Abstract
Description
The invention relates to medicine, in particular to dentistry. The method consists in a new scheme for preventing bacterial complications after dental interventions and accelerating the healing time, removing edema and inflammatory changes in tissues using inhalation of a thermal helium-oxygen mixture in a ratio of 30% oxygen and 70% helium. The method is based on a new approach to the management of patients after surgical dental interventions and allows 100% of patients to improve the condition after implantation, tooth extraction, bone grafting, open and closed sinus lift, cystectomy and minimizes the postoperative period of healing with the removal of edema and prevention of the development of bacterial complications without antibiotic therapy.
Также известен способ купирования боли и отека приемом нестероидных противовоспалительных препаратов (https://e-stomatology.ru/pressa/periodika/st_d_f/1_20121).The prior art known method for the treatment of postoperative pain and swelling after dental interventions by injecting drugs based on glucocorticoids (prednisolone) intramuscularly (https://dental-press.ru/ru/nauka/textbooky2291/view), as well as the use of antibiotics in the postoperative period (amoxiclav, azithromycin in various dosages) (https://megadenta.ru/blog/article/_aview_b79). These methods are widely used in dental practice to prevent pain and swelling in the postoperative period. Also known is a method of stopping pain and swelling by taking non-steroidal anti-inflammatory drugs (https://e-stomatology.ru/pressa/periodika/st_d_f/1_20121).
The disadvantage of the introduction of glucocorticoids in the postoperative period is their immunosuppressive properties, as well as the suppression of microcirculation. The disadvantages of antibiotic therapy in the postoperative period include the possibility of intolerance to allergic reactions, the development of complications from the gastrointestinal tract, and dysbacteriosis. The disadvantages of the use of non-steroidal anti-inflammatory drugs in the postoperative period include the possible development of complications from the gastrointestinal tract due to stimulation of the production of prostaglandins and an irritating effect on the gastrointestinal tract, the development of anemia.
The objective and technical results of the present invention are:
- расширение спектра средств для снятия отека и ускорение заживления после различных операций;- expanding the range of means for removing edema and accelerating healing after various operations;
- улучшение газового состава крови и снижение уровня лактаты крови, что свидетельствует об уменьшении тканевой гипоксии по сравнению с традиционным лечением;- improvement of the gas composition of the blood and a decrease in the level of blood lactate, which indicates a decrease in tissue hypoxia compared to traditional treatment;
- улучшение реологического свойства крови и уменьшение ее вязкости в сосудах малого диаметра на фоне нормализации процессов агрегации и дезагрегации тромбоцитов, что устраняет явление ишемии и способствует снятию отека тканей, профилактике гипоксии и скопления недоокисленных продуктов обмена тканей;- improvement of the rheological properties of blood and a decrease in its viscosity in vessels of small diameter against the background of normalization of the processes of aggregation and disaggregation of platelets, which eliminates the phenomenon of ischemia and helps to relieve tissue edema, prevention of hypoxia and accumulation of underoxidized products of tissue metabolism;
- ускорение регенерации тканей за счет эффекта выраженной оксигенации и стимуляции микроциркуляции.- acceleration of tissue regeneration due to the effect of pronounced oxygenation and stimulation of microcirculation.
The task and the technical result are achieved by using a method for preventing bacterial and viral complications, relieving edema and postoperative discomfort, post-traumatic inflammation in the tissues of the oral cavity by exposure to a helium-oxygen mixture at a ratio of helium and oxygen of 70% to 30%, respectively, with a temperature equal to 90 °, on the pathogenic oral cavity and respiratory tract by inhalation, each lasting 10 minutes, immediately after surgery or after 24 hours, or after 48 hours.
During surgical interventions in the postoperative period in tissues due to damage to the vascular bed, post-traumatic edema and inflammation always develop, which leads to stagnation in the microcirculatory bed and a decrease in local immunity, as well as a violation of metabolic processes in tissues, temporary tissue hypoxia. Due to its low density, helium is a very fluid gas, effectively penetrating through cell membranes. So helium provides a linear (laminar) movement of oxygen gas, dragging oxygen molecules with it and thus ensuring the delivery of sufficient oxygen to the alveoli, more than when breathing ordinary air or even pure oxygen.
As a result of postoperative intervention, as a result of temporary local hypoxia, scar connective tissue is formed at the intervention site, since this is the only tissue that can exist in our body under conditions of hypoxia and reduced microcirculation. The use of a helium-oxygen mixture heated to 90 ° for inhalation normalizes the gas composition of the blood and acid-base balance, improving the diffusion of oxygen through the alveolar-capillary membrane, has a powerful thermal and heat-reflex effect on the body, prevents and treats local tissue hypoxia of tissues due to the fact that that helium five times increases the penetration of oxygen into tissues and improves microcirculatory blood flow and blood gas composition. Also, due to the special thermal conductivity of helium at a helium temperature of 90 °, there is no burn and damage to the mucous membranes of the oral cavity and tissues of the respiratory tract, but a pronounced bacteriostatic and antiviral effect is noted. This reduces the likelihood of infectious complications in the postoperative period. By activating microcirculation in tissues, a local immune response is also stimulated, providing natural protection.
The maxillofacial region is characterized by the most developed network of vessels, both blood and lymphatic. Therefore, surgical interventions in this area are accompanied by significant damage to blood and lymphatic vessels, severe edema in the postoperative period. This can lead to the development of complications - deformation and violation of the consistency of the sutures in the surgical wound with the formation of deforming rough scars that worsen the aesthetic and functional result of the operation.
Helium is an inert gas, which, due to its physical properties, is able to accelerate the delivery of oxygen to tissues without any negative effect on the respiratory and circulatory systems, as well as on brain function. This ability of helium makes it possible to treat any form of tissue hypoxia, achieving a reduction in the healing time in the postoperative period, and improves the results of surgical intervention due to the prevention of edema and the formation of rough scars on the tissue, normalizing cellular respiration, metabolic processes and microcirculation, improving blood oxygenation and its rheological properties.
