Одним из основных недостатков их применения является высокая травматичность мочеиспускательного канала при их введении, особенно при наличии сужений канала. Сущность способа состоит во введении в просвет катетера Фолея, вставленного в мочеиспускательный канал, уретрального катетера и ригидного утерескопа. При визуальном обнаружении, при помощи уретерескопа, сужения уретры, при помощи надувного баллона катерера Фолея сужение расширяется и уретральный катетер под эндоскопическим контролем направляется по естественному расширенному, с помощью баллона катетера Фолея, ходу уретры, исключая установку уретрального катетера по противоестественному ходу. Также после установки уретрального катетера по естественному ходу, возможно установить в мочевой пузырь и катетер Фолея, перемещая его по катетеру как по направляющей. Возможно срезание тупого конца катетера Фолея для лучшего прохождения уретрального катетера и уретероскопа, то есть катетер Фолея будет представлять собой трубку без заглушек с обеих сторон.The essence of the method consists in the introduction into the lumen of a Foley catheter inserted into the urethra, a urethral catheter and a rigid uterescope. Пример. Больной К, 63 лет. Жалобы при поступлении на резкое затруднение при мочеиспускании. Считает себя больным в течение 2 недель, когда мочеиспускание стало резко затрудненным, струя мочи - диаметром около 1 мм. В анамнезе 1 год назад выполнена трансуретральная резекция предстательной железы по поводу доброкачественной гиперплазии предстательной железы. Пациенту проведено клинико-лабораторное обследование. Выявлено 450 мл остаточной мочи, объем предстательной железы - 16 мл. При попытке установки уретрального катетера Фолея - безуспешно, также не проведен катетер Нелатона и металлический катетер. При попытке бужирования уретры встречено непреодолимое препятствие в мембранозном отделе уретры. Выполнена уретроскопия ригидным уретероскопом - выявлена стриктура в мембранозном отделе уретры протяженностью 3-4 мм. Установлен мочеточниковый катетер №4. Далее выполнена внутренняя оптическая уретротомия и резектоскоп проведен в мочевой пузыря. При эндоскопической ревизии - без особенностей. Резектоскоп извлечен, при попытке установки уретрального катетера Фолея - встречает препятствие. Далее принято решение установки катетера Фолея под видеоэндоскопическим контролем. Ригидный уретероскоп введен во внутренний просвет уретрального катетера Фолея на 5 мм дальше проекции манжетки, после чего тупой конец уретрального катетера Фолея срезан при помощи ножниц и ригидный уретероскоп продвинут еще на 3 мм. Это позволила беспрепятственно провести уретральный катетер Фолея №20 по мочеиспускательному каналу. На 10 сутки уретральный катетер удален и у пациента восстановлено естественное мочеиспускание.Example. Patient K, 63 years old. Complaints upon admission to a sharp difficulty in urinating. The rigid ureteroscope is inserted into the inner lumen of the Foley urethral catheter 5 mm further than the projection of the cuff, after which the blunt end of the Foley urethral catheter is cut with scissors and the rigid ureteroscope is advanced 3 mm. This made it possible to pass the Foley urethral catheter # 20 through the urethra without hindrance. On the 10th day, the urethral catheter was removed and the patient's natural urination was restored.
Каждые 3 месяца больной обследовался - выполнялось ультразвуковое исследование остаточной мочи. В течение 4 лет рецидива не выявлено, жалоб нет.Every 3 months the patient was examined - an ultrasound examination of residual urine was performed.
