Abstract
Description
The invention relates to medicine, namely to a gastroenterologist and can be used for the effectiveness of the treatment of reflux disease.
– 221 с.]. Currently, among the diseases of the digestive tract are increasingly common duodenogastric, gastroesophageal, as well as duodenogastraesophageal refluxes. Their main cause is the insufficiency of the switching function of the pylorus, chronic duodenitis and increased pressure in the duodenum [Galiev Sh. Z., Amirov NB Duodenogastric reflux as a cause of the development of reflux gastritis // Bulletin of modern clinical medicine. 2015. Vol. 8. No. 2. P. 50-61.]. Duodenogastric reflux (GHD) - throwing the contents of the duodenum (duodenum) into the stomach, where the pressure is lower than in the duodenum. DGR leads to the defeat of the gastric mucosa with bile acids, their salts, pancreatic enzymes and other components of the duodenal contents [Frolkis AV Functional diseases of the gastrointestinal tract. - L .: Medicine, 1991. - 221 p.].
In advanced cases, refluxes lead not only to erosive-ulcerative lesions of the gastric mucosa, but also to the esophageal mucosa, which is fraught with the development of precancerous esophageal diseases at the beginning, in particular Barrett's esophagus, and then malignant esophageal formations.
– 175 с.]. With the use of endoscopic technology, it became possible to easily diagnose various types of refluxes — throwing the contents of secreted physiological juices from the distal parts of the digestive tract into its proximal parts [Babak O.Ya., Fadeenko GD. Gastroesophageal reflux disease. - Kiev .: Interpharma, 2000. - 175 p.].
– 2002. – № 2. – С. 17–20]. According to literary sources, bile (alkaline) refluxes are considered the most unfavorable factors. The contents of bile from the duodenum is thrown into the stomach, and then into the esophagus. The mucous membrane of the stomach is not adapted to the bile, bile acids, as well as to the secret of the pancreas. The action of the contents of the duodenum especially marked changes in the mucous membrane of the esophagus [Belova EV, Vakhrushev Ya.M. Characteristics of aggressive protective factors in the erosive lesion of the mucous membrane of the gastroduodenal zone // Therapist. arch. - 2002. - № 2. - p. 17–20].
One of the protective (adaptive) factors of the mucous membrane of the stomach and esophagus with such pathophysiological phenomena are hyperplastic states. However, as the compensatory capacity in the mucous membrane is exhausted, first inflammatory events in the form of catarrhal changes occur, then structural damage to the mucous membrane occurs, as a result of which erosive-ulcerative defects occur. In the future, cicatricial deformities of the lower esophagus may occur, and then strictures, and later - malignant tumors, mainly with damage to the lower esophagus.
В.Т. – 1046 с.].Currently, various traditional pharmacological methods of treatment of duodenogastroesophageal reflux are used [Rational pharmacotherapy of diseases of the digestive organs: A guide for practicing physicians / V.T. Ivashkin, T.L. Lapin et al. / Under total. Ed. V.T. Ivashkina. - M .: Litter, 2003. - 1046 p.].
1) Применяет ингибиторы протонной помпы (ИПП) (Омез, Омепрозол, Нольпаза и т.п.);1) Applies proton pump inhibitors (IPP) (Omez, Omeprozole, Nolpaz, etc.);
2) Prescribe antocidal drugs:
д. a) Absorbable: magnesium oxide, calcium carbonate, etc.
б) невсасывающиеся: альмагель, маалокс, фосфолюгель;b) nonabsorbable: Almagel, Maalox, phospholgel;
3) Н-2 блокаторы рецепторов гистамина: Гастросидин, Фамоцидин и т.д.; 3) H-2 histamine receptor blockers: Gastrosidin, Famocidine, etc .;
4) Холеолитические средства: Хенофальк, Урсосан;4) Choleolytic agents: Henofalk, Ursosan;
5) Прокинетики: Церукал, Домперидон, Гонатон;5) Prokinetics: Zeercal, Domperidone, Gonaton;
– 2011. – № 2. – С. 1-10.) However, the known method has the following disadvantages. To achieve the effect, a long period of treatment is needed - more than 5-7 days. In some cases, patients after this kind of treatment with esophagogastroscopy retain bile in the contents of the stomach, therefore, patients still have symptoms of reflux disease (treatment failure). In addition, not all patients tolerate drugs. So, Reglan often causes anxiety, headache. The drug of the proton pump inhibitor group (Omez, Omeprozole) causes osteoporosis, impotence, precancerous diseases - polyps, dysplasia, candidiasis of the stomach, etc. Ursosan cause dyspepsia, is contraindicated in gastric ulcer and duodenal ulcer. Preparations of other groups also have side effects and contraindications (Pasechnikov VD, Pasechnikov DV, Goguyev RK Refractoriness to the therapy of gastroesophageal reflux disease: definition, prevalence, causes, diagnostic algorithm and patient management // Perspekt. gastroenterol., hepatol. - 2011. - № 2. - p. 1-10.)
The technical result is to increase the effectiveness of conservative therapy to eliminate duodenogastroesophageal reflux with the least toxic effects on the body of drugs in general and accelerated removal of symptoms of the syndrome complex.
The essence of the invention lies in the fact that in the method of treatment of duodenogastroesophageal reflux, the patient takes 150 ml of cool water 15-20 minutes before the procedure of endoscopic intervention, then endoscopic single-stage removal of bile contents from the duodenum, stomach and esophagus, and then through the endoscope into the duodenal lumen intestines injected 10 ml of infusion of yarrow.
The method is as follows. The patient for 15-20 minutes before the procedure of endoscopic intervention on an empty stomach takes 150 ml of cool (approximately 10 ° C) water, which allows you to create favorable conditions for the successful evacuation of the contents by reducing its density. Next, carry out endoscopic intervention, which produces simultaneous removal of bile contents from the duodenum, stomach and esophagus. Next, through the endoscope into the lumen of the duodenum through the catheter injected 10 ml of infusion of yarrow.
The infusion of yarrow is pre-prepared according to the following well-known scheme: 15 g of crushed raw materials are poured in one glass of boiling water for 45 minutes, filtered. Infusion has anti-inflammatory, insecticidal, antibacterial and astringent action.
This procedure is repeated after 15 days.
100 %. After applying the claimed method, even after the first procedure, almost all (more than 75%) patients relieve symptoms of reflux disease, and after two procedures - 100%.
In the analysis of 450 treated patients, it was found that in the group of patients who followed a diet and diet with minimal use of drugs, clinical remission of reflux disease after this kind of treatment lasts up to 6 months. in 340 (75.6%) patients, up to 5 months. - in 76 (16.9%) patients, up to 4 months. - in 34 (7.6%) patients.
Compared with the known solution, the claimed method allows to increase the effectiveness of conservative therapy for the elimination of duodenogastroesophageal reflux with the least toxic effects on the body of drugs as a whole and to accelerate the removal of symptoms of the syndrome complex.
