Воспалительные и дегенеративные заболевания тазобедренных суставов у собак являются проблемными хирургическими патологиями, поскольку плохо поддаются терапевтическому лечению при энтеральном и парентеральном путях введения лекарственных средств. Это связано с тем, что при данных способах введения крайне сложно создать в области патологического очага (костная и хрящевая ткань головки и шейки бедренной кости) их терапевтическую концентрацию.Inflammatory and degenerative diseases of the hip joints in dogs are problematic surgical pathologies, since they are poorly amenable to therapeutic treatment with enteral and parenteral routes of drug administration. С целью наиболее адресного и эффективного лекарственного воздействия на патологический очаг при лечении воспалительных и дегенеративных заболеваний тазобедренного сустава у собак, существует метод введения препаратов непосредственно в шейку и головку бедренной кости (Crovace A.M. Crovace // VCOT Open. - January 2020. - №3. - PP. 1-10.).With the aim of the most targeted and effective medicinal effect on the pathological focus in the treatment of inflammatory and degenerative diseases of the hip joint in dogs, there is a method of administering drugs directly into the neck and head of the femur (Crovace AM Treatment of Monolateral Disease with Autologous Bone Marrow Mononuclear Cells in 32 Dogs / AM Crovace, L. Известен способ 1 - процедура проводится под общей анестезией животного под контролем рентгеновской С-дуги или компьютерной томографии. Для этого, ориентировочно, между большим и малым вертелом бедренной кости, производится прокол кожи и мягких тканей иглой Джамшиди 18-го калибра, с проникновением в толщу шейки и головки бедра. После введения вышеуказанной иглы, к ней присоединяется шприц с незначительным объемом рентгеноконтрастного средства, а затем проводится его инъекция с одновременным рентгеновским мониторингом. Рентгеноконтрастное вещество инъецируется непосредственно перед использованием лекарственного средства, что позволяет предварительно оценить точность вкола иглы (Фиг. 2 - А и Фиг. 3 - A) (Crovace A.M. Недостатки по способу 1:Disadvantages of method 1:
a) - сложность осуществления точного продольного проведения иглы по центру шейки бедренной кости характеризуется тем, что при введении иглы без рентгенологического контроля можно допустить ее выход за пределы компактного вещества суставной поверхности головки бедра. Это может привести к выходу лекарственного вещества не в состав костной ткани, а в полость тазобедренного сустава, и как следствие этого, снижению терапевтического эффекта применяемого препарата; б) - дополнительные затраты времени и денежных средств на введение рентгеноконтрастного йодсодержащего вещества и проведения контрольного рентгенографического исследования; в) - увеличение анестезиологических рисков пациента вследствие более длительного нахождения животного под общим наркозом; г) - вынужденное увеличение лучевой нагрузки на хирурга при рентгенографическом мониторинге введения иглы; д) - вероятность изменения свойств, лекарственных и биологических препаратов, применяемых для лечения хирургических патологий головки бедра, при контактировании с йодсодержащими рентгеноконтрастными веществами, используемыми для проведения контрольного рентгенографического исследования.a) - the difficulty of implementing an accurate longitudinal passage of the needle along the center of the femoral neck is characterized by the fact that when the needle is inserted without x-ray control, it can be allowed to go beyond the compact substance of the articular surface of the femoral head. Технический результат достигается тем, что инъекцию лекарственного средства проводят под общим наркозом животного, с латеральной поверхности бедра, в верхней части большого вертела бедренной кости, с отклонением иглы краниально, под углом 15градусов по отношению к оси шейки и головки бедра. При этом иглу проводят вдоль бедренной шейки на уровне нижней границы верхней трети межвертлужного гребня по упора в кость, затем при помощи мандрена вкручивают и проталкивают в толщу кортикального слоя кости еще на 0,3-0,5 см глубины, достигая центра внутри бедренной головки.The technical result is achieved by the fact that the injection of the drug is carried out under general anesthesia of the animal, from the lateral surface of the thigh, in the upper part of the greater trochanter of the femur, with the needle deflected cranially, at an angle of 15 degrees relative to the axis of the neck and femoral head.
