnostic bandage is stored, takes it out, presses the switch button of the bandage electronic device, brings her smartphone to her and sets it up and/or makes sure that information is received from the electronic device. After that, the woman puts the bandage on the couch, sofa or bed, straightens it across the sleeping place, exposes her stomach, lies with her lumbar part of her back on the straightened bandage, pulls the bandage bands under her stomach and fixes them in the chosen position by fastening the band fasteners. Before measurement, by palpation determines the location of the switch button, brings the pad of the selected finger of the working hand close to it and leaves it in a position of light contact with the button without pressure. Then the woman holds her breath and at the same time presses the switch button once with her finger. After that, the woman removes her finger from the button, takes the smartphone in the palm of her working hand and, after the apnea stops, brings the smartphone to the front surface of the bandage in the area where the switch button is located, turns on the smartphone and reads the value of the duration of the immobile state of the fetus during diagnostic apnea from its screen, after which the woman presses the switch button and turns off the electronic device of the brace. Then the woman unfastens the fastener of the tape under her stomach, turns the selected end of the tape inside out, straightens the tape and the table on it. Before that, the woman determines the estimated gestational age and finds this period in weeks in the top row of the table. Immediately in one of the cells of the corresponding empty column, using a writing device, the received value in seconds is registered in one of the cells of the table. Before this, the woman studies the recommendations indicated in the leftmost column, and to register the value, she selects the row of red, yellow or green, respectively. So, in the case of a value equal to or greater than 30 seconds, the woman evaluates the resistance of the fetus to hypoxia as good and prepares for the birth of the fetus through natural childbirth. If the value is equal to or less than 10 seconds, the woman assesses the resistance of the fetus to hypoxia as unsatisfactory, excluding the birth of a healthy fetus through natural childbirth, requiring urgent medical care and delivery by Caesarean section. In the case of a value in the range of 11-29 seconds, a woman assesses the resistance of the fetus to hypoxia as satisfactory, but requires careful monitoring by obstetrician-gynecologists, therefore, hospitalization of the woman is desirable for the purpose of additional diagnosis and preservation of pregnancy. After completing the registration of the obtained data in the table and diagnosing the state of health of the fetus, the woman again lays and straightens the bandage on the surface of the selected lying place, exposes her back and stomach, lies with her lumbar part of the back on the straightened bandage across its length, tightens the bandage bands and fastens them with a clasp below the abdomen. After that, the pregnant woman uses a bandage according to generally accepted technology to support her abdomen until the need to remove it and / or re-diagnose the health of the fetus.
Thus, the proposed diagnostic bandage for a pregnant woman, equipped with a sensor placed in the upper edge of each tape in the middle of its length, an electronic device for collecting, processing and transmitting information, made with the function of an electronic stopwatch with automatic measurement of the duration of the immobile state of the fetus during diagnostic apnea, with a push-button switch, with the inner surface of the tapes made of white oilcloth with a table placed on it with four longitudinal rows of identical cells placed one under the other and forming transverse columns, with the cells of the upper row having a sequential designation of gestational age in the range from 20 up to 42 weeks, with the cells of the lower rows made empty with the possibility of written manual recording of the measured values of the diagnostic apnea test in them in the range of up to 10 seconds in the upper row, in the range of 11 - 29 seconds in the middle row, in the range of more than 30 seconds in the lower m row, with these designations indicated in the cells of the leftmost column of the table, with green, yellow and red colored cells of the lower, middle and upper rows of cells, respectively, expands the range of application, improves efficiency, accuracy and safety.
