Ривароксабан (Ксарелто) отдельно не используют как препарат для лечения осложнений синдрома диабетической стопы. Пациент А., 63 лет поступил с жалобами на боли, отек, гиперемию правой стопы, наличие обширной гнойно-некротической раны на подошвенной поверхности стопы. Больной страдает инсулиннезависимым сахарным диабетом второго типа в течение 6 лет. Принимал таблетированные сахароснижающие препараты Манинил 3,5 мг два раза в сутки. Длительность от начала проявления гнойно-некротических осложнений диабетической стопы до поступления в стационар - 18 суток. На вторые сутки стационарного лечения выполнена ампутация 3-4-5 пальцев правой стопы с резекцией головок плюсневых костей. На первом этапе, длительностью 17 суток, назначен Тромбовазим по 800 мг два раза в сутки перорально, Вессел ДУЭ Ф (Сулодексид) по 600 ЛЕ внутривенно капельно на 200 мл 0,9% раствора натрия хлорида ежедневно, Вазапростан по 60 мкг один раз в сутки внутривенно капельно на 200 мл 0,9% раствора натрия хлорида. Для местного лечения раны стопы применили портативный аппарат вакуумной аспирации VIT Medical. Для чего на рану стопы наложили гипоаллергенную губку Vivano Med Foam Hartmann, поверх губки наклеили стерильную пленку Hydrofilm Hartmann. Поверх пленки приклеили порт VivanoTec Port Hartmann, который через дренажную пленку соединили с портативным аппаратом вакуумной аспирации. Создавали отрицательное давление в ране стопы на уровне 100 мм рт.ст., которое поддерживали круглосуточно. В начале лечения отмечалось обильное фибринозно-гнойное отделяемое. As a result of the treatment, a sharp decrease in edema and pain in the lower extremity was noted by the 6th day.
При переходе на второй этап комплексного лечения по характеру и количеству удаляемого раневого экссудата и субъективных симптомов оценивали динамику изменения в ране. Отмечалось прекращение выделения фибринозно-гнойного экссудата к 7-м суткам от начала лечения. Признаки перехода раны в фазу регенерации. Продолжали активную вакуум-аспирацию аппаратом VIT Medical путем изменения отрицательного давления в ране от 100 до 20 мм рт.ст. и обратно в течение суток, сроком 8 суток. Снижение давления производилось автоматически при помощи встроенного программного обеспечения, на 20 мм рт.ст. за каждый час до уровня 20 мм рт.ст. Аналогично проводился и подъем уровня отрицательного давления до уровня 100 мм рт.ст. В дополнение к этому, Вессел ДУЭ Ф (Сулодексид) назначали перорально по 1 капсуле (250 ЛЕ) два раза в сутки, а также продолжали прием Тромбовазима по указанной выше схеме.During the transition to the second stage of complex treatment, the dynamics of changes in the wound was assessed by the nature and amount of the removed wound exudate and subjective symptoms. При переходе на третий этап оценивали характер раневого отделяемого. К 14-м суткам экссудация из раны прекратилась, отмечался переход раны в фазу эпителизации. После снятия портативного аппарата VIT Medical проводили лечение раны стопы при помощи гистоэквивалент-биопластического материала «Джи-Дерм». Увлажнение повязок производили два раза в сутки при помощи 0,9% раствора натрия хлорида. Смену повязок проводили один раз в 3 суток. На третьем этапе продолжили прием препарата Тромбовазим по указанной выше схеме, общим курсом 20 суток. А также продолжен пероральный прием Вессел ДУЭ Ф по указанной выше схеме, общим курсом 8 недель. После окончания приема Тромбовазима был назначен Ривароксабан (Ксарелто) 2,5 мг два раза в сутки сроком на 12 недель.During the transition to the third stage, the nature of the wound discharge was assessed.
