На данный момент в практике лечения опухолей головы имеет место способ химиотерапии путем интракаротидного последовательного введения озонированного физиологического раствора (ОФР) и химиосенсибилизатора с последующим введением химиопрепарата. При реализации данного метода отмечается повышение эффективности лечения за счет синергического действия ОФР, повышающего оксигенацию опухоли с возбуждением радикально-цепных реакций окисления (реакция Габера-Вейса) и, вместе с тем, потенцирующего действие химиосенсибилизатора в отношении восприимчивости опухолевой ткани к химиопрепарату в условиях повышенного напряжения кислорода (Медяник И.А., Мухина И.В., Дворников А.В. патент на изобретение RU2418587C1 2010-01-21). Существенным недостатком данного метода является «открытость» системы, что подразумевает поступление введенных ОФР, химиосенсибилизатора и химиопрепарата в системный кровоток с последующим угнетающим воздействием указанных препаратов на печень и другие органы. Указанные явления влекут за собой снижение возможности реализации указанного метода за счет как токсического воздействия на органы, так и повышения объема противопоказаний к осуществлению данного метода.At the moment, in the practice of treating head tumors, there is a method of chemotherapy by intracarotid sequential administration of ozonized saline (OSS) and a chemosensitizer, followed by the administration of a chemotherapy drug. A significant disadvantage of this method is the “openness” of the system, which implies the entry of the administered OFR, chemosensitizer and chemotherapy drug into the systemic circulation, followed by a depressing effect of these drugs on the liver and other organs. These phenomena entail a decrease in the possibility of implementing this method due to both toxic effects on organs and an increase in the volume of contraindications to the implementation of this method.
При разработке предлагаемого нами метода была рассмотрена идея методики гипертермической химиоперфузии плевральной полости, суть которой заключается в создании изолированного контура кровообращения в плевральной полости, что позволяет проводить химиотерапию плевральной полости при минимальном попадании цитостатика в системный кровоток. Данная методика успешно зарекомендовала себя при реализации лечения новообразований плевральной полости. С этой целью используется аппарат для гипертермической перфузии, состоящий из резервуара на основе плевральной банки, набора стерильных магистралей, теплообменника с термостатом, роликового насоса, гравитационного лейкоцитарного фильтра, следящей аппаратуры и термостата. Цель предлагаемого нами метода - обеспечить лечение нерезектабельных новообразований головы и шеи за счет таргетного действия более высоких доз цитостатика, минимизируя при этом его поступление в системный кровоток, что существенно облегчит процесс лечения онкологических пациентов и, как следствие, качество жизни после комбинированного лечения.The purpose of our proposed method is to provide treatment of unresectable neoplasms of the head and neck due to the targeted action of higher doses of cytostatic, while minimizing its entry into the systemic circulation, which will greatly facilitate the treatment of cancer patients and, as a result, the quality of life after combined treatment.
