Description

Ureter plugging device
Technical Field
The invention relates to the technical field of medical instruments, in particular to a ureter plugging device.
Background
The upper urinary tract (including kidneys and ureters) urothelial cancer mainly includes renal pelvis cancer and ureter cancer, and the upper urothelial cancer is a common disease of urinary surgery, and the incidence rate thereof is continuously rising in recent years, and the surgical treatment methods for the upper urothelial cancer are gradually diversified. Due to the anatomical structure and functional specificity of the urinary system, tumor control and reduction of post-operative tumor cell implantation in the surgical treatment of upper urothelial cancer has been one of the challenges facing urologists. The upper urothelial cancer is easy to relapse after operation and is planted in the abdomen and the pelvis, multiple operations, chemical drug treatment or radiation treatment are often required, and various complex complications frequently appear in the course of treatment often bring trouble to clinicians and patients. At present, the known surgical treatment method for the upper urinary tract epithelial cancer mainly comprises the following steps: radical resections of renal pelvis cancer and radical resections of ureteral cancer (the resection range is the whole section of the kidney and ureter on the diseased side), both open surgical routes and laparoscopic assisted surgical routes may be used. However, in the operation process, the treatment methods of the tumor distal ureter are various, and at present, no unified operation method and technical specification exist, and the treatment methods mainly comprise: transurethral ureter distal cuffed electrotomy (including end jaw occlusion and no occlusion treatment), open surgery or laparoscopic distal ureter and surrounding bladder wall excision. Different methods for treating the tumor distal ureter operation have advantages and disadvantages, but no unified standard operation mode is available at present. The risk of tumor cell abdomen pelvic cavity planting can be reduced by the end clamp plugging after the sleeve-shaped electrotomy of the tail end of the transurethral ureter, but the operation space is limited by the limitation of transurethral operation, the cost of the clamp plugging equipment is high, and the operation learning curve is long; the risk of tumor cell abdomen pelvic cavity planting is greatly increased by the patients with the tail end not blocked after the sleeve-shaped electrotomy at the tail end of the urethra and the ureter; due to the limitation of pelvic cavity operation space, the risk of peripheral large blood vessels and important nerve injury exists in the excision process of the distal ureter and the peripheral bladder wall under an open operation or a laparoscope, the bladder wall is often closed by using a non-absorbable biological clamp, a metal suture nail or a suture line during the operation, and the probability of secondary bladder calculus and infection after the operation is high. In resecting the distal ureter, well-known surgical principles are: firstly, controlling the ureter at the far end of the tumor to reduce the planting of tumor cells along with urine; the kidneys and free whole ureter were resected. Some patients have local tumor recurrence or tumor implantation in abdomen and pelvis or great vessel and important nerve injury after radical excision operation of renal pelvis cancer or ureter cancer, and secondary bladder stones and infections after operation. The main reasons for this analysis may be: (1) The ureter end is not sealed in advance in the operation, and tumor cells are removed from the ureter end along with urine or exudates after tumor extrusion in the operation, so that tumor implantation is caused; (2) The tissue structures of the tail end of the ureter, the blood vessels around the bladder, nerves, lymph nodes, lymphatic vessels and the like are complex, the pelvic space is narrow, the tissue structures are easy to damage in the operation process of pelvic surgery, and the surgery risk of a patient is increased; (3) If the non-absorbable biological clamp, the metal suture nail or the suture is used for closing the bladder wall in the operation, the biological clamp, the suture nail or the suture nail is always exposed in the bladder cavity in the healing process of the bladder wall of the patient after the operation, so that infection or vesical calculus is induced, and part of the patient needs to be continuously treated for a long time, thereby greatly increasing psychological and economic burden of the patient. So far, no very effective ureter blocking device or equipment for blocking a ureter at a tumor distal end is reported in radical resection operation of renal pelvis cancer or ureter cancer, and therefore, a ureter blocking device needs to be designed.
