Герминативный состав 13 мужчин (60%), 9 женщин (40%). Тяжесть панкреатита определяли по шкале Ронсон (1972), вариации показателя от 1.8±0,5 баллов у пациентов с острым отечным панкреатитом, до 6,9±1,2 баллов с инфицированным деструктивным панкреатитом. Определение количества десквамированных эндотелиальных клеток (ДЭК) производили дважды: первый раз на 1-2 сутки поступления в хирургический стационар и второй раз на 7-9 сутки. Исследование осуществляли в 5 полях зрения размером 100 мм2, проводили окрашивание 1% раствором метиленового синего объемом 0,2 мл, определяли количество ДЭК, а по визуальной шкале определяли их степень окраски. Сравнение ДЭК проводили с физиологической нормой. В норме у соматически здоровых людей молодого и среднего возраста количество ДЭК составляет 1-10 нормохромных ДЭК в поле зрения. У пациентов группы А (острый отечный панкреатит при первом исследовании (2-3 сутки) ДЭК возрастает до 80 в поле зрения, из них 60% нормохромные, 40% гиперхромные. Для оптимальной оценки функционального состояния ДЭК определяли коэффициент окраски (КО), представляющий собой отношение нормохромно окрашенных эндотелиоцитов к гиперхромно окрашенным. Значение КО при первом исследовании составило 1,5. При втором измерении (на 7-9 сутки) на фоне интенсивной консервативной терапии количество ДЭК снижается до 45. Количество гиперхромно окрашенных ДЭК возрастает до 85-88%. КО - 0,2. У пациентов группы В, с дальнейшей трансформацией ОП в некротический инфицированный процесс, количество ДЭК при 1 измерении увеличивается до 200, из них гиперхромные составляют 57%, нормохромные 43%, КО - 0,8, при 2 измерении в группе В, отмечено снижение ДЭК до 50 из которых 56% нормохромные, 44% гиперхромные. У пациентов подгруппы В с трансформацией в инфицированный деструктивный панкреатит, требующий инвазивных методов лечения, также отмечено снижение ДЭК с 19,5 до 5 раз, на 74,4%, однако степень тяжести ОП снижается на 15,9%, КО увеличивается с 0,8 до 1.2, на 50%. Анализируя результаты, сделан вывод о том, что степень тяжести ОП определяется не только степенью увеличения ДЭК, но и функциональным состоянием эндотелиоцитов. По способности ДЭК концентрировать краситель можно судить о продолжении системной воспалительной реакции.
