Abstract
Description
The invention relates to the field of medicine, namely to ophthalmology, and can be used for intraocular operations, in particular, vitrectomy and phacoemulsification.
The vitreous body is a transparent, gel-like substance that fills the space between the lens and the retina in the eye. It is 99% water, 0.9% salt, and 0.1% high molecular weight components, including soluble protein, collagen fibers, and hyaluronic acid. The combination of large molecular elements with the water part provides the viscoelastic properties of the vitreous body, which in turn provides mechanical protection for surrounding tissues during eye movement, physical activity and provides an elastic response to the applied stress. In the vitreous body, the central (nuclear) part and the peripheral (cortical, preretinal) part, which is in direct contact with the retina, is isolated.
In diabetic proliferative vitreoretinopathy, hemophthalmus and a number of other pathologies, it is necessary to remove the altered vitreous humor, and in the case of the macular aperture, rheumatogenous detachment - and virtually unchanged vitreous.
In connection with the viscoelastic characteristics of the vitreous body, it is necessary to apply a cutting, grinding action in relation to it.
Usually removal of the vitreous body is performed using a vitreotome. Vitreot aspirates the vitreous body in small portions, then produces cutting the fibers into small fragments, which are removed through the aspiration line.
с. 47).Aspiration and portion cutting of the fibers of the vitreous body, inevitably leads to traction effects on nearby tissues, which manifests itself in the form of pulsating vitreoretinal traction with a frequency corresponding to the frequency of cutting of the vitreotomy. Traction effects are undesirable because they can cause damage to the retina and the formation of iatrogenic ruptures. In order to reduce the severity of pulsating traction, the manufacturers of ophthalmic surgical systems follow the path of increasing the working frequency of the vitreotome, which leads to the formation of a multitude of low-amplitude pulses with effects that are much less remote from the area of the vitreotomy working, resulting in pulsating vitreoretinal tractions than is observed at a lower rate. Such low-amplitude flows do not have time to spread in the vitreous body, which reduces the likelihood of iatrogenic damage to the retina (Charles S., Kalsada X., Wood B. Microsurgery of the retina and vitreous body. Moscow, 2012, p. 47).
Н., Ohji М., Akduman L., eds. Vitreoretinal surgery. London: J.P. Medical Ltd.; 2012, 442 p.)To date, the vast majority of manufacturers of ophthalmic equipment use guillotine-type vitreotomes with pneumatic drive. The mechanism of his work is as follows: the vitreous body is aspirated into the lumen of the vitreotoma through its working window, and then the knife, moving along the needle of the vitreotoma, makes a cut, cutting off the aspirated part of the vitreous body. Further, this portion of the vitreous body moves along the aspiration line, and begins a new cycle with a vitreote (Saxena S., Meyer S.N., Ohji M., Akduman L., eds. Vitreoretinal surgery. London: JP Medical Ltd .; 2012, 442 p .)
Преимуществами высокоскоростной витрэктомии являются уменьшение тракционного воздействия на сетчатку, исключение резких изменений скорости аспирационного потока при переходе витреотома из плотной среды в менее плотную, возможность максимального приближения к сетчатке и иссечения преретинальных слоев стекловидного тела, а при необходимости - и клапана ретинального разрыва, что позволяет значительно снизить риск ятрогенных осложнений (Teixeira A., Chong L.P., Matsuoka N., Arana L., Kerns R., Bhadri P., Humayun M. Vitreoretinal traction created by conventional cutters during vitrectomy. Ophthalmology. 2010; 117 (7): 1387-1392.).The main focus of vitrectomy is to reduce surgical trauma and increase the effectiveness and safety of the intervention. For a long time this was achieved by reducing the caliber of vitreotomes and increasing the frequency of cuts. To date, 23G and 25G gauges are considered to be the global standard. Technical solutions have been developed that ensure the frequency of vitretome cuts from 5000 to 7500 cuts / min. Tools of 27G caliber with a cut frequency of 10,000–15,000 c / min are being gradually introduced (Aznabayev BM, Shirshov MV, Mukhamadeev TR, Ramazanov VN, Yamlikhanov AG, Dibaev T.I. New control algorithms vitrectomy system. Cataract and refractive surgery. 2013; 13 (2): 37-40.). This made it possible to make vitrectomy a microinvasive, seamless operation, which is performed on an outpatient basis in many clinics today. The advantages of high-speed vitrectomy are the reduction of traction effects on the retina, the exclusion of abrupt changes in the speed of the aspiration flow during the transition of the vitreotome from a dense to a less dense environment, the possibility of approaching the retina as much as possible and excision of the preretinal layers of the vitreous body and, if necessary, the retinal rupture valve, which allows reduce the risk of iatrogenic complications (Teixeira A., Chong LP, Matsuoka N., Arana L., Kerns R., Bhadri P., Humayun M. Vitreoretinal traction created by conventional cutters during vitrectomy. Ophthalmology. 2010; 117 (7) : 1387-1392.).
Porcine vitreous flow behavior during high-speed vitrectomy up to 7500 cuts per minute. Transl. Vis. Sci. Technol. 2016; 5 (1): 7.). However, with a decrease in the vitreotomy caliber, the problem of its productivity, i.e. removal rate of the vitreous body. In addition, there is a certain technical limit, above which it is not possible to increase the frequency of cuts using traditional means, and therefore manufacturers are trying to achieve this goal in other ways, creating vitreotomes with so-called technology. “Double cut” and double pneumatic drives (push-pull-vitreotoma) (Abulon DJK, Buboltz DC) up to 7500 cuts per minute. Transl. Vis. Sci. Technol. 2016; 5 (1 ): 7.). These methods are quite effective, however, vitreotomes of similar structures are actually inaccessible for widespread use in practice because of their high cost.
