Синдром фето-фетальной трансфузии (СФФТ) является одним из самых тяжелых осложнений монохориальной беременности, который сопровождается патологическим шунтированием крови через сосудистые анастамозы от одного плода к другому. Выполненные 26 амниотических септотомий механическим путем с последующей амниоредукцией подтвердили эффективность применения данного метода в коррекции СФФТ. В группе исследования смертность из 26 плодов составляла: 10 случаев в утробе матери и 4 случая в течение недели жизни. Частота выживания обоих плодов - 46%. Гестационный возраст родов при септотомий регистрировался в интервале от 20 до 34 недели и пролонгирование беременности после септотомий - от 0,3 до 13,6 недель. Данный способ коррекции СФФТ является ближайшим аналогом заявленного способа. Сущность способа состоит в том, что септотомия механическим путем позволяет провести коррекцию проявлений синдрома фето-фетальной трансфузии многоплодной беременности.In 2003, Adedayo et al. Технический результат заключается в снижении частоты осложнений и смертности плодов за счет того, что заявляемый способ является бесконтактным, бескровным и точным. Способ позволяет получить пролонгирование беременности после операции - от 2 до 11,5 недель. Пример 1. Пациентка С, 22 лет, поступила в плановом порядке по направлению женской консультации по месту жительства с диагнозом «Беременность 20 недель. Двойня монохориальная диамниатическая. Селективная задержка развития I плода. Отягощенный акушерско-гинекологичесий анамнез (медицинский аборт, эрозия шейки матки). Хронический гастрит, вне обострения». Из анамнеза известно: вторая беременность, первые по роды. Данная беременность наступила сразу, прегравидарная подготовка не проводилась. На диспансерном учете с 9 недель. Течение I триместра без особенностей, первый УЗИ скрининг - двойня монохориальная диамниотическая. 17-18 недель - признаки синдрома фето-фетальной трансфузии, по данным ультразвукового исследования: селективная задержка развития I плода, маловодие у I плода (вертикальный карман 1,3 см), многоводие у II плода (вертикальный карман 9,5 см), кардиомегалия у II плода. Пациентка была направлена на консультативный прием в НИИ ОММ г. Екатеринбург для уточнения внутриутробного состояния плодов с дальнейшим решением вопроса о необходимости применения фетальной хирургии. Пациентке С.после коллегиального обсуждения было решено провести септотомию механическим путем с последующей амниоредукцией. Во время операции возникли трудности с перфорацией амниотической мембраны. Послеоперационный период без особенностей. Коррекция СФФТ была достигнута. В сроке 32 недель пациентку С. родоразрешили путем операции кесарево сечение. Родились девочки, первая 500 г с рождения на ИВЛ (не выжила), вторая массой 1500 г. Девочка здорова, развивается по возрасту. Настоящая беременность наступила сразу, прегравидарная подготовка проводилась (калий йодид 100 мг в сутки, фолиевая кислота 400 мг в сутки). На диспансерном учете с 8 недель. В I триместре был эпизод ОРВИ без подъема температуры тела, лечилась симптоматически. В сроке 15-16 недель находилась в стационаре с диагнозом - угроза прерывания беременности (жалобы при поступлении на мажущие выделения из половых путей). По данным ультразвукового исследования в сроке 19-20 недель выявлен СФФТ (многоводие у I плода с вертикальным карманом до 8,7 см, маловодие у II плода с вертикальным карманом до 1,0 см; селективной задержкой развитии II плода). Пациентка Р. была госпитализирована в отделение патологии НИИ ОММ для дальнейшего решения вопроса о вмешательстве: внутриутробная хирургия, септотомия при помощи лазера. На этапах оперативного вмешательства причин и трудностей в перфорации амниотической мембраны выявлено не было. Синдром фето-фетальной трансфузии был скорректирован. Пролонгирование беременности до 33 недель. В данном сроке была произведена операция кесарево сечение. Первый мальчик весом 1300 г, второй - 1050 г (здоровы и развиваются соответственно возрасту). При осмотре последа эпитализации отверстий в амниотической мембране не было выявлено.Example 2. According to the data of ultrasound, 19–20 weeks revealed SFTT (polyhydramnios in the first fetus with a vertical pocket up to 8.7 cm, oligohydramnios in the second fetus with a vertical pocket up to 1.0 cm; selective retardation of the development of the second fetus). Patient R. was hospitalized in the pathology department of the Research Institute of OMM to further resolve the issue of intervention: intrauterine surgery, laser septotomy. At the stages of surgical intervention, causes and difficulties in perforation of the amniotic membrane were not identified. Feto-fetal transfusion syndrome has been adjusted. Prolongation of pregnancy up to 33 weeks. In this period, a cesarean section was performed. The first boy weighs 1300 g, the second - 1050 g (healthy and develop according to age). When examining the subsequent epithelialization of holes in the amniotic membrane was not revealed.
Заявляемый способ решает проблему проявлений СФФТ. Во всех случаях первоначально произошло перераспределение околоплодных вод и возобновилась подвижность плодов-доноров. При этом проблем с перфорацией амниотической мембраны и последующей эпителизацией отверстий не наблюдалось. Пролонгирование беременности после операции - от 2 до 11,5 недель. Родоразрешение происходило в сроке беременности от 26 недель до 34 недель. Эффективность составила 62%.The inventive method solves the problem of the manifestations of SFT.
