Нарушения координированной работы желчного пузыря и сфинктерного аппарата лежат в основе дисфункции билиарного тракта и являются причиной формирования клинической симптоматики. Дискоординация в работе желчного пузыря и сфинктерного аппарата билиарного тракта является одной из причин формирования билиарной недостаточности и нарушения желчеотделения, обусловленного функциональными нарушениями билиарного тракта, что приводит к несвоевременному и недостаточному поступлению желчи в двенадцатиперстную кишку. Учитывая физиологическое значение желчи для пищеварения, в основном, ее участие в липолизе, недостаточное поступление желчи приводит к развитию синдрома мальдигестии. В регуляции двигательной активности билиарной системы принимают участие парасимпатический и симпатический отделы вегетативной нервной системы, эндокринная система. Усиление активности симпатической системы способствует расслаблению желчного пузыря, а доминирующее влияние парасимпатической системы вызывает его спастические сокращения с последующей задержкой эвакуации желчи. Снижение сократительной функции желчного пузыря может быть обусловлено снижением чувствительности рецепторного аппарата к нейрогуморальной стимуляции. Функциональное состояние билиарного тракта во многом обусловлено активностью гастроинтестинальных гормонов: холецистокининапанкреозимина, гастрина, секретина, мотилина, глюкагона. Среди гастроинтестинальных гормонов наиболее сильный эффект оказывает холецистокининпанкреозимин, который, наряду с сокращением желчного пузыря, способствует расслаблению сфинктера Одди. На сократительную функцию желчного пузыря и сфинктера определенное влияние оказывает нарушение равновесия между продукцией холецистокинина, секретина, других нейропептидов. Причиной развития дисфункциональных расстройств являются чаще всего анатомические особенности: перегибы пузыря или наличие перетяжек внутри него. Среди многих причин вторичной билиарной дисфункции можно назвать генетически детерминированное заболевание целиакию. Это заболевание характеризуется поражением желудочно-кишечного тракта, обусловленное отсутствием пептидных ферментов, расщепляющих белки злаковых культур. Целиакия часто сопровождается функциональными нарушением функции желчного пузыря на фоне различных врожденных его аномалий. При данной патологии иммунопатологические процессы, развивающиеся в слизистой оболочке тонкой кишки в ответ на поступление антигенов в виде нерасщепленных пептидов некоторых злаков (пшеницы, ржи, овса, ячменя), приводят к атрофическим процессам энтероцитов со снижением высоты ворсинок и углублением крипт, выраженной лимфоцитарной инфильтрации и снижению активности гастроинтестинальных гормонов. При повреждении структуры слизистой оболочки тонкой кишки развиваются процессы нарушение всасывания в тонкой кишке. Among the many causes of secondary biliary dysfunction can be called a genetically determined disease of celiac disease. This disease is characterized by damage to the gastrointestinal tract, due to the absence of peptide enzymes that break down proteins of cereal crops. Celiac disease is often accompanied by functional dysfunction of the gallbladder on the background of its various congenital anomalies. In this pathology, the immunopathological processes developing in the mucous membrane of the small intestine in response to the receipt of antigens in the form of unsplit peptides of certain cereals (wheat, rye, oats, barley) lead to atrophic processes of enterocytes with a decrease in the height of the villi and deepening of the crypts, marked lymphocytic infiltration and decreased activity of gastrointestinal hormones. If the structure of the mucous membrane of the small intestine is damaged, processes of malabsorption in the small intestine develop. Absorption deficiency syndrome in celiac disease is exacerbated by concomitant pathology of the hepatobiliary system with impaired outflow of bile into the duodenum [1].

В качестве прототипа по достигаемому эффекту нами выбран способ лечения больных целиакией с желчнокаменной болезнью (ЖКБ) 1-ой стадии на фоне аномалии желчного пузыря с использованием препаратов урсодезоксихолевой кислоты на фоне патогенетической диетотерапии. Как видно из представленных данных, результаты лечения дисфункционального расстройства желчного пузыря билиарного сладжа у больных целиакией с применением остеопатического воздействия и УДХК сравнимо по эффективности, однако УДХК не влияет на сократительную функцию желчного пузыря, а использование комплексного остеопатического лечения повышает контрактильную способность желчного пузыря, уменьшает застой и улучшает отток желчи.