2005).One of the options for the destruction of tissue in ophthalmosurgery is the use of ultrasound. For ultrasonic fragmentation of eye tissues, an ultrasonic instrument is used, the typical design of which includes a housing with a waveguide placed in it consisting of a concentrator, to which a hollow ultrasonic needle joins, a multiple of the piezoelectric elements and a coupling in the center of which passes a channel for aspirating fluid from the eye, and between the housing and the waveguide have a space filled with irrigation solution, which enters the eye as a replacement fluid during the operation (Aznabayev BM Ultraz Vukovaya cataract surgery - phacoemulsification, 2005).
There are various versions of the ultrasonic handpieces with the possibility of their use for vitrectomy.
One of them is a working tip of an ultrasound device for intraocular operations, performed in the form of a hollow rod with a threaded shank on one side and a hole in the bottom on the other, characterized in that, in order to increase the efficiency of the surgical intervention, an undercut is formed on the bottom of the working tip with the side surface of the rod cutting edge, while the rod is made tapering from the shank to the bottom (SU 1050702). The disadvantage of this design is that the hole is located on the bottom of the tip and coincides with its longitudinal axis, which makes it almost impossible to visualize the working part during the operation and increases the likelihood of iatrogenic complications. In addition, the destructive action of ultrasound in the tips of this design is carried out on the undercut surface, and therefore is not effective enough to emulsify the pathologically changed tissue of the eye with a fibrous structure.
Also known cannula attached to the distal end of the ultrasonic instrument, having a distal end with at least one port located along the axis or on the side of the cannula associated with the suction line of the instrument, while the cross-section of the port is smaller than the cannula lumen cross section. This cannula can have a different number and geometry of the ports, such as triangular, rectangular, square, oval, octagonal, etc., with the desired port diameter in the range of 130-205 μm (US 20140074013,
The closest analogue of the invention is an ultrasound needle for dissection and aspiration of vitreous tissue, including a cannula, having a port at the end, and a lumen extending from the distal end of the cannula to the proximal one. The cross section of the cannula port is smaller than the cross section of the cannula lumen. A sleeve attached to the proximal end of the cannula is attached to the ultrasound instrument. The needle clearance is formed from the cannula port to the proximal end of the sleeve. The length of the needle lumen is about a quarter of the operating frequency wavelength of the ultrasonic instrument and the length of the cannula, measured from the distal end to the distal end of the sleeve, is sufficient to reach the posterior pole of the eye. The outer diameter of the cannula is 23, 25, 27 gauge (US 20160100982,
The objective of the invention is to increase the efficiency of ultrasonic destruction of tissues, including dense, fibro-altered pathological structures of the eyeball.

According to the present invention, an ultrasound needle for ophthalmosurgery used for intraocular operations, in particular, vitrectomy and phacoemulsification, is considered.
In the general case, an ultrasound needle for ophthalmic surgery, intended for dissection and aspiration of the vitreous body and other eye tissues, contains a sleeve for connection with an ultrasound instrument, a hollow cannula having at least one working opening at the distal end and made with a lumen extending from the proximal end of the cannula to distal.
1.In particular, in FIG. 1 shows an
At the distal end of the cannula, one or two (or more, if required) working through holes are made, essentially communicating the cavity of the cannula with the external environment. The cross section of such a working hole is smaller than the cross section of the cannula lumen (or the area of the working hole is less than the cross section of the cannula along the inner surface of the side wall of this cannula).
A feature of the claimed invention is that in the side wall of the cannula at the distal end additionally made at least one through slit. Slit can have a different width, but, as a rule, does not exceed the diameter of the cannula lumen. At the same time, such a slot-like slot can be obtained by various metalworking methods, for example, by electrochemical shaping, turning, laser processing, etc.
In the first embodiment shown in FIG. 2 and 3, at the
3.According to the second embodiment shown in FIG. 4, at the
89°.In the third exemplary embodiment shown in FIG. 5, at the
6, щелевидная прорезь 9 выполнена по спирали вокруг стенки дистального конца 5 канюли 3 (то есть имеет спиралевидную форму).According to the fourth embodiment shown in FIG. 6, the slit-
The analysis of the presented examples of execution of the slit slot showed the following. The presence of the slit-like slit itself made it possible to increase the efficiency of ultrasonic fragmentation as a whole, and for this process the position and shape of such a slit at the distal end of the cannula have no significant effect. Therefore, all four examples of the slot design are of practical importance. However, from the standpoint of convenience of the operation, the position of the slit-like slot at an angle of 90 ° to the longitudinal axis is optimal, and the slot length is on the circumference of the outer surface of the cannula. Due to this slot-type configuration, an increase in the suction capacity of the ultrasonic needle and the maximum increase in the efficiency of ultrasonic fragmentation are observed. When performing a slit at an angle other than 90 °, the efficiency remains the same, but visual control of the ultrasonic needle is difficult, and with a length of the slot more from the circumference of the outer surface of the cannula, a decrease in the mechanical strength of the ultrasonic needle and the appearance of a tendency to fracture are observed.
The use of the invention is as follows. Include and prepare ultrasonic fragmenter to work. An ultrasonic needle is attached to the working end of the ultrasonic fragmentation tool by means of a threaded connection. An ultrasound needle is inserted through the incision into the eye cavity, and ultrasound is activated, which results in fragmentation of the eye tissues with the formation of an emulsion, which is aspirated using an aspiration pump of the ultrasonic fragmentator. Due to the use of an ultrasonic needle, which has at least one slit-like slot throughout, an increase in the suction capacity of the ultrasonic needle and an increase in the efficiency of ultrasonic fragmentation are observed.
