Синдром диабетической стопы (СДС) является поздним осложнением сахарного диабета (СД) и одной из основных причин выполнения нетравматических ампутаций нижних конечностей. Стойкая утрата трудоспособности, инвалидизация больных, высокая послеоперационная летальность, повышенный риск потери контралатеральной конечности относят осложнение к социально значимым. В последние годы интерес к данной проблеме существенно возрос. Следует отметить, что в изучении СДС достигнут значительный прогресс. Обозначены конкретные мероприятия, направленные на снижение количества высоких ампутаций. Доказано, что внедрение междисциплинарного подхода к ведению данной категории больных приводит к значимому снижению количества ампутаций нижних конечностей. Определены факторы, замедляющие репарацию хронических ран (Международное Соглашение по диабетической стопе, 2019). Однако, многие вопросы требуют своего дальнейшего изучения. Прежде всего, большую озабоченность вызывают сроки заживления ран и частота их рецидивов. На фоне стандартного лечения лишь 20-30% язвенных дефектов эпителизируются за 12 недель лечения (Jeffcoat W. (2006), Lev-Tov Н. (2013), Удовиченко О.В. (2014)). В 70-80% случаев сроки заживления составляют год и более. Риск рецидивов высок: от 40% в течение года, до 65% в течение 5 лет. (Armstrong D., 2016). С этой точки зрения особый интерес приобретает возможность прогнозирования исхода хронической раны.Diabetic foot syndrome (DFS) is a late complication of diabetes mellitus (DM) and one of the main reasons for performing non-traumatic lower limb amputations. Persistent disability, disability of patients, high postoperative mortality, an increased risk of loss of the contralateral limb, the complication is considered socially significant. In recent years, interest in this problem has increased significantly. It should be noted that significant progress has been made in the study of VTS. Specific measures are outlined to reduce the number of high amputations. It has been proven that the introduction of an interdisciplinary approach to the management of this category of patients leads to a significant decrease in the number of lower limb amputations. Factors slowing down the repair of chronic wounds have been identified (International Agreement on the Diabetic Foot, 2019). However, many questions require further study. First of all, the timing of wound healing and the frequency of their recurrence are of great concern. Against the background of standard treatment, only 20-30% of ulcerative defects are epithelized within 12 weeks of treatment (Jeffcoat W. (2006), Lev-Tov N. (2013), Udovichenko O.V. (2014)). In 70-80% of cases, the healing time is a year or more. The risk of relapse is high: from 40% within a year to 65% within 5 years. (Armstrong D., 2016). From this point of view, the possibility of predicting the outcome of a chronic wound is of particular interest.
Проблема прогноза заживления ран, как острых, так и хронических является предметом пристального изучения специалистов. Известен метод динамического измерения площади раневой поверхности. Данный метод был предложен Поповой Л.Н. в 1942 году. Заключается в том, что на рану накладывается стерильный прозрачный материал (целлофан, пленка), на который наносятся контуры раны с последующим вычислением ее площади. Через 6-10 дней процедуру повторяют, сравнивают полученные значения площади раны и вычисляют процент уменьшения площади за сутки по специальной формуле. По мнению авторов, в норме рана уменьшается в размерах за сутки не менее чем на 4%. Соответственно, если сокращение размеров раны идет согласно графику, то можно рассчитать сроки заживления. Если же рана сокращается медленнее, то следует пересмотреть лечебную тактику. Впоследствии предложенная методика усовершенствовалась и модифицировалась, в частности к формуле добавлялись коэффициенты, учитывающие форму раны (М.И. Кузин, Б.М. Костюченок "Раны и Раневые инфекции", 1990 г.). Данная методика проста в применении, однако следует отметить, что при хронических ранах стоп у больных СД в силу одновременного воздействия дополнительных факторов (например, плантарная локализация, длительность существования язвенного дефекта, состояние магистрального кровотока, степень инфицированности раны) нет возможности рассчитать скорость заживления по заявленной формуле, кроме того нельзя оценить прогноз: заживет ли рана в результате консервативного лечения или понадобится хирургическое вмешательство.The problem of predicting the healing of wounds, both acute and chronic, is the subject of close study of specialists. Кроме того, основной целью использования индексов была разработка метода, помогающего выбрать оптимальную модель ушивания послеоперационной раны. Определение его содержания в основном используется в онкологии с целью оценки активности опухолевой клетки. Нами было показано, что содержание Ki67 в грануляционной ткани раневого дефекта 12% и более является прогностически благоприятным признаком и означает, что рана может зажить в результате проведенных консервативных мероприятий, тогда как низкое его содержание указывает на необходимость хирургического вмешательства. В рамках дообследования взята биопсия раны с определением Ki67 в ране. Его содержание в грануляционной ткани составило 15%. As part of an additional examination, a biopsy of the wound was taken with the determination of Ki67 in the wound. Its content in the granulation tissue was 15%. The wound healed as a result of ongoing conservative measures in 126 days.

Пациент Л., 1960 г.р. страдает Сахарным диабетом 1 типа в течение 20 лет. Обратился в отделение диабетической стопы с жалобами на наличие язвенного дефекта правой стопы в течение нескольких месяцев. После дообследования диагностирован синдром диабетической стопы, нейропатическая форма. Язвенный дефект II степени по Вагнеру правой стопы. Было назначено лечение в соответствии с международными рекомендациями оказания помощи данной категории больных: разгрузка пораженной конечности; местное лечение с регулярным удалением окружающего гиперкератоза и некротических масс, применением современных атравматичных перевязочных средств. Через 6 месяцев от начала терапии состояние раны оставалось без изменений. При исследовании уровня содержания Ki67 в ране было получено его содержание 3%. Пациент был направлен на консультацию к ортопеду. Выполнена остеотомия 5 плюсневой кости (Фиг. 1). Язвенный дефект эпителизирован в течении 68 дней.Patient L., born in 1960 has been suffering from type 1 diabetes for 20 years.