Решение поставленной задачи достигается осуществлением открытого доступа к сосудам сердца методом срединной стернотомии, мобилизации и взятии на сосудистые обтяжки брахиоцефального ствола (БЦС), левой общей сонной артерии (ЛОСА), левой подключичной артерии (ЛПА) (возможно предварительное формирование сонно-подключичного анастомоза слева для перфузии левой сонной артерии из бассейна подключичной артерии). Затем трансперикардиально мобилизуется верхняя полая вена (ВПВ). БЦС, ЛОСА и ВПВ канюлируются. За счет канюли в ВПВ заполняют кардиотомный резервуар. Предварительно проводят системную гепаринизацию пациента. Для пережатия артерий и вен верхних конечностей используют пневматические манжеты, которые раздувают к моменту заполнения кардиотомного резервуара. Изоляция кровообращения достигается пережатием БЦС, ЛОСА, ЛПА тотчас в месте отхождения от дуги аорты. ВПВ пережимают в месте слияния брахиоцефальных вен. Пережатие крупных сосудов осуществляется с целью минимизирования сброса цитостатика в системный кровоток. По созданному замкнутому изолированному контуру начинают изолированную нормотермическую химиоперфузию головы, что уменьшает вероятность возникновения воспалительного процесса и отека в перфузируемых тканях. Длительность процедуры составляет 30-60 минут, что достаточно для попадения цитостатика в сосуды, питающие опухоль. Использование мембранного оксигенатора позволяет достигнуть состояния гипероксии, потенцирующей противоопухолевый эффект комплексной терапии злокачественных новообразований. В ходе перфузии проводится постоянный мониторинг газового состава крови и ее кислотно-щелочных показателей. Заканчивается процедура вымыванием цитостатика из сформированного контура кровообращения путем перфузии специальным раствором, состоящим из 1500 мл 0,9% раствора хлорида натрия, 500 мл гелофузина и 300 мл эритроцитной взвеси. To clamp the arteries and veins of the upper extremities, pneumatic cuffs are used, which are inflated by the time the cardiotomy reservoir is filled. Isolation of blood circulation is achieved by clamping the BCS, VOC, LPA immediately at the point of origin from the aortic arch. The SVC is clamped at the confluence of the brachiocephalic veins. Clamping of large vessels is carried out in order to minimize the release of cytostatics into the systemic circulation. According to the created closed isolated circuit, isolated normothermic chemoperfusion of the head is started, which reduces the likelihood of an inflammatory process and edema in the perfused tissues. The duration of the procedure is 30-60 minutes, which is enough for the cytostatic to enter the vessels that feed the tumor. The use of a membrane oxygenator makes it possible to achieve a state of hyperoxia, which potentiates the antitumor effect of the complex therapy of malignant neoplasms. During perfusion, blood gas composition and its acid-base indicators are constantly monitored. The procedure ends with the washing out of the cytostatic agent from the formed circulatory circuit by perfusion with a special solution consisting of 1500 ml of 0.9% sodium chloride solution, 500 ml of gelofusin and 300 ml of erythrocyte suspension. The washing out of the cytostatic agent is carried out so that the chemotherapy drug does not enter the general circulation after the implementation of the proposed method.
Предлагаемый нами метод позволяет решить ряд проблем современной онкологии области головы и шеи, а именно опасность осложнений, полученных в ходе системной химиотерапии. К тому же появляется возможность использовать более высокие дозы цитостатиков, что усилит терапевтический эффект, снизит проявление системной интоксикации организма после системной химиотерапии и повысит медиану выживаемости данной категории пациентов.The proposed method allows us to solve a number of problems of modern oncology of the head and neck region, namely the risk of complications resulting from systemic chemotherapy. Метод основывается на создании замкнутого контура путем канюляции брахиоцефального ствола (1), левой общей сонной артерии (2), левой подключичной артерии (3), верхней полой вены (4) и использовании пневматических манжет (5) на верхних конечностях (уровень верхней трети).The method is based on the creation of a closed circuit by cannulating the brachiocephalic trunk (1), the left common carotid artery (2), the left subclavian artery (3), the superior vena cava (4) and the use of pneumatic cuffs (5) on the upper limbs (upper third level) .
