Abstract
Description
The invention relates to medicine, namely to surgery of childhood and pediatric otorhinolaryngology.
There is a method of eliminating stenosis of the sub-vocal part of the larynx with the staging of a costal autograft in the anterior wall of the larynx and subsequent stenting and various ways of uncoupling of the vocal folds with inter-voice stenosis of the larynx [Pavlov P.V. Optimization of surgical tactics for chronic laryngeal stenosis in children: Abstract. dis. Dr. med. sciences. St. Petersburg 2010]. However, all these methods are performed in several stages, and the treatment takes about 3-4 months.
[Sandu K. et al. Combined endoscopic and open approach in treating congenital laryngeal web //International Journal of Pediatric Otorhinolaryngology Extra. - 2015. - T. 10. - №. 2. - C. 22-24.] Closest to the claimed is a method of simultaneous dissection of the inter-voice membrane of the true vocal folds with congenital inter-voice and sub-voice stenosis [Sandu K. et al. Combined endoscopic and open approach in treating congenital laryngeal web // International Journal of Pediatric Otorhinolaryngology Extra. - 2015. - T. 10. - No. 2. - C. 22-24.] And plastics of the sub-vocal laryngeal stenosis using the anterior autologous graft cartilage graft and subsequent stenting for 4 weeks.
The disadvantage of the method selected as a prototype is the installation of a rib autograft under the anterior commissure, which does not provide sufficient isolation of the wound surfaces after dissection of the inter-voice membrane and, thereby, increases the risk of restenosis of this area.
In addition, stenting suggests the presence of a tracheostomy in a patient, and therefore, the method cannot be applied in previously non-tracheostomy patients.
Also, when stenting in 71% of cases, granulations are formed in the area of the junction of the stent and tracheostomy cannula, which requires additional surgical intervention and therefore lengthens the treatment time.
The objective of the invention is to create conditions for the early restoration of breathing through the natural airways while reducing treatment time.
The technical result of the proposed invention, the result is achieved by the fact that in the method of laryngotracheoplasty with combined inter-voice and sub-voice stenosis of the larynx, consisting in endoscopic dissection of the membrane, the cricoid cartilage arc and thyroid cartilage with external access, followed by the installation of the endoprosthesis and the installation of the costal autograft transplant on the anterior wall at the level of the front commissure.
When a rib autograft is placed in the anterior wall of the larynx at the level of the anterior commissure, we achieve separation of the previously contacting wound surfaces of the true vocal folds. The installation of an endotracheal tube as a stent allows this operation to be performed in previously not tracheostomized children without its application. And in the case of a tracheostomy, decanulation is performed at the time of surgery, which significantly reduces the treatment time.
The principal difference of the proposed method is the setting of the rib autograft at the level of the anterior commissure, and not at the level of the cricoid cartilage arc, which allows the use of an endotracheal tube and stenting time, since the lumen is formed due to the engraftment of the cartilage, and not due to the formation of scar tissue .
Example. Patient Zh., 2 years old, was admitted on September 16, 2017 to SPbSPMU with complaints of persistent hoarseness from birth, difficulty breathing through the natural airways with minimal physical exertion. With fibrolaryngoscopy, an inter-voice membrane was found occupying 3/4 of the length of the vocal folds and extending to the sub-vocal section of the larynx with a narrowing of its lumen to the opening of the 3 mm passage for the endotracheal tube.
Given the combination of laryngeal pathology and damage to the voice and sub-vocal sections, it was decided to conduct a one-stage laryngotracheoplasty with combined access with a costal autograft placed on the front wall of the larynx at the level of the anterior commissure and subsequent intubation. The patient was transferred to the intensive care unit for further observation. Extubation was performed on the 14th day. The breath is satisfactory, there are no signs of stenosis. With delayed follow-up for 6 months, there are no signs of restenosis in the area of true vocal folds.
Thus, the inventive method allowed to eliminate sub-vocal stenosis of the larynx and simultaneously split the true vocal folds in the anterior commissure, thereby preventing the development of relapse. The hospitalization time is 21 days, which is significantly less than with multi-stage plastics. In addition, tracheostomy was avoided, which also reduced treatment time and the risks of possible complications.
The inventive method allows you to restore breathing through the natural airways and reduce treatment time due to one-stage elimination of inter-voice and sub-voice stenosis and a shorter period of stenting.
