Abstract
Description
The invention relates to medicine, namely to surgery and can be used to treat trophic ulcers of the lower extremities in diabetic foot syndrome.
1998.).Diabetes mellitus is recognized worldwide as one of the most significant noncommunicable diseases. According to WHO experts, the number of patients in 2010 amounted to 239.4 million people, and by 2025 it will increase to 380 million people. Among patients, more than 70% develop SDS, and in 40-85% of cases it is accompanied by foot ulcers of varying severity, and is their most common manifestation. The duration of treatment of such patients is from 4 weeks to 2-3 months and often stretches over years, in addition, in this category of 60-80% there is a need for surgical interventions. After limb amputation, two years later, 30% to 50% of patients die, and the life expectancy of patients decreases by 12.5%. (Bakharev I.V. 2003; Galstyan G.R., 1998; Pokrovsky A.V. et al., 1998.).
Currently, in the treatment of patients with trophic ulcers, drugs containing cellular growth factors, cytokines, biotechnological materials are used. However, despite the large number of proposed options, the results of local treatment of trophic ulcers in diabetic foot syndrome remain unsatisfactory. Therefore, the problem of treating patients with diabetic foot syndrome needs research, the search for an effective solution.
A known treatment method proposed by Lipova E.V., Pokrovsky K.A., Gryazevoy N.V. "A method of treating erosive and ulcerative skin lesions", which consists in collecting blood from a peripheral vein, mixing it with an anticoagulant, centrifuging for 5 minutes at a centrifuge speed of 800 rpm. The extracted top layer of platelet-rich autologous plasma in a separate tube activates calcium gluconate at a rate of 50 μl per 1 ml of plasma. Ready-to-use plasma is used for 5-10 minutes, while at least a third of the plasma obtained in liquid form is collected in 1 ml insulin syringes and an ulcer-erosive defect is punctured around the periphery intradermally at the rate of 0.1-0.5 ml for each injection. The remaining plasma is placed in a glass Petri dish and placed in a thermostat for 20 minutes at a temperature of 37 degrees until a transparent thin film of pale yellow is formed, which is applied to the ulcer defect in the form of an application, covered with a dry cloth and bandaged for 24 hours. The procedure is carried out 1 time per week until complete healing.
The disadvantage of this method is its lack of effectiveness due to the short time of centrifugation, which can not always lead to the destruction of platelets and the release of growth factors. In addition, injections are not the best way for local administration of platelet concentrate (TK), since there is a high probability of additional infection. And the procedure 1 time per week does not provide the necessary concentration of growth factors at the site of application.
Autologus Platelet Rich Plasma in Chronic Venous Ulcers: Study of 17 Cases // J. Cutan Aesthet Surg. A known method of treating patients with erosive and ulcerative skin lesions: Autologus Platelet Rich Plasma in Chronic Venous Ulcers: Study of 17 Cases // J. Cutan Aesthet Surg. 2013; 6 (2), taken as a prototype, in which increasing the effectiveness of treatment is achieved through the use of growth factors in the complex of accompanying therapy. The method consists in collecting 20 ml of blood from a patient’s peripheral vein into tubes containing citric acid, sodium citrate and dextrose in a ratio of 9: 1 (blood: anticoagulant). Centrifugation is carried out at a speed of 5000 rpm to separate red blood cells from plasma with platelets. The plasma with platelets is centrifuged again at a speed of 2000 rpm for 5-10 minutes. Then take the lower layer of the obtained plasma (approximately 1.5 ml) and add 10% calcium chloride in the ratio of 0.3 ml per 1 ml of plasma. A pre-sanitized wound is coated with an autologous platelet-rich plasma. Dressing is done 1 time per week until complete epithelization.
The disadvantages of this method is its lack of effectiveness due to the addition of a large number of "extraneous" components and an insufficient number of growth factors in the plasma applied to the wound (since the high speed and duration of centrifugation do not lead to the release of growth factors, but to their destruction due to chemical and mechanical impact).
The objective of the claimed invention is to increase the efficiency of the method.
The problem is solved in that according to the method of treating trophic ulcers in diabetic foot syndrome, comprising collecting a patient’s blood from a peripheral vein in a volume of 15-20 ml, centrifuging it, and then applying the resulting autoplasmic platelet concentrate to a previously sanitized wound and covering it with an aseptic dressing, centrifugation is carried out in three stages, the total duration of which does not exceed 30 minutes, and in the first stage, centrifugation is carried out for 5 minutes per 1000 rpm inut, in the second stage, the upper platelet-poor plasma layer is removed and Ringer's solution is added to the remaining plasma in a 2: 1 ratio and centrifuged for 3 minutes at 2700 rpm; in the third stage, the upper plasma layer and platelet-rich plasma with volume up to 2-3 ml. add short-acting insulin "Biosulin R" at the rate of 1.0-1.5 units per 1 ml. plasma and centrifuged for 5 minutes at 1000 rpm, the resulting autoplasmic platelet concentrate is applied to the wound no later than 10 minutes after receiving it, the procedure is repeated 5-7 times at intervals of 3 days.
The claimed method has high efficiency. Its use can significantly reduce the size of ulcerative defects, and in most cases - achieve complete epithelialization. The biologically active drug obtained by the implementation of the claimed method includes growth factors (contained in platelets: PDGF-AB (platelet growth factor AB), TGFβ-1 (transforming growth factor β-1) and VEGF (vascular endothelial growth factor), capable of activating processes of epithelization and angiogenesis, and insulin-like factors (IGF 1 and IGF 2, which have an affinity of 64%) contained in insulin), which have the ability to control cell proliferation, differentiation and directional movement.
The method is as follows. Blood from patients is drawn from the cubital vein into three sterile tubes, with the addition of an anticoagulant, with a total volume of 15.0-20.0 ml. Next, triple centrifugation of blood is performed on a centrifuge TsLMN P10-01 at a time as close as possible to the time of blood sampling. The time taken to prepare platelet concentrate should not exceed 30 minutes. At the first stage, centrifugation is performed in the “soft” mode (1 revolution per minute for 5 minutes), in order to separate the blood into three layers: red blood cells, acellular plasma (platelet-poor plasma-upper layer) and the middle layer, in which they are concentrated platelets, the so-called "buffy-coat" (BC-layer). At the second stage, in order to obtain an intermediate product, the entire acellular plasma, the BC layer and Ringer's solution are transferred to the tube in a ratio of 2: 1 (per 1 ml of plasma, 0.5 ml of solution) and centrifuged in a “hard” mode (2700 rpm , within 3 minutes). Ringer's solution is introduced to facilitate physico-chemical fractionation of blood plasma during further centrifugation. At the third stage, to obtain the final complex product, a small amount (2-3 ml.) Of acellular plasma and the BC layer are transferred to the test tube, short-acting insulin “Biosulin R” is added at the rate of 1.0-1.5 units per 1 ml. plasma and again centrifuged in the "soft" mode. Consecutive triple
центрифугированная, с поэтапным отделением промежуточного продукта позволяет получить препарат с уровнем тромбоцитов в 7,5±0,5 раз больше, чем в плазме крови.centrifuged, with a gradual separation of the intermediate product allows you to get a drug with a platelet level of 7.5 ± 0.5 times more than in blood plasma.
The obtained volume of TC is applied to the ulcer surface under dressing conditions in compliance with the principles of asepsis and antiseptics. Before applying the final product, pretreatment with an isotonic sodium chloride solution and removal of necrotic tissue is performed. Dressings are performed after 3 days (due to the duration of the effects of growth factors for several days). The total duration of treatment is up to 5-7 dressings.