Способ «открытой» нормотермической изолированной химиоперфузии головы и шеи реализуется посредством следующих действий: выполняется срединная стернотомия, устанавливается реечный ранорасширитель (расширитель Финочетто). Мобилизуются и фиксируются сосудистыми турникетами-обтяжками: БЦС (1), ЛОСА (2), ЛПА (3). Т-образным разрезом вскрывается перикард, который фиксируется 4 отдельными узловыми швами к коже. Мобилизуется и берется на сосудистую обтяжку ВПВ (4). На БЦС и ЛОСА на расстоянии 1-3 см от устья накладывается кисетный шов нитью пролен 3/0, нить берется на турникет Руммеля. Выполняется тотальная системная гепаринизация до достижения активированного времени свертывания 400-500 секунд (внутривенно вводят гепарин из расчета 100 Ед/кг массы тела пациента). Канюлируется БЦС и ЛОСА канюлями диаметром 1/4-3/8 дюйма, в направлении «к голове». Канюлируется ВПВ канюлей диаметром 1/4-3/8 дюйма, в направлении «к голове». Замыкают изолированный контур изолированной перфузии головы: забор венозной крови от головы, шеи, верхних конечностей: кровь из ВПВ в кардиотомный резервуар, а возврат - из последнего в БЦС и ЛОСА. Запускают параллельное кровообращение по этому контуру. Осуществляют сосудистую изоляцию головы: а) в манжеты (5) установленные на право и левом плече нагнетают воздух до давления 300 мм рт ст.; б) пережимают БЦС, ЛОСА, ЛПА тотчас у устья; г) пережимают ВПВ в тотчас в месте перед впадением в правое предсердие. Начинается изолированная перфузия головы. Перфузию производят с объемной скоростью 800 мл в минуту по сформированному временному контуру раствором, состоящим из 300 мл эритроцитной взвеси, 700 мл 0,9% раствора хлорида натрия, высоких доз цитостатика (мелфалан 1,5 мг на 1 кг массы тела) в условиях гипероксии и нормотермии (37,5 градусов Цельсия), которая достигается подключением мембранного оксигенатора с теплообменником. Длительность изолированной перфузии головы - 30-60 минут. Каждые 15 минут выполняется анализ газов крови, контролируется кислотно-основное состояние. По окончании процедуры проводят перфузию головы раствором, состоящим из 1500 мл 0,9% раствора хлорида натрия, 500 мл гелофузина и 300 мл эритроцитной взвеси. Удаляют канюли и ушивают места канюляции сосудов. Нейтрализуют гепарин раствором протамина сульфата из расчета 1,5 мг протамина сульфата на каждые 100 ME гепарина. A purse-string suture with a 3/0 prolene thread is applied to the BCS and VOC at a distance of 1-3 cm from the mouth, the thread is taken on the Rummel tourniquet. Total systemic heparinization is performed until an activated clotting time of 400-500 seconds is reached (heparin is administered intravenously at the rate of 100 U/kg of the patient's body weight). BCS and LOSA are cannulated with 1/4-3/8 inch cannulas, in the direction “toward the head”. The SVC is cannulated with a cannula with a diameter of 1/4-3/8 inches, in the direction "towards the head". The isolated circuit of the isolated perfusion of the head is closed: venous blood sampling from the head, neck, upper limbs: blood from the SVC to the cardiotomy reservoir, and return from the latter to the BCS and VOC. Parallel blood circulation is started along this circuit. Vascular isolation of the head is carried out: a) air is pumped into the cuffs (5) installed on the right and left shoulder to a pressure of 300 mm Hg; b) pinch BCS, VOC, LPA immediately at the mouth; d) clamp the SVC immediately in place before flowing into the right atrium. Isolated perfusion of the head begins. Perfusion is performed at a volumetric rate of 800 ml per minute along the formed time contour with a solution consisting of 300 ml of an erythrocyte suspension, 700 ml of a 0.9% sodium chloride solution, high doses of a cytostatic agent (melphalan 1.5 mg per 1 kg of body weight) under conditions of hyperoxia and normothermia (37.5 degrees Celsius), which is achieved by connecting a membrane oxygenator with a heat exchanger. The duration of isolated perfusion of the head is 30-60 minutes. Every 15 minutes, a blood gas analysis is performed, the acid-base state is monitored. At the end of the procedure, the head is perfused with a solution consisting of 1500 ml of 0.9% sodium chloride solution, 500 ml of gelofusin and 300 ml of erythrocyte suspension. The cannulas are removed and the cannulated vessels are sutured. Neutralize heparin with a solution of protamine sulfate at the rate of 1.5 mg of protamine sulfate for every 100 IU of heparin. Layered suture of the sternotomy wound.
