Abstract
Description
The invention relates to medicine, in particular to otorhinolaryngology, therapy, reconstructive medicine, plastic surgery, reconstructive surgery and can be used to treat inflammatory diseases and restore mucous membranes.
The group of patients with inflammatory diseases of the skin and soft tissues, nasopharyngeal mucosa is numerous in the practice of general practitioners, dermatovenerologists, otorhinolaryngologists, and restorative medicine doctors. The treatment of these diseases remains relevant due to the lack of effective therapy; accordingly, the patient’s functional ability can significantly decrease, up to the chronic process, loss of sensitivity, smell, thereby significantly reducing the patient’s quality of life.
Inflammatory diseases of the paranasal sinuses occupy one of the leading places in the structure of diseases of the upper respiratory tract, purulent sinusitis affects from 10 to 30% of the urban and rural population. In some cases, there is an association of this pathology with inflammatory lung diseases.
Currently, despite the improvement of methods for the diagnosis and treatment of sinusitis, their prevalence is not decreasing. Moreover, the inflammatory pathology of the paranasal sinuses maintains a growth trend, and there is an increase in recurrent and chronic forms of the disease, worsening the quality of life of patients.
The traditional treatment of inflammatory diseases is aimed at the destruction of pathogens and includes the evacuation of pus from the cavities, local and general antibiotic therapy, antihistamines and vasoconstrictors. In the treatment of acute and chronic inflammation of the mucous membrane of the paranasal sinuses, the greatest therapeutic effect is achieved with local administration of drugs.
However, in recent years, there has been increasing evidence of the ineffectiveness of antibiotic therapy due to the emergence of antibiotic-resistant strains of microorganisms. It is also known that most antibiotics, when applied topically at therapeutic concentrations, inhibit the activity of the ciliated epithelium of the mucous membrane, thereby complicating the natural cleansing of cavities. Moreover, antibiotics contribute to the destruction of the necessary saprophytic flora, and as a result, the development of dysbiosis. Many of the commonly used antibiotics also have an immunosuppressive effect.
The specific protective mechanisms of the mucous membranes, forming a barrier that protects the macroorganism from the pathogenic effects of various pathogenic and conditionally pathogenic microflora, are called the immune system of the mucous membranes, including immunocompetent and auxiliary cells, immunoglobulins of various classes, including secretory, cytokines (Belyakov I.M. Mucosal Immune System Immunology 1997. 4: 7-13 Bykova VP Lymphoepithelial organs in the system of local immunity of the mucous membranes Arch. Patol. 1995. 1: 11-16. Kozlov .K. Modern immunotherapy with infectious pathology. Roncoleukin Previous clinical use of the drug. // Manual for Doctors. Publ .: St. Petersburg State University, 2001.24 s).
An interesting direction for the treatment and stimulation of the mucosal immune system is autologous plasma therapy.
Autologous plasma is promising in the treatment of inflammatory diseases, due to growth factors contained in platelets.
Growth factors are polypeptides with a molecular weight of 5-50 kDa, which have a wide range of biological effects. Growth factors, as a rule, are produced by non-specialized cells, including platelets. These include transforming growth factor (TGF), platelet growth factor (PDGF), vasculoendothelial growth factor (VEGF). TGF inhibits the synthesis of inflammatory cytokines, enhances the synthesis of proteins of the intercellular matrix, promotes wound healing, has anabolic effects, stimulates fibroblasts, the development of endothelium, is involved in the regulation of cell proliferation. PDGF is involved in the regulation of acute inflammation and wound healing.
For the treatment of inflammatory diseases of the nasopharynx and skin, the most convenient form of autologous plasma lyophilisate.
Known technologies for the preparation of autologous plasma lyophilisate used in traumatology and orthopedics.
Известна заявка JP 2011120763 «MATERIAL FOR REGENERATION TREATMENT» 23.06.2011; A61L 15/16; 2009281382. Known application JP 2011120763 "MATERIAL FOR REGENERATION TREATMENT" 06/23/2011; A61L 15/16; 2009281382. This methodology is aimed at obtaining materials for tissue regeneration using autologous plasma and not requiring preparation before use or cryopreservation. The treatment regeneration material contains plasma, frozen and dried. The material is applied to the hard surfaces of instruments intended for osteosynthesis, used after storage in the refrigerator for at least one day, freezing and drying. Material for regeneration can be used after autologous plasma has been stored in the refrigerator for at least thirty days, after freezing and drying.
The disadvantages of this method: the complexity of the process, there is no information about the degree of preservation of growth factors after freezing and prolonged drying of platelet autologous plasma.
Another well-known method is also used in traumatology and orthopedics (patent No. 2506946, dated 11.28.2014), it is also characterized by a large amount of blood taken, freezing, drying and mandatory sterilization before use.
The inventive method is devoid of these disadvantages, characterized by a short and affordable material and technical stage.
The technical result of the claimed method is to improve mucosal trophism, restore the epithelial layer, improve the condition of the nasopharyngeal mucosa, reduce secretions, improve microcirculation and increase tissue oxygenation due to growth factors and biologically active substances contained in platelet-type autologous plasma.
The method is as follows. Intranasal administration of platelet autoplasma is carried out 1 time in 7 days, for a course of 2-3 procedures.
Venous blood was taken from a patient in a volume of 9 ml, by a vacuum blood sampling system, through a 21G catheter. Then the blood is centrifuged with an acceleration equal to 1000G for 5 minutes. After centrifugation from a test tube with a syringe of 5.0 ml, 3.5-4.0 ml of platelet autologous plasma is taken.
The obtained autologous plasma is applied in a thin layer on vinyl material at the rate of 2 ml per 100 cm 2 , dried for 7 minutes at a temperature of 37 degrees, then the plasma in the form of a powder is collected manually by rolling movements into a sterile tray. This procedure is carried out twice to obtain the desired volume of powder.
The powder is collected in a sterile tray and introduced onto the affected surface: intranasally or application to the wound surface. The procedure is aimed at removing the vicious cycle of chronic inflammation, restoring microcirculation and tissue oxygenation, thus improving the sense of smell, restoring breathing volume, reducing pathological secretions, in case of intranasal administration. In the case of application to the wound surface, an acceleration of physiological regeneration and an antibacterial effect are observed.
At the end of the course, patients note restoration of breathing, restoration of the nasal mucosa, restoration of smell, reduction of secretions. This course is recommended to be repeated 3-4 times a year in patients with a tendency to chronic inflammatory diseases of the nasopharynx, in patients with atrophy of the mucous membranes.
The practical applicability of the method and the features of its use are illustrated by the following example:
