Изобретение относится к ветеринарной хирургии и может быть использовано для удаления межпальцевой фолликулярной кисты у собак.The invention relates to veterinary surgery and can be used to remove interdigital follicular cyst in dogs.
Межпальцевая киста является разновидностью заболеваний, называемых пододерматитами. Киста формируется из сальных желез, локализующихся в толще ткани межпальцевой складки, вследствие закупорки их выводящих протоков с подошвенной стороны волосистой части межпальцевой кожи. Данная киста, вторично вызывает воспалительный процесс и болевую реакцию (dolor), сопутствующую ему. И как следствие индуцирует нарушение локомоторной функции конечности, диагностируемое как хромота опорного типа. Образовавшаяся киста склонна к разрыву, в результате чего формируется дренажный сток с дорсальной стороны пясти/плюсны. По причине развития реактивного воспалительного процесса, на дорсальной поверхности межпальцевого пространства возникает фурункул, что осложняет течение данной патологии. При хроническом и рецидивирующем течении заболевания в межпальцевом пространстве образуются рубцы (см. Бокарев, А.В. Диагностика и лечение собак с хроническими пододерматитами: Диссертация на соискание ученой степени доктора ветеринарных наук / СПбГАВМ., 2014. - С. 60-61.). Специфические консервативные методы лечения данного заболевания отсутствуют, так как имеет место образование кист из фолликулярных мешочков сальных желез с подошвенной стороны кожи. И без удаления кисты вылечить заболевание практически невозможно. Однако, количество мягких тканей в области межпальцевого пространства незначительно, поэтому ушивание хирургической раны в этом месте представляет сложность (см. Duclos D.D., Hargis A.M., Hanley P.W. Pathogenesis of canine interdigital palmar and plantar comedones, and follicular cysts and their response to laser surgery. // Veterinary Dermatology. - 2008. - N 3. - P. 134-141.).Interdigital cyst is a type of disease called Pododermatitis. The cyst is formed from the sebaceous glands, localized in the thickness of the tissue of the interdigital fold, due to the blockage of their excretory ducts on the plantar side of the hairy part of the interdigital skin. This cyst, for the second time, causes the inflammatory process and the painful reaction (dolor) accompanying it. And as a result, it induces a violation of the locomotor function of the limb, diagnosed as a lameness of the supporting type. The resulting cyst is prone to rupture, as a result of which a drainage drain is formed from the dorsal side of the metacarpus / metatarsus. Due to the development of a reactive inflammatory process, a furuncle appears on the dorsal surface of the interdigital space, which complicates the course of this pathology. With chronic and relapsing course of the disease in the interdigital space, scars are formed (see Bokarev, AV Diagnosis and treatment of dogs with chronic Pododermatitis: Thesis for the degree of Doctor of Veterinary Sciences / SPbGAVM., 2014. - P. 60-61.) . Specific conservative methods of treatment of this disease are absent, as there is a formation of cysts from the follicular sacs of the sebaceous glands on the plantar side of the skin. And without removing the cyst, it is almost impossible to cure the disease. Известен способ 1 - производят удаление кисты с захватом здоровых тканей и удалением межпальцевой складки, затем сшивают дорсальные и подошвенные края кожи межпальцевого пространства, отдельно на каждом пальце. При этом пальцы ничем не связаны друг с другом, а межпальцевая кожная складка практически исчезает. Способ 2 - производят удаление кисты с захватом здоровых тканей и удалением межпальцевой складки, затем сшивают дорсальные и подошвенные края кожи вместе и плотно сближают пальцы друг с другом. При этом межпальцевое пространство исчезает (см. Douglas Slatter, Textbook of Small Animal Surgery-3rd. По способу 2 - отсутствие межпальцевого пространства и невозможность расхождения пальцев в момент опоры конечности. Техническим результатом предлагаемого изобретения является сохранение анатомической целостности функциональной способности межпальцевой складки после удаления межпальцевой фолликулярной кисты. Технический результат достигается тем, что после удаления межпальцевой фолликулярной кисты у собак, путем удаления кисты с частью тканей межпальцевой складки, а ушивание раны производят путем наложения восьмиобразных швов на свободные кожные лоскуты межпальцевого пространства, формируя новую межпальцевую складку.The technical result of the invention is the preservation of the anatomical integrity of the functional ability of the interdigital fold after removal of the interdigital follicular cyst. в) После остановки кровотечения, на края хирургической раны накладывают восьмиобразные швы (Фиг. 3), для которых, в зависимости от размера удаленной складки, требуется всего 3-5 узловатых шва, на расстоянии 0,3 см от края раны и 0,5 см друг от друга. Далее производят вкол иглы с дорсальной стороны кожного лоскута одного пальца, затем нить проводят по диагонали межпальцевого пространства, а иглой прокалывают кожный лоскут противоположного пальца, со стороны подкожной клетчатки. После этого иглой прокалывают подошвенную кожную складку с противоположной стороны, и проводят нить далее, по диагонали вверх к кожной складке другого пальца, прокалывая ее со стороны подкожной клетчатки. Таким образом, шовная нить формируется в виде восьмерки и удерживает кожные лоскуты межпальцевого пространства (Фиг. 3).
