В настоящее время в стране сохраняется сложная ситуация по туберкулезу, эффективность химиотерапии не превышает 70%, не излеченные больные продолжают выделять микобактерии туберкулеза, представляя эпидемическую угрозу обществу. В лечении таких пациентов особое значение приобретают хирургические методы. Наиболее эффективными являются резекционные вмешательства, позволяющие радикально добиться закрытия полостей распада и прекращения бактериовыделения у больных туберкулезом, в том числе с двусторонней локализацией патологических изменений.Currently, the country faces a difficult situation with tuberculosis, the effectiveness of chemotherapy does not exceed 70%, untreated patients continue to excrete tuberculosis mycobacteria, posing an epidemic threat to society. Способ осуществляют следующим образом. Операцию производят под общей анестезией с искусственной вентиляцией легких в условиях миоплегии. На стороне большего поражения в положении больного лежа на противоположном боку с ротацией кзади выполняют миниторакотомию по ходу VI межреберья без рассечения широчайшей мышцы спины длиной до 5 см и установкой торакопорта во II межреберье по средне-ключичной линии с введением видеоторакоскопа. Под видеоконтролем первым этапом производят пневмолиз и запланированный объем хирургического пособия с раздельной обработкой элементов корня легкого. Перед вторым этапом операции выполняют тщательный гемо- и аэростаз. Далее резецированное легкое отводят кзади для максимального освобождения пространства в проекции переднего средостения. Производят разрез медиастинальной плевры и клетчатки параллельно диафрагмальному нерву на протяжении 5-7 см для вскрытия контрлатеральной плевральной полости. В IV-V межреберье с контрлатеральной стороны по среднеключичной линии устанавливают торакопорт для видеассистентторакоскопии, позволяющей выполнить ревизию противоположной плевральной полости, выявить плевральные сращения и патологический фокус. Пневмолиз выполняют из медиастинального доступа под видеоконтролем, при необходимости через торакопорт, устанавливаемый в IV-V межреберье. После мобилизации легкое выводят через медиастинальный доступ в противоположную плевральную полость, пальпаторно оценивают объем операции и выполняют атипичную сегментарную резекцию в пределах здоровой легочной ткани. В конце операции после заключительного гемостаза обе плевральные полости дренируют силиконовыми трубками, медиастинальный доступ не зашивают. На кожные раны накладывают швы по обычной методике. Дренажи удаляют через несколько дней при суточной экссудации 50-100 мл.The method is as follows. Под оптическим контролем произведен пневмолиз справа, гемостаз по ходу операции. Частично иссечена париетальная плевра с мелкими, белесоватыми, пррсовидными образованиями до 2 мм в диаметре. При дальнейшей ревизии в верхней доле на фоне фиброза определяется полость до 3,0 см в диаметре и множественные очаги отсева по границе верхней доли. Принято решение выполнить резекцию верхней доли правого легкого. Выделен и обработан верхнедолевой бронх, ушит с помощью аппарата УО-30, отсечен, остальные элементы верхней доли прошиты с помощью аппарата СУ-80. Механические швы укреплены отдельными капроновыми швами. Контроль гемостаза, аэростаза. При дальнейшей ревизии в легком патологических образований нет. Разрушена легочная связка, гемостаз. Легкое полностью выполняет объем гемиторакса. Далее, через переднее средостение произведен доступ в контрлатеральному легкому и под контролем пальца в V межреберье по среднеключичной линии слева установлен торакопорт для видеоторакоскопии. Destroyed pulmonary ligament, hemostasis. The lung fully fulfills the volume of hemithorax. Further, through the anterior mediastinum, access was made to the contralateral lung and, under the control of a finger, a thoracoport for video thoracoscopy was installed on the left midclavicular line along the midclavicular line. During the audit of the left pleural cavity, single pleural fusion of the apex of the lung with the dome of the pleura was found, in S1-2 a dense formation of up to 2.5 cm is determined and in S6 a dense formation of up to 2 cm in diameter. The wound is sutured. The duration of the operation was 2.5 hours, blood loss 410.0 ml. The operation was completed by installing drainages in the right and left pleural cavity, with aspiration according to Bulau. Drainages removed on the 5th day.

