Эта цель достигается тем, что полученная на компьютерном томографе Sirona Galilleos номинальной мощностью 85 кВ компьютерная томограмма (КТ) анализируется рентгенологом и хирургом стоматологом в зоне отсутствующего зуба, на сагиттальном срезе КТ производится замер оптической плотности от края альвеолярного отростка до анатомического образования, у которого будет располагаться апикальная часть индивидуального дентального имплантата. После чего определяется длина и диаметр индивидуального дентального имплантата. Данные оптической плотности заносятся в таблицу соотношения оптической плотности (Таблица 1) с геометрией поверхности дентального имплантата. Информация о геометрии, поверхности, диаметре и длине индивидуального дентального имплантата передается в производственный отдел технопарка СамГМУ где изготавливают индивидуальный дентальный имплантат. Способ дает возможность выбора оптимальной геометрии профиля резьбовой поверхности индивидуального дентального имплантата в зависимости от оптической плотности костной ткани. Профили иллюстрируются графическим материалами. Установлено, что профиль резьбы (В) создает минимальную нагрузку на костную ткань самой высокой плотности D1, способствует улучшению остеоинтеграции и снижает риск возникновения постоперационных осложнений. Для костной ткани наименьшей плотности D4 оптимален профиль резьбы (Н), который создает наибольшее напряжение в костной ткани и способствует лучшей интеграции индивидуальных дентальных имплантатов в мягкой кости, что снижает риск возникновения осложнений и дает возможность давать непосредственную нагрузку на индивидуальный дентальный имплантат сразу после его установки. Преимущество предложенного способа заключается в более точной оценке состояния костной ткани, расположения внутрисуставных элементов, в возможности выбора оптимальной геометрии профиля резьбы индивидуальных дентальных имплантатов в соответствии с плотностью воспринимающего ложа в костной ткани в месте установки имплантата. Выбранные имплантаты обеспечивают равномерное и не превышающее допустимых нагрузок на костную ткань распределение давления от имплантата, создается возможность непосредственно после установки индивидуального дентального имплантата давать на него нагрузку и устанавливать ортопедическую конструкцию. Больной П., 27 лет обратился на кафедру стоматологии ИПО СамГМУ с жалобами на боль в области нижней челюсти с двух сторон и отсутствие центральных зубов на верхней челюсти. При осмотре полости рта: зубы 1.1,4.5, 4.6, 3.6, 3.7 разрушены. Зубы 2.1,2.2, отсутствуют. Предварительный диагноз: Частичное отсутствие зубов, периодонтит зубов 1.1,4.5, 4.6, 3.6, 3.7 Снижение межальвеолярной высоты на 2-3 мм. Больной был направлен на компьютерную томографию верхней и нижней челюсти. По результатам анализа компьютерных томограмм в сагиттальной плоскости были получены данные о том, что плотность костной ткани в области зубов 4.5, 4.6, 3.6, 3.7 соответствует показанием оптической плотности 1000-1500 единиц, показатели оптической плотности в области зуба 1.1 соответствовали 1600-2000. Indicators of optical density were listed in table 1.
В соответствии с таблицей 1 определено, что для зубов 4.5, 4.6, 3.6, 3.7 оптимальными профилями индивидуальных дентальных имплантатов будут профили резьбы D, Н, или С, а для зуба 1.1 оптимальным профилем резьбы дентального имплантата будет профиль В. После анализа компьютерной томографии и получения данных о конституции, геометрии, будущей резьбовой поверхности индивидуальных дентальных имплантатов данные о пропорциях и размерах индивидуальных дентальных имплантатов передали в производственный технопарк СамГМУ где были изготовлены индивидуальные дентальные имплантаты, которые в дальнейшем прислали на кафедру ИПО СамГМУ где их подвергли стерилизации и установили больному П.. Через 2 месяца после оперативного вмешательства больному была произведена компьютерная томография, на которой отмечается полная остеоинтеграция всех дентальных имплантатов и увеличение плотности костной ткани на границе индивидуальных дентальных имплантатов и челюстной кости. Применение индивидуальных дентальных имплантатов, форма резьбы на которых оптимально соответствует плотности костного ложа, обеспечивает лучшую остеоинтеграцию, сокращает сроки лечения и снижает количество возможных осложнений.