Послеоперационный период протекал гладко. Послеоперационные раны зажили первичным натяжением без признаков воспаления. Рентгенологически (рис. 3.) оба оперированных легких полностью выполняют объем гемиторакса. Больной в удовлетворительном состоянии был выписан на 65 сутки после операции (рис. 4.).The postoperative period was uneventful. Способ лечения двустороннего туберкулеза легких путем применения одномоментной двусторонней резекции легкого из одностороннего межреберно-средостенного доступа, отличающейся тем, что эта операция выполняется из миниторакотомного переднебокового доступа 5 см без рассечения широчайшей мышцы спины, производится дополнительная установка видеоторакоскопа для визуализации обеих плевральных полостей был применен 91 больному двусторонним туберкулезом легких с наличием показаний к резекции легкого с обеих сторон.The results of our own research. На стороне большего поражения перед операцией диагностировался фиброзно-кавернозный туберкулез у 36 (39,6%) больных. У всех остальных пациентов были туберкулемы средних и крупных размеров с подтвержденной рентгенологически (МСКТ) фазой распада. Верхнедолевая локализация очага поражения легочной ткани наблюдалась у 66 (72,5%) больных.On the side of a larger lesion, fibro-cavernous tuberculosis was diagnosed before surgery in 36 (39.6%) patients. Интраоперационные осложнения при выполнении двусторонней резекции легких из одностороннего межреберно-средостенного доступа допущены не были. Осложнения в послеоперационном периоде возникли у 4 (4,4%) человек (у 2 больных возникло кровотечение, у 2 - глубокое нагноение послеоперационной раны). Все осложнения были ликвидированы при реторакотомиях по поводу кровотечений и путем перевязок с наложением вторичных швов при нагноениях ран.Intraoperative blood loss of less than 300 ml was observed in the majority - 88 (96.7%) patients. Применение двусторонних одномоментных резекций из одностороннего межреберно-средостенного доступа с ВАТС позволило ликвидировать полости распада у всех больных основной группы и абациллировать все 40 (100%) пациентов с сохраняющимся бактериовы дел ением.The results of treatment were evaluated 90-180 days after the operations. Способ лечения двустороннего туберкулеза легких путем применения одномоментной двусторонней резекции легкого из одностороннего межреберно-средостенного доступа, отличающейся тем, что двустороннюю резекцию легкого выполняют со стороны большего по объему поражения из переднебокового миниторакотомного доступа в VI межреберье размером до 5 см без рассечения широчайшей мышцы спины с установкой ранорасширителя и установкой торакопорта во II межреберье по средне-ключичной линии с введением видеоторакоскопа, со стороны меньшего поражения - из медиастинального доступа через переднее средостение с установкой торакопорта в V межреберье по средне-ключичной линии с введением видеоторакоскопа позволяет минимизировать травматичность вмешательства, интраоперационную кровопотерю, частоту послеоперационных осложнений и, тем самым, повысить эффективность лечения больных двусторонним туберкулезом легких.Conclusion A method for the treatment of bilateral pulmonary tuberculosis by applying simultaneous bilateral lung resection from unilateral intercostal-mediastinal access, characterized in that bilateral lung resection is performed from the side of a larger lesion from the anterolateral minicarotomic access in the VI intercostal space up to 5 cm in size without dissection of the latissimus dorsi with installation retractor and the installation of a thoracoport in the II intercostal space along the mid-clavicular line with the introduction of a video thoracoscope, from the side of lesser damage - from mediastinal access through the anterior mediastinum with the installation of a thoracoport in the V intercostal space along the mid-clavicular line with the introduction of a video thoracoscope, it minimizes the invasiveness of the intervention, intraoperative blood loss, the frequency of postoperative complications and, thereby, increases the effectiveness of treatment of patients with bilateral pulmonary tuberculosis.
